GRAVE EMERGENCY SHUTDOWN

4.1 Grave Emergency Shutdown Procedure for Urut-1


If the Urea/Utilities/PM&S or Ammonia-2/Urea-2 Shift Coordinator has decided that the
Emergency is of Level-II, plant will have to be shutdown as quickly as possible ensuring
safety of personnel and equipment. The following actions should be taken on hearing LevelII emergency alarm:
4.1.1

CO2 Compressor Operator

1. Puts on Scott Air Pak.


2. Trip one CO2 compressor immediately and 2nd compressor after 15 seconds.
3. Close 5th stage discharge plug cock valve and open 5th stage vent valves (both C2A&B).
4. Close 1st stage suction CO2 valves (both C-2A&B).
5. Closes 6 dia. natural gas line valve to Ammonia-2 upstream of PRC-2/3.
6. Stops E-8 fans.
7. Stop P-6.
8. Informs Boardman or Supervisor and vacates the area. Rush towards PM&S-3 Gate or
Plant Gate/Air Port Gate depending upon wind direction equipped with Scott Air Pak.
4.1.2

Urea-I Boardman

1. Puts on Scott Air Pak.


2. Stop control room blower.
3. Close TRC-102.
4. Check that CO2 compressors have been tripped.
5. Shut off P-101A/B/C, P-102A/B/C and P-162A/B from panel.
6. Leave PRC-105V on auto control.

7. Close FRC-101 on manual control.


8. Close TRC-122/124 and TRC-145.
9. Close FRC-165.
10. Full open PRC-141.
11. Ensure that PIC-162, TIC-168 and TRC-165 are open.
12. Cracks open RCV-180 (5 # signal).
13. Close TIC-147D.
14. Close LRC-167V on manual control.
15. Rush towards PM&S-3 gate or Plant Gate/Air Port Gate depending upon wind direction
equipped with Scott Air Pak.
4.1.3

Urea-1 Synthesis Operator (Emergency Squad Member)

1. Puts on Scott Air Pak.


2. Checks wind direction. Puts on Scott Air Pak attached to the Urea-1 north wall.
3. Contacts Urea-1 control room for the safest route to the assembly point for emergency
squad as communicated by respective Shift Coordinator.
4. Trips P-161s, P-163s and P-165s if not already done by recovery and decomposition
operator.
5. Reaches the assembly location following the safest route and follows instructions of the
Shift Coordinator in handling the emergency.
4.1.4

Urea Recovery & Decomposition Operator

1. Puts on Scott Air Pak.


2. Close FRC-101 block valve.
3. Shut off P-144C or D.
4. Open drain of melt line to TK-161.
5. If time permits, take as many of the following steps as possible.
6. Close CO2 block valve to R-101.
7. Switch off:

P-164s.

Inform Boardman of actions taken and vacates area.


Rush towards PM&S-3 gate or Plant gate/Air Port gate depending on wind direction,
equipped with Scott Air Pak.
4.1.5

Urea-1 Finishing Post Operator

1. Puts on Scott Air Pak.


2. Shut off P-144A/B.
3. Open drain of PT-1 melt line to TK-161.
4. Switch off P-141A/B.
5. Inform Boardman of actions taken and vacates area.
6. Rush towards PM&S-3 gate or Plant gate/Air Port gate depending on wind direction,
equipped with Scott Air Pak.
4.1.6

G. T. Operator

1. Puts on Scott Air Pak.


2. Leave all GTs and C-702 as such.
3. Ensure that C-701 A/B have started on auto cut-in.
4. Drop trip levers of P-601B and C.
5. Switch off P-601A & D.
6. Inform Control Room and rush to PM&S-3 Gate or Plant Gate/Air Port Gate depending
on wind direction equipped with Scott Air Pak

Boiler Operator 1st Man

4.1.7

1. Puts on Scott Air Pak.


2. Start P-401.
3. Bypass LLCO-651/661/691.
4. Close PRC-604V on manual control.
5. Close LIC-421V from DCS. Give signal of 100%.
6. Open PIC-651/661/691 by 25-30% on manual control.
7. Controllers of operating boilers should be on auto control.
8. Inform control room and rush to PM&S-3 gate or Plant gate/Air port gate depending on
wind direction equipped with scott air pak.
Boiler Operator 2nd Man

4.1.8

1. Puts on Scott Air Pak.


2. Stop phosphate & hydrazine pumps.
3. Trip auxiliary firing on HRSG-651/691.
4. Close down stream isolating valves of:

Main fuel gas.

Purge gas.

Pilot gas.

