Petitioner, Kristian Jacob Abad Lora, (hereinafter Petitioner) respectfully states: 1. Petitioner is a citizen of the Philippines, born on December 12, 1990, over

21 years of age and has completed and finished his law degree at University of Cebu College of Law; 2. Petitioner is a member of the Philippine Bar having been admitted to the

practice of law on April 24, 2013 as evidenced by a photocopy of his Certificate of Membership attached hereto and made an integral part hereof as Annex A; 3. Petitioner has been a resident in the Philippines for more than one (1) year

and holds regular office at OSORIO REQUINA RIZON & ALMAGRO LAW OFFICES Room 305, G. K. Chua Building, Sanciangco Street, Cebu City, Philippines; 4. Petitioner has not been accused of any crime involving moral turpitude

before any court or tribunal in the Philippines; 5. Petitioner is not a member of any subversive association or any association

advocating to overthrow the existing government; 6. Petitioner is not a government employee nor has he applied for the same

commission in any other jurisdiction;

Petitioner is a member of good standing of the Integrated Bar of the

Philippines Cebu City Chapter as evidenced by IBP Official Receipt No. 386942 dated June 6, 2013, a photocopy of which is attached hereto as Annex B and the Certification of good moral character issued by two (2) executive officers of the local chapter of the Integrated Bar of the Philippines, copies of which are hereto attached as Annex C; 8. Annex D; 9. Petitioner undertakes to immediately inform the Honorable Court of any Petitioner has also paid the Professional Tax Receipt due his as evidenced

by PTR No. 385194 dated May 2, 2013 a photocopy of which is also attached herewith as

change in his address and/or telephone numbers; 10. The true impression of Petitioners notarial seal is attached to this Petition

together with the three specimens of his signature as Annexes E and F respectively.

PRAYER WHEREFORE, petitioner respectfully prays that he be commissioned as Notary Public for and in the City of Cebu for the term ending on December 31, 2016. Respectfully submitted. Cebu City, March 12, 2014.
