TOALLBHIEMPLOYEES,CONTRACTORS,VENDORSANDVISITORS: B.H.IncsSafetyProgramhasbeenpreparedtoassistyouinyourpersonal safetyneeds.Thismanualcannot,norisitintendedto,addressevery situationthatmaybeencounteredintheworkplace.Shouldyoufindthat theseworkrulesarenotexplicitforaparticularsituation,youareexpected tousesoundjudgmentandcommonsense.However,ifyouhavedoubtsas totheprocedure(s)thatyoushouldfollow,youaretoimmediatelycontacta Companyrepresentativeforfurtherinstructions. Therules,guidelinesandpracticesincludedinthismanualarebasedon regulations,commonsense,andourpastexperience.Yourcompliancewith ourSafetyProgramisessentialtoyourownpersonalsafetyaswellasthatof others. TheseworkrulesaresupplementarytotheCodeofFederalRegulationsand theFederalMineSafetyandHealthActof1977.Pleasereadthismanual carefullyandrememberthatthesafetyofeachemployeeandvisitorisof upmostimportancetoBHI.Safeworkperformancemustbethemost importantaspectofyourdailyroutine.Weexpectyoutoreturnhomeatthe endofeachandeveryshiftinthesameconditionyoureportedtoworkthat day. ThegoalofBHIiszeroinjuries,illnessesandincidents. Thisrequiresdedicationandcommitmentfromall employees. Thehealthandsafetyofouremployeesisavaluethat willnotbecompromised.Wewilladheretothehighest standardstoprovideandmaintainasafeandhealthy workplace.Thiscanonlybeachievedthroughintelligent actionandcooperativeeffortineducation,engineering andenforcement. WeatBHIhaveanobligationtoourselves,ourfamilies andourclientstocarryoutourdutieswiththegreatest regardforsafety.Oursuccessrequirescompliancewith rules,regulations,andmostimportantlydeveloping personalhabitsandpracticeswhichallowustoworksafe atalltimes.Nojobissoimportant,noordersourgent thatwecompromisethehealthandSafetyofour employees.

Section1Safety&HealthResponsibilities
PolicyStatement TheobjectiveofBHIsHealthandSafetyProgramistocontinuallyimproveupontheoverallsafety performanceatBHI.Recognizingthatunsafeworkpracticesgenerallycauseasmanyormoreaccidents asunsafeconditions,afocuswillbeplacedonbothinanefforttocreateasafeandhealthyworkplace forallBHIemployees.TheBHIHealthandSafetyProgram(s),aswellastheBHIHealthandSafetyRules, clearlysetforthexpectationsforemployeesandmanagementalike.Theymustbeclearlycommunicated andunderstoodbyallemployees.ItisimportantthatallBHIemployeesunderstandtherolewhichthey fillandtheresponsibilitiesofthatrole. ManagementandSupervisionResponsibilities Generally,itistheresponsibilityofmanagementtoprovideguidanceandleadershipfortheoverall safetyandhealtheffort.BHIManagementisresponsiblefortheimplementationoftheHealthand SafetyProgram.Specificmanagementresponsibilitiesforhealthandsafetyinclude,butarenotlimited to,thefollowing: 1.Communicate,implementandenforcetheBHIHealthandSafetyPolicy,Responsibilities,Rulesand Guidelines. 2.OperateincompliancewithBHIpolicies,governmentregulations,andsoundsafetypractices. 3.Supportandfacilitatesafetyinspectionsandreviewsbyresolvingallidentifiedissuesinatimely manner. 4.Coordinate,conduct,orparticipateintheinvestigationandreviewofallaccidentsandincidents. 5.Correctallknownsubstandardconditionsandpracticestopreventneedlesssafetyandhealthrisksto employees. 6.Reportallaccidents/incidentstothesafetydepartmentsoanaccident/incidentreportcanbe completedandanaccident/incidentinvestigationcanbeconducted. 7.Coordinateorconductperiodic(daily,weekly,monthly,etc.)safetymeetingswithemployees. Attendancesheetsaretobefilledoutandsubmittedtothesafetydepartmentforeverymeetingheld. 8.Coordinateorconductjobtaskinstructionsforemployeeswithnewtaskassignments.Coordinateor conductallrequiredsafetytrainingfornewemployeesandadditionaltrainingforexistingemployees andbeincompletecompliancewithrefreshertrainingguidelines. 9.Keepsafetyequipmentandpersonalprotectiveequipmentreadilyavailable. 10.Positivelyacknowledgesafeworkperformancesandimmediatelydisciplineemployeesforunsafe behaviors.