5. Inform control room and rush to PM&S-3 gate or plant/air port gate depending on wind
direction equipped with Scott Air Pak.
4.1.9

Water Treatment Operator (Emergency Squad Member)

1. Puts on Scott Air Pak.


2. Stop P-423A/B
3. Shutdown all acid/caustic pumps P-423A/B, P-508A/B, P-510A/B.
4. Start P-401.
5. Ensures that P-723 A or B is running and the standby pump of the two is on auto cut-in
6. Start P-721.

7. Start P-722. and adjusts RPMs.


8. Adjust fire water pressure between 110-120 psig.
9. Open make up water to TK-701.
10. Check from Urea-I C/R the assembly point of emergency squad, puts on Scott Air Pak
and approaches to assembly point following safest route
4.1.10 Lime Softening Operator (Emergency Squad Member)
1. Puts on Scott Air Pak.
2. Switch off P-415A/B and P-416A/B.
3. Starts P-403 and adjust RPMs to 1200 - 1300.
4. Start P-401.
5. Checks wind direction and report to the PM&S supervisor at PM&S-2.
6. Accompanies the PM&S supervisor/squad members to the emergency site following the
safest route.
7. Follows instructions of the Emergency squad leader in handling the emergency.

4.2 Grave Emergency Shutdown Procedure for Ammonia-2 Unit


4.2.1

Front - End Boardman

1. Puts on Scott Air Pak.


2. Actuate I-1 from ESD
3. Actuate I-6 from ESD
4. Trip KGT - 2501 from 5R on Control room panel.
5. and Trip KGT-2506 from itstheir panels
6. Stop middle I. D. Fan.
7. Stop P-2504 & A. Ensure that catacarb circulation has stopped. (P-2503's will also trip
due to I-7 actuation).
8. Isolate LTS (close MOV-504, 505).

9. Stop P-2515C/D from control room panel.


10. Stop P-2504 & A. Ensure that catacarb circulation has stopped. (P-2503's will also trip
due to I-7 actuation) (if step 11 has not been carried out).
11. Call Group # 1 at TDC and Close FRC-104/105, FRC-109, PRC-101, PRC-319, FRC801V, PRC-805V, PRC-306V on Manual.
12. Call Group # 2 at TDC and give set point of 30 psig to PIC-102, PIC-103, PIC-131.
13. Call Group #2 at TDC and Close PRC-725 and open PRC-1053Von Manual.
14. Open PRC-192A by 55% from TDC.
15. Trip MG-2501. sSet from SGR (ensure MOV-1010 has been closed by back end
Boardman).
16. Stop middle I. D. Fan (if step 11 has not been carried out).
17. Stop P-2504 & A. Ensure that catacarb circulation has stopped. (P-2503's will also trip
due to I-7 actuation) (if step 11 has not been carried out).
18. Stop Exhaust Fan's of Am-2 Control Room Tea Stall / Toilet.
19. Rush towards PM&S-3 Gate or Plant Gate/Air Port Gate depending upon wind direction
equipped with Scott Air Pak.
20. Puts on Scott Air Pak.
21. Actuate I-1 (PSL - 110) from ESD.
22. Actuate I-6 (LSL - 335).
23. Trip KGT - 2501.
24. Trip KGT-2506, NGBC.
25. Stop middle I. D. Fan.
26. Isolate LTS (close MOVs).
27. Stop P-2504/A & A. Ensure that catacarb circulation has stopped.
28. Close HIC-102/103, FRC-104/105, FRC-109, HIC-171, PRC-101V, PRC-319/AV, HIC1015, PRC-725V, PIC-1012 BV & LIC-331V.
29. Open PIC-102V, PIC-103V, PIC-131V & PIC-192B/CV, as per requirement.
30. Close LRC-126Vs on manual.
31. Closes FRC-801V and PRC-805V on Manual.
32. Coordinate with Reforming and /Catacarb/Ammonia-1 Operators to take appropriate
steps for grave emergency shutdown.

33. Rush towards PM&S-3 Gate, or Plant Gate or /Air Port Gate depending upon wind
direction equipped with Scott Air Pak.

Back - End Boardman

1. Puts on Scott Air Pak.


2. Stop Control Room AC's
3. Trip K-2503.
4. Close MOV-1010 and HIC-1011
5. Close RBV-200/A & RBV-2528
6. Trip P-2531.
7. PIC-132 set point change to 30 psig#.
8. Put FRC-147 on auto with set point of 500 KSCFH.
9. Call Group # 3 at TDC and Close HIC-141, 142, 143, 144, LRC-366, 368, 1017, 1018 on
Manual.
10. Call Group # 4 at TDC and Close LRC- 359, 374, 367, HIC-1018 on Manual.
11. Call Group # 5 at TDC and give 1500 psig set point to PIC-1033 and 15 psig set point
to , PIC-1027. ,
12. Open HIC-147 and Reduce Set point of PRC-152 upto 30 psig# ( keep it on Auto ).
13.
14. Rush towards PM&S-3 Gate or Plant Gate/Air Port Gate depending upon wind direction
equipped with Scott Air Pak.
15. Puts on Scott Air Pak.
16. Trip K-2502 & K-2503.
17. Trip P-200A/B/C and .
18. Close RBV-200/A.
19. Trip P-2509/A from panel and Close RBV-2528.
20. Isolate Converter (D-2525) by closing HIC-141V / 142V / 143V / 144V.
21. Crack open FRC-147V.
22. Crack open PIC-1033V and PIC-1027V.

