Emotional and sociological stresses of expected pregnancy and motherhood in women with diabetes mellitus type 1: How to increase their stress-coping capability?

Results
Of the 91 women under study, 69 expressed depression due to the medical limitations for the succesful the pregnancy and concern about fulfilling the role of the potential organizer of new family life due to the presence of diabetic retinopathy, nephropathy, arterial hypertension, active viral hepatitis, heart disturbances, autonomic neuropathy, or disturbances in ovarian function. Among the spiritual life values and stresses, the examined women underlined: 1) the need for moral, marital love - 59; 2) the rational recognition of the limitations in marital and family life due to diabetes mellitus type 1 by the future husband - 55; 3) planning the marriage only but not pregnancy - 42; 4) living as single, lack of belief for finding a proper husband - 39; 5) a feeling of being socially isolated or even discriminated against - 38; 6) a disdainful approach to contraception - 35. Among 91women under study, the number of cases with statistically valid presence of the following sociological stresses were: 1) strong dependence on other people (family, friends) - 72; 2) low level of therapeutic education - not fitting the needs of pregnancy and motherhood - 71; 3) poor control of diabetes mellitus type 1 - especially in

comparison with pregnancy standards - 60; 4) smoking 36; 5) economical powerty 31.

Conclusion
Conflict between the spiritual, emotional and sociological aspects of planning and future realization of pregnancy, of motherhood and of the role of the new family life organizer and the studied markers of depression, stressful pathogenic emotions and personality disturbances was evidenced as very essential. It is intensified by the indifferent or even hostile circumstances in the real world of diabetic type 1 women care and life and needs planned counteraction in the practice of diabetes mellitus care.

Nale tutaj 4 kategorie stresu: 1. Stresory poznawcze: - zrozumienie mechanizmw choroby, jej objaww icelw leczenia; - poznanie zabiegw sucych leczeniu wstrzyknicia, testowanie, sporzdzanie diety itd.; - logika ogranicze yciowych narzucanych przez chorob; - poszukiwanie, ktre strefy ycia osobistego nie s dotknite przez chorob; - racjonalizacja akceptacji dla powyszych potrzeb. 2. Stresory emocjonalne: - poczucie winy, kalectwa, inwalidztwa; - poczucie zagroenia egzystencji, bezradnoci icierpienia; - zmniejszenie odczuwania wasnej wartoci izwikszenie poczucia zalenoci od osb drugich; - obawa przed sprawianiem blu rodzinie ibliskim; - odczuwanie ostatecznoci rozpoznania klinicznego cukrzycy lub jej powika. 3. Stresory behawioralne: - potrzeba wytrenowania w realizacji reimu leczniczego, w spenieniu roli leczniczej, asymilacja zachowa leczniczych; - potrzeba korzystnego istabilnego stosunku chorego irodziny zfachowymi, medycznymi opiekunkami; - potrzeba normalnoci zachowa w ksztatowanym cigle na nowo ukadzie psychicznym, intelektualnym ispoecznym, rozwijania poczucia wasnej wartoci; - potrzeba szybkiego reagowania na stany wymagajce nagej interwencji hipoglikemia, ketoza, dodatkowa choroba; - potrzeba pewnoci wwykonywaniu samokontroli iwasnych, autonomicznych decyzjach leczniczych. 4. Stresory spoeczne: - przygotowanie do roli pracownika, maonka, rodzica; - uznanie koniecznoci interwencji medycznej wtym zakresie; - ograniczenia zawodowe, wymuszone warunkami zdrowotnymi ksztatowanie kariery profesjonalnej, trudno wzapewnieniu stabilizacji ekonomicznej; - wybr wtworzeniu postawy zainteresowania innymi ludmi, odruchw altruistycznych lub odwrotnie egocentryzm iizolacja; - konieczno relacji zformaln stron wpyww spoecznych: prawo, przepisy, urzdy.