11.KeeppersonalcopiesofBHIsHealthandSafetyProgramManualcurrent.Allcorrespondence containingupdates,changesandadditionsaretobeproperlyinsertedintothemanual. 12.ReadandunderstandtheappropriateActfortheregulatoryauthoritygoverningthefacility.(e.g. OSHAorMSHA) EmployeeResponsibilities 1.ReadandunderstandBHIsSafetyPolicies,GeneralRequirementsandResponsibilities. 2.Inspectworkareasandequipmentonadailybasisandimmediatelycorrectsubstandardconditions. Substandardconditionsthatyouareunabletocorrectaretobereportedtoyourimmediatesupervisor. 3.Immediatelyreportanypropertydamage,personalinjuryorillnesstoyourimmediatesupervisor. 4.Reportconditionssurroundingnearmissincidentswhichcouldcausereoccurrencesandhazardsto otheremployeesifnotcorrected. 5.Properlyusepersonalprotectiveequipment(PPE)asrequired. 6.Conductpreshiftanduseinspectionsonmobileequipmentandlightdutyvehiclesasrequiredand immediatelyreportsubstandardconditions. 7.Performworkinasafemanneraccordingtocompanysafetyrules,guidelines,regulationsandsound safetypractices. 8.Activelyparticipateinallrequiredtraining. 9.Acceptandtakearoleofownershipinyourowntraining. 10.Beresponsiblefornotifyingyourimmediatesupervisoreitherwhentrainingisneededand/orwhen refreshertrainingisrequired. Contractor,VendorandDeliveryPersonnel 1.IndependentContractorsshallparticipateinaContractorPreworkMeetingtodiscusscontract provisionsandspecifications. 2.AllprospectiveindependentcontractorsmustsatisfyBHIandanyClientofBHIsprequalification requirements. 3.IndependentcontractorsthatworkonMSHAregulatedsitesshallinsurethatMSHAPart48 Comprehensive(Annual)Trainingisprovidedforeachemployeeontheproject.Documentationofthis trainingmustbeprovided.

4.Independentcontractorsshallprovideawrittenhealthandsafetyprogrampriortobeginningany project.Theplanmustincludeanddetail: Accidentinvestigationandreportingprocedures. Emergencyproceduresforfirstaidandmedicalservices. Fireprotectionandemergencyevacuation. Elevatedworkprocedures. Lockout/Tagoutprocedures. HazcomproceduresandpracticesforprovidingcopiesofallMaterialSafetyDataSheets(MSDS) foreachchemicalorhazardousmaterialdeliveredtoaprojectorsiteaswellascopiestoBHIs ProjectSuperintendent. Personalprotectiveequipment(PPE). Safetymeetings. Worksiteinspections. Healthandsafetytraining. Employeesanctionsforsafetyviolations. Electricalsafety. Confinedspaceentry. Hotworkpermits. DrugandalcoholScreening. 5.IndependentcontractorsshallprovideacopyoftheHazardTrainingPlanforpersonnelthatmaybe presentintheworkarea.TheplanshoulddetailameanstoinsurethatallCompany/Owneremployees, subcontractor(s)employeesandvisitorsarehazardtrainedpriortoenteringtheareaunderthe IndependentContractorscontrol. 6.IndependentcontractorsinconjunctionwiththeBHIshalldeterminetheareacontrolandthe feasibilityofbarricadinginthatarea. 7.Independentcontractorsshallcomplywithallapplicablefederalandstateregulationsand/orstate andcountycodes. 8.Dependingonthescopeofworkfortheproject,afulltimeonsiteSafetyManager/Supervisormay berequired.ThisdeterminationwillbemadebyBHIsSafetyDirectororhisdesigneeandtheBHI DivisionManager.TheBHIDivisionManagerisresponsibleforcommunicatingthisrequirementtothe contractor. 9.Independentcontractorsshallhavesoleresponsibilityforprovidingasafeworkplaceforyour employees,foremployeesofyoursubcontractorsandsuppliersofmaterialandequipment,for adequacyofandrequireduseofallsafetyequipmentandforfullcompliancewithaforesaidlaws, orders,citations,rules,regulations,standardsandstatutes.Youwillconductdailyworkplace examinationstodeterminethatsafeworkingconditionsandequipmentexist.Theseinspectionsshallbe documented,includinganyfindingsandthecorrectivemeasurestakentomitigatethehazard. 10.IndependentcontractorsshallcomplywiththeBHIshealthandsafetyrules,policiesandprovisions inordertosafeguardthepersonnelandpropertyoftheCompany. 5

