As people a+e/ stomach acid often decreases/ which impairs the di+estion and absorption of vitamin !"12 from animal foods. Crystalline vitamin !"12 from fortified cereals and supplemental nutritional formulas protect a+ainst !"12 deficiency in older adults &Ra9an et al./ 22220 ?arcia et al./ 2222(. Crystalline vitamin !"12 is easier for the body to absorb/ because it does not re;uire di+estion in the stomach. itamin !"12 is ti+htly bound to the protein in animal foods such as beef/ por*/ poultry/ fish/ e++s/ and dairy foods. 8tomach acid is re;uired to di+est and separate the vitamin !"12 from the protein in animal foods. 5any older adults have low stomach acid because they have atrophic +astritis &which causes the stomach to decrease acid production( or they re+ularly ta*e antacids or acid bloc*ers that lower stomach acid. About 321 of older adults have atrophic +astritis/ and a lar+e number of older adults also ta*e antacids. =here is some evidence that other medications that decrease transit time &laEatives( or dama+e the intestine &anti" inflammatory a+ents( may also increase the ris* of vitamin !"12 deficiency.

A less common cause of vitamin !"12 deficiency is the loss of intrinsic factor. Intrinsic factor is produced by the stomach where it binds vitamin !"12 and carries it to the intestine for absorption. =his type of vitamin !"12 deficiency is called pernicious anemia. >nli*e other !"vitamins/ such as folic acid/ vitamin !"12 is added to only a few foods. 4ame brand brea*fast cereals usually/ but not always/ have vitamin !"12 added. ?eneric or store brand brea*fast cereals usually do not contain vitamin !"12. 5anufacturers are be+innin+ to add vitamin !"12 to more of their brea*fast cereals. Dlder adults who do not consume a brea*fast cereal fortified with vitamin !"12 should ta*e a supplement that contains vitamin !"12. 5ulti" vitamins formulated for older adults are a +ood choice for a supplement. 3espite the hi+h prevalence and serious conse;uences/ older adults are not routinely screened for vitamin !"12 deficiency. =here are no public health policies/ such as there are for hi+h cholesterol or hi+h blood su+ar/ that recommend routine screenin+ for vitamin !"12 deficiency. >nfortunately/ most people are dia+nosed with vitamin !"12 deficiency after they develop the symptoms. =herefore/ prevention of vitamin !"12 deficiency is vitally important. Dfficially/ the Recommended 3ietary Allowance for vitamin !"12 is 2.) micro+rams &mc+( per day. =he 3aily alue for vitamin !"12 is 6 mc+Kday. #owever/ some studies have shown that 12 mc+ or more of vitamin !"12 daily is needed to prevent vitamin !"12 deficiency in older adults. Althou+h currently there are no scientific studies that document the amount of vitamin !" 12 that will prevent vitamin !"12 deficiency in the elderly/ it may be prudent to consume between 12 and 2' mc+Kday &16G1 to )1G1 of the 3aily alue(. =hese amounts may sound eEcessive. #owever/ vitamin !"12 is not toEic/ there is no >pper :evel/ and vitamin !"12 can be safely consumed in amounts up to at least 2/222 mc+ each day &2 m+(. =here is concern about the use of many dietary supplements. #owever/ older adults who do not consume vitamin !"12 from fortified foods such as brea*fast cereals should use vitamin !"12 supplements. It is actually cheaper to consume vitamin !"12 from a supplement than from a fortified cereal. %or eEample/ a daily dose of 6 mc+ of vitamin !"12 &1221 of the 3aily alue( from a multi" vitamin"mineral supplement costs about 6 to 12 cents/ while a daily servin+ of brea*fast cereal costs about 1' to 33 cents.
