Critical Care Clinical Case Study NURS 451- Clinical Management of Adult Health Nursing III Purpose of Assignment

The purpose of this assignment is to utilize the Health Planning Model to improve aggregate health and to apply the nursing process to the larger aggregate within a systems framework. Student Approach to Assignment This paper is a reflection of nursing diagnoses and assessments of a specific population in the community. The residents of Barrett House Transitional Home were assessed, and interventions planned to address the findings. Several hardships were experienced in the process, and this papers serves to summarize those hardships and the solutions to them. Reason for Inclusion of the Assignment in the Portfolio This assignment is included in the portfolio because it highlights the care that I gave to a complex patient and demonstrates integration of a wide variety of end of program behaviors. Leadership Assumes a leadership role within ones scope of practice as a designer, manager, and coordinator of health care to meet the special needs of vulnerable populations in a variety of practice settings In this paper, I took on a leadership role at various times in the community health group. One such time was during our yard beautification for Barrett House. This intervention was a coordinated effort to clean up the grounds of Barrett House to increase the motivation and sense of self-worth in the women of the house. During this evolution, we had to make do with limited tools and equipment available. I took the initiative to organize and delegate the group into working parties which allowed for the women to be dispersed amongst the groups and not grouped by themselves. This was coordination of the team to meet our intended goal of a vulnerable population. There were also many planning meetings where further interventions were discussed and planned, giving the opportunity to design and manage interventions in the team meeting setting. Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals This paper describes the efforts made to create a partnership with the women of Barrett House. To do this, we did a small number of group projects, including the mentioned yard work. Once our foot was in the door, we broke up and went one-onone with the women, staggering our times at the house and getting on a more personal level. Throughout, we met and assessed the women, planning our interventions and

the effect desired. Through all this, we initiated the partnership, established goals and planned out how we would meet them. Organizes, manages, and evaluates the development of strategies to promote healthy communities As already mentioned, this paper outlines our planning meetings and strategies to reach our desired goals with the women of Barrett House. Specifically, we wanted to increase the motivation and sense of self-worth in the women shortcomings we noticed in the women during our evaluation. Between visits to the house, the strategies and interventions were planned that would bring the goal closer to completion. Delegates and supervises the nursing care given by others while retaining the accountability for the quality of care given to the patient This paper describes how I delegated tasks, but retained accountability through the planning sessions and yard work project. During the yard work project, I organized the team into working parties and delegated the various tasks needed to be completed. Professionalism Demonstrates accountability for ones own professional practice. This paper outlines my accountability of professional practice by the required interactions to gain the trust of Barrett House. We describe various difficulties getting the women to be as excited to work together as we were, and parts of our progress were vulnerable at times. This meant that any unprofessional behaviors could spell disaster for our goals. By maintaining professionalism and conducting myself as a nurse, with the knowledge and skills expected of one, we were able to make great strides. Not only does professional practice guide our professional appearance and actions, but it demands a certain quality of work. With Barrett House, the paper shows that our efforts were with the highest motivation, and we were frequently asked medical related questions by the women. By having professional practice standards, we provided accurate and digestible information for the women which is reflected by our success in establishing a partnership with the women. Culture Maintains an awareness of global environmental factors that may influence the delivery of health care services In our time with Barrett House, we assessed the surrounding areas during a windshield survey. The purpose of this survey was to evaluate how the area would affect both the women, and our likelihood of success. In this case, we noted that the only sources of food within walking distance of the house were fast food restaurant chains. This meant that our teaching of nutrition and proper eating habits would be difficult to adhere to with limited resources for such choices.