11.Withintwentyfour(24)hoursofoccurrencetheindependentcontractorshallprovideBHIwith copiesofaccident/incidentreportsforthefollowing: PersonalinjurytoanemployeethatrequiresmedicaltreatmentasdefinedbyOSHAorMSHA30 CFRPart50 Significantpropertydamage Seriousnearmissincidents 12.Independentcontractorsshallprovideallfireprotectionequipmentandmedicaltreatmentsupplies requiredfortheiruse.BHIsfireprotectionequipmentandmedicalsupplieswillbeprovidedonlyto assistinemergenciesasappropriateandweacceptnoliabilityforanydefectiveorinoperativedevice and/ormedicalsupply. 13.Independentcontractorsshallrequireallemployeestousethenecessarypersonalprotective equipment(PPE)andworkclothingwhileonownerproperty.Ataminimum,thefollowingarerequired; longorshortsleeveshirts,longpants,hardhats,leathershoesorboots,safetyglasseswithsideshields andotherappropriatePPEasrequired. 14.Priortocommencingwork,whichcouldpossiblyimpactBHIsortheClientsemployeesorproperty, theindependentcontractorshallcontactaBHIcompanyrepresentative. 15.IndependentcontractorsshallparticipateinsafetyreviewmeetingswithBHIsProject Superintendentorhisdesigneetoassessjobrelatedissues.Ataminimum,suchmeetingswillbeheld weeklyormorefrequentlyasthecontractmayrequireordictate. 16.InareaswheretheCompanyrequiresrespiratoryprotectiontobeworn,theindependentcontractor shallprovidetheappropriatedevices,requiresaiduseanduseapropermedicalclearanceandfittest program. 17.BHIsequipmentcannotbeoperatedbyanindependentcontractorand/oritsemployeeswithout priorapprovalofBHIorBHIsProjectSuperintendent.Areleaseofliabilitydocumentmustbesignedby theindependentcontractorandapreoperationalinspectionofthemobileequipmentcompletedand documentedbeforeuse. 18.Whenweldingorburningisperformedaprehotworkinspectionshallbeconductedpriortothe issuanceofahotworkpermit.Theindependentcontractormustbeinvolvedandincludedinthesignoff process. 19.Independentcontractorsshalluseproperlockout/tagoutprocedurespriortoworkingon equipmentand/orcircuitsthatcouldcausebodilyinjurythroughcontactwithelectricallyenergized parts,accidentalstartupofmachineryorreleaseoffluidpressureetc. 6

