COURSE DETAIL 1. Period: 5 days 2. Training outcomes After completion of the course, the learners will be able to: o o o Understand the Layered Risk Management Approach; Understand the ISO 31 000 - 2009 Standard Requirements; Conduct Issue Based Risk Assessments by applying the following techniques: Hazard Study 1 Preliminary Hazard Analysis; Hazard Study 2 Major Plant Hazards; Hazard Study 3 HAZOP Studies; Project Risk Assessments; Bowtie Analysis and Fault Tree Analysis; SWIFT, Human Error Analysis and FMECA. 3. SAQA Unit Standards This training is aligned to Unit Standard 120330 - Conduct a continuous risk assessment in a workplace and Unit Standard 115101 - Address workplace hazards and risks and Unit Standard 24487 Conduct a base-line risk assessment and take corrective actions. 4. International Standards and Guidelines The training will also address the requirements and contents of ISO 31000-2009 Risk Management Standard as well as SIMRAC Practical Guide to the Risk Assessment Process and the Australian National Minerals Industry Safety and Health (NMISH) Risk Assessment Guideline. 5. Contents o Module 1: Layered Risk Management Process Level 1 Macro risk assessments and ERM Level 2 - Issue based risk assessments Level 3 Task/Activity based risk assessments Level 4 Continuous risk assessments o Module 2: International Standards and Guidelines ISO 31000 - 2009 SIMRAC practical guide to the risk assessment process NMISH risk assessment guideline o Module 3: Hazard Studies Hazard study 1 preliminary hazard review Identify hazards associated with the process Identify major environmental problems and assess suitability of proposed sites Criteria for hazards, authorities to be consulted, standards and regulations, COPs Collect/review information on previous hazardous incidents

Hazard study 2 major plant hazards Examine plant items and equipment on process flow sheet and identify significant hazards Identify areas where redesign is appropriate Assess plant design against relevant hazard criteria Prepare environmental impact assessment Hazard Study 3 HAZOP study Critical examination of plant operations also described as hazard and operability study (HAZOP) and described as FMECA, (failure mode, effect and criticality analysis) Guidewords for HAZOP study Hazard Study 4 plant and process design Reservation review verifying that the provisions in all previous studies are fully implemented Check that the operating instructions and emergency procedures comply with any requirements identified by previous hazard studies Hazard Study 5 pre-commissioning Inspection Safety health and environmental audit (SHE) of constructed plant before introducing hazardous materials Satisfy team that the detailed implementation of the project meets company and legislative requirements in cold and hot conditions Hazard Study 6 documentation and review Final review to confirm that design has been fulfilled opposite SHE aspects Compare early plant operational experience with assumptions made in hazard studies Ensure that all documentation is available and in place o Module 4: Project Risk Assessment Life cycle phases Concept phase risk assessments Design phase Construction and commissioning Operation and maintenance Decommission or disposal o Module 5: Bowtie Analysis and Swiss Cheese Model Swiss cheese model Prevention barriers Mitigation barriers Escalation factors Escalation controls o Module 6: Fault Tree Analysis Problem definition; Develop a thorough understanding of the system; Fault tree construction; and Fault tree solution

o Module 7: SWIFT, FMECA and Human Error Analysis Structured what-if technique Failure mode effect and control analysis Human error analysis 6. Course and Material Application o o o o PowerPoint presentation. Course workbook. Pre and final course assessment will be conducted. This course is a combination of classroom and practical exercises to ensure competency.

7. Assessments After completion of this course, the attendee will be able to demonstrate the ability to conduct advanced issue based risk assessments in the workplace. The competency includes all the activities required to successfully conduct risk assessments applying advanced techniques. Assessment will be conducted at the end of the course and includes: o o Exercise: identify and analyse hazards, score sheet; human failure analysis; barrier analysis; judging the risk; practical application Project: conduct an issue based risk assessment and present outcome to peer group

8. Who Should Attend This course will be presented as an open course and should be attended by: o o o Safety and health practitioners Engineers and senior technical personnel Mining/Operational managers that may be involved in issue based risk assessments

9. Costs The course can be attended at R1 300 per person per day or R5 900 per person attending the complete course. Additional discount of 10% for block booking of more than five (5) attendees can be offered.