23. Coordinate with Synthesis, /Compressor and/ Uty-2 Operators to take appropriate steps
for grave emergency shutdown.
24. Stop P-2531 from Control Room.
25. Rush towards PM&S-3 Gate, or Plant Gate or /Air Port Gate depending upon wind
direction equipped with Scott Air Pak.
4.2.3

Reforming Operator

1. Puts on Scott Air Pak & both operator will assemble near Reforming Lab. depending on
wind direction, then one operator will take Zone-1 Steps and other will take Zone-2 Steps
of following.
Zone-1
2. Close FRC-116V D/S isolating valve.
3. Switch Auto/Manual switch to Off & Stop TP-2501A by dropping lever
4. Isolate LRC-118 & LRC-122 upstream isolating valves.
5. Inform Shift Supervisor / Control Room, if possible and rush to PM&S-3 Gate or Plant
Gate/Air Port Gate depending on wind direction equipped with Scott Air Pak.
Zone-2
6. Close upstream isolating valves / bypasses (if open) of FRC-104/105V, FRC-109V, HIC170V D/S valve.
7. Inform Shift Supervisor / Control Room, if possible and rush to PM&S-3 Gate or Plant
Gate/Air Port Gate depending on wind direction equipped with Scott Air Pak.Puts on
Scott Air Pak.
8. Close upstream isolating valves / bypasses (if open) of FRC-104/105, FRC-116, FRC109, HIC-171, PRC-101V, PRC-319V & HIC-1015V.
9. Put MP-2501 selector switch to Stop Position. Stop TMP-2501A. (Also drop the trip level
of stand-by steam driven pump)
10. Trip KGT-2501 if not already tripped.
11. Close D-2505 and D-2511 continuous blow down valves.
12. Trip MG Set (MG-2501).
13. Isolate PRC-306V.
14. Inform Control Room and rush to PM&S-3 Gate or Plant Gate/Air Port Gate depending
on wind direction equipped with Scott Air Pak.

Catacarb Operator

1. Puts on Scott Air Pak & both operator will assemble near Catacarb Lab. depending on
wind direction, then one operator will take Zone-1 Steps and other will take Zone-2 Steps
of following.
Zone-1
2. Close ECPL gas line # 2 battery limit valve south of EM-2511
3. Crack open E-2507A vent valve.
4. Inform Shift Supervisor / Control Room, if possible and rush to PM&S-3 Gate or Plant
Gate/Air Port Gate depending on wind direction.
Zone-2
5. Trip TP-2515B.
6. Close TSH-129Von Hand jack.
7. Drop TP-2505A &TP-2508A T&T levers.
8. Inform Shift Supervisor / Control Room, if possible and rush to PM&S-3 Gate or Plant
Gate/Air Port Gate depending on wind direction.Puts on Scott Air Pak.
9. Ensure that catacarb circulation has been stopped by the Boardman from Control Room.
10. Stop P-2550/A, P-2515A-D, P-2505/A & P-2508/A.
11. Close HTS inlet valve, if possible.
12. Crack open 600# steam to all vent stacks.
13. Control PRC-2 pressures by adjusting local controller.
14. Ensures that P-200 A/B/C have been tripped.
15. Inform Control Room and rush to PM&S-3 Gate, Plant Gate or Air Port Gate depending
on wind direction equipped with Scott Air Pak.
4.2.5

Compressor Operator

1. Puts on Scott Air Pak.


2. Ensure TK-2502/2503 T&T valves are closed, if not then close both valves.

3. Open Syn. Compressor 2nd & 3rd inter stage 1" vents.
4. Open K-2503 2nd discharge 3" vent.
5. Trip seal oil pump of TP-2513A/TP-2514A by trip lever.
6. Isolate BFW to E-2524A/B.
7. Inform Shift Supervisor / Control Room, if possible and rush towards PM&S-3 Gate or
Plant Gate/Air Port Gate depending on wind direction equipped with Scott Air Pak.Puts
on Scott Air Pak.
8. Close TK-2502/2503 T&T valves.
9. Open recycle suction to 3rd discharge jump over valve.
10. Open Syn. Compressor 3rd discharge vent.
11. Close 1st suction block valve.
12. Isolate liquid Ammonia to Chillers.
13. Isolate BFW to E-2524A/B & depressurize.
14. Close E-2524A/B continuous blow down valves.
15. Trip seal oil pump.
16. Open K-2503 2nd discharge 3 vent.

