AGREEMENT FOR SPECIAL SERVICES AMENDMENT

BY AGREEMENT of both parties this 28
th
day of April, 2014, the Agreement for Special Services between the
State of Maine, Department of Health and Human Services, hereinafter called Department, and The
Alexander Group Inc., hereinafter called Provider, is hereby amended as follows:


1. The termination date is extended from 5/15/14 to 7/15/14.

Reason: Agreement is being extended in order to incorporate the revised timeline for the final
deliverable.

2. The Scope of Services, Deliverables Summary in Rider A is amended in order to extend
Deliverable 4 System-wide program integrity review and action plan due date to 7/15/14. The Scope
of Services, Payment Schedule, Deliverable Schedule in Rider A target due date for Deliverable 4 is
extended to 7/15/14 along with the payment amount of $100,000.

All other terms and conditions of the original Agreement dated 9/16/13 remain in full force and effect.
WITNESSETH, that this Agreement is consistent with Executive Order 01 FY 11/12 or a superseding
Executive Order, and complies with its requirements.

IN WITNESS WHEREOF, the Department and Provider, by their duly authorized representatives, have
executed this amendment as of the day and year first above written.
