RECENT INCIDENTS 24 February 2008 Restricted Work Day Case When an employee stepped back off the framework of a fan, he stepped on an unused scaffold base plate resulting in a sprain to the ankle. Lessons Learnt Employees must be responsible for ensuring their work surface is clear of any tripping hazards. Dont wait to be told to carry out housekeeping duties. If you see something that could cause harm to you or other employees, either fix it or report it to your supervisor. Employees must always use a safe work platform. If you cannot reach the area you need to work on, use a ladder or other approved work platform. 26 February 2008 Near Miss A scaffold tube fell down a staircase, rebounded off the landing and fell out of the building to the ground. Lessons Learnt When shifting equipment material from one area to the other, supervisors must plan the work and ensure the pathway for the moving of the material is the safest way. If shortcuts and haste become part of employees work behaviour, risks are increased and injuries can occur.
Recently there have been a several incidents where employees were undertaking tasks they had done many times before. When the incident occurred, they did not follow the procedure they had used before, which resulted in injuries to employees. Employees must discuss the steps to complete their work tasks as well as the hazards associated with the task at the STARRT meeting. They must follow the planned steps, or discuss any change with their supervisor.