Synthesis Operator (Emergency Squad Member)

Ammonia-2 Synthesis Operator is a member of the emergency squad. He will take following
steps and report to the squad leader as soon as possible:
1. Puts on Scott Air Pak.
2. Isolate LRC-366A/B D/S plug Cock valve.
3. Isolate LRC-368.
4. Checks from the control room/Supervisor the safest route and assembly point as
communicated by the respective Squad leader.
5. Inform Shift Supervisor / Control Room, if possible and reports to the squad leader
following safest route.Puts on Scott Air Pak.
6. Isolate LRC-366A/B.
7. Stop P-2547A.

9. Ensure that P-2509/A have been stopped.

Utility-2 Operator

1. Puts on Scott Air Pak.


2. Isolate Chlorine injection to CT-2.
3. Isolate LC-302 by pass valves if open (Make up Water of CT-2) near CT-2 Basin.
4. Trip P-801/A, P-802/A, P-803/A, P-25208B, P-25213B/C, P-2550/A, P-2528/A, EM2511's, EM-2509's from 50 Hz SGR thru cubicles.
5. Crack open 6" vent upstream of PRC-805V.
6. Put Demin. Trains off line from PLC and stop Acid & Caustic pump from Panel.
7. Isolate LC-302 by pass valve if open (Make up Water of CT-2) at control valve.
8. Inform Shift Supervisor / Control Room, if possible and rush to PM&S-3 Gate or Plant
Gate/Air Port Gate depending on wind direction equipped with Scott Air Pak.Puts on
Scott Air Pak.
9. Put Demin. trains off line.
10. Stop Demin. Water Pumps P-25213/A/B/C.
11. Stop CT-2 and CT-3 cooling tower No:2 and 3 fans.
12. Stop chemical injection pumps.
13. Stop Cooling Water Pumps P-25201/A/B/C/D/E/F.
14. Stop surface condenser pumps P-25202/A & P-25215/A.
15. Isolate LC-302 (Canal Water) and LC-324 (L/S Water).
16. Stop G-604, if on line.
17. Stop P-801/802/803.
18. Crack open 6 vent upstream of PRC-805V.

9. Trip SG-25681.
10. Open PIC-686V.
11. Isolate fuel/purge isolating valves.
12. Isolate BFW supply to the boiler.
13. Close the boiler continuous blow down valve.
14. Open Steam Drum vent valve.
15. Inform control room and rush to PM&S-3 Gate or Plant Gate/Air Port Gate depending
upon wind direction and equipped with Scott Air Pak.
4.2.9

Ammonia-1 Operator

1. Puts on Scott Air Pak.


2. Control PRC-2 pressures by adjusting local controller.
3. Ensures that P-200 A/B/C have been tripped.
4. Closes 6 dia natural gas valve to Ammonia-2 upstream of PRC-2/3.
5. Stops E-8 fans.

4.3 Grave Emergency Shutdown Procedure for Urea-2 Unit


All steps shall be taken in the same order of priority.
4.3.1

Front End Board man

1. Puts on Scott Air Pak.


2. Put PIC-1155 on auto at a set pressure of 215 psig.
3. Put PIC-1151 on auto.
4. Shut down ACES section by pressing PB-1 of ESD (Emergency Shut Down).
5. Ensure that CO2 compressors have been tripped.
6. Ensure that PIC-37 E is open.
7. Ensure that operating Uracas & Carbamate pumps are tripped from ESD switch.
8. Close FCV-201 A/B.
9. Reduce PIC-1754 set point to 50 psig.
10. Close HV-1100.
11. Close HIC-202 A/B.
12. Vacates the control room and reports to PM&S-3 Gate or Plant Gate/Airport Gate
depending on the wind direction.
4.3.2

Front End Board man

1. Puts on Scott Air Pak.


2. Stops Control Room exhaust fans, A/C blowers and pressurizing fan.
3. Trips P-262s from the control room, ensuring that Front End Boardman has pressed PB1.
4. Open RCV-280 by 10%.

4. Ensure that operating compressors have been stopped.
5. Ensures that working people have vacated the area.
6. Vacates the area & reports to PM&S-3 Gate or Plant Gate/Airport Gate depending on
wind direction.
4.3.4

ACES Operator (Emergency Squad Member)

1. Puts on Scott Air Pak.


2. Informs synthesis operator.
3. Seeks guidance from the control room on the assembly point for the emergency squad .
4. Report to the respective shift coordinator following the safe route as intimated by the
boardman.
5. Follows instructions of the squad leader.