Section2 HealthandSafetyRequirements
PolicyStatement AqualityhealthandsafetyprogramisvitaltothesuccessofBHI;thereforeworkingsafelyisacondition ofemployment.TheruleslistedbelowareanintegralpartofBHIsHealthandSafetyProgramand failuretocomplywiththemcouldleadtoinjuryofanotherindividual,BHIsorClientspropertyand/or disciplinaryactionuptoandincludingtermination.Employeesmustknowandfollowthesehealthand safetyrulesandallsupervisoryandmanagementpersonnelshallberesponsibleforenforcingthem. Requirements A.GeneralSafetyRules 1.Allclientjobsitesafetyrequirementswillbefollowedunlesstheydonotmeetthesafety requirementsofBHI,inwhichcase,themoststringentguidelineswillbefollowed. 2.Consumptionorpossessionofalcohol,oranycontrolledsubstancewithoutaprescription,willnotbe tolerated.ReferencetheBHIDrugPolicyforfurtherdrugrelatedrequirements. 3.Firearmsoncompanyproperty,incompanyvehicles,equipmentoratcompanyjobsites,willnotbe toleratedunlesswrittenapprovalisobtainedfromamemberofExecutiveManagement. 4.Vandalismofcompanypropertyand/orequipmentisprohibitedandanyviolationofthispolicyis groundsfordisciplinaryactionuptoandincludingtermination.Thisincludesmisuseand/orabuseof saidcompanypropertyand/orequipment. 5.Correctorcontrolhealthandsafetyhazardsastheyareidentifiedinyourworkareas. 6.Followgoodhousekeepingproceduresatalltimes. 7.Unlessotherwisespecificallydesignated,smokingisforbiddeninallCompanyoffices,restrooms, breakrooms,conferencerooms,parkinglotsandCompanybuildings.Smokingisalsoprohibitedin Companyownedorleasedvehiclesandequipment.Youwillalsobeexpectedtofollowthesmoking policiesatallClientworklocations. 8.Alwaysusetherighttooland/orequipmentforthejobandtakedefectivetoolsandequipmentoutof serviceimmediately. 9."Horseplay"includingfightingwhileworkingorwhileatworkisprohibitedandwillnotbetolerated. 10.Employeesshallreportallaccidents,incidents,propertydamagesandanyunsafeconditionsto his/hersupervisorimmediately.

11.Noemployeeshallbeassigned,allowedorberequiredtoperformworkaloneinanyareawhere hazardousconditionsexistthatwouldendangerhisorhersafetyunlesstheycancommunicatewith others,canbeheardorcanbevisuallyseen. 12.Threepointsofcontactshallbemaintainedwhileclimbingfixed,portableorequipmentaccess ladders.Handholdsandfootholdsshallbeusedwhenmountinganddismountingmachines.Never mountordismountmovingequipment. 13.Defectiveorbrokentoolsmustbepromptlyrepairedorreplaced.Theuseofbrokenorcrackedtool handles,mushroomedheadsonhammers,chisels,punches,barsandotherunsafeequipmentisstrictly prohibited. 14.Neveruseahardmetalhammertostrikeahardsteelchisel,wedgeorothertool.Oneorbothareto beofasoftsteeltypetopreventmetalfragments. 15.Keepsharpedgedtoolsinasafepositionatalltimes. 16.Neverremoveanequipmentormachineguarduntilyouhavedeenergizedandlockedoutthe affectedequipment. 17.Neverstartanymachineorequipmentuntilguardsareinplaceandallemployeesareoutofharms way. 18.Donottamperwithorbypassanysafetydeviceotherthanfortestpurposesduringscheduledor plannedtesting.Duringthetestingalwaysfollowapprovedtestingprocedures. 19.Familiarizeyourselfwiththelocationsoffirstaidequipmentinyourworkarea. 20.Firstaidsuppliesshallnotbeusedforanypurposeotherthancaringforinjuredemployeesor personswhobecomeill. 21.Nopersonispermittedtooperateanyequipmentand/orperformanyassignedtaskuntiltheproper tasktraininghasbeencompleted. A.CompressedAir 1.Compressedairmustneverbeuseddirectlyonthebodyorpointedtowardsanyperson.Faceshields orgogglesarerequiredwhencleaningsurfaceswithcompressedair. 2.Compressedairhosesandassociatedconnectorsmustberoutinelyinspectedandanydefects correctedpriortolinesbeingcharged. 3.Whipchecks,safetychainsorothersuitablesafetydevicesaretobeusedatconnectionsofairhoses. 4.Repairsinvolvingpressuresystemsofcompressors,receiversandfixedandflexiblelinesofpneumatic equipmentmustnotbeattempteduntilallpressurehasbeenbledoff.

5.Locateandidentifythemainshutoffvalve(s)closesttotheworkareabeforeworkingwithpneumatic equipment. B.PersonalProtectiveEquipment 1.Hardhats,safetyglassesandsteeltoedbootsorequivalentaremandatoryinallareasexceptoffices, lunchroomsandotherexemptareas. 2.Itisprohibitedtoalterorchangethedesignofanypersonalprotectiveequipmentoruseany defectivepersonalprotectiveequipment. 3.Protectiveclothing,faceshieldsandeyeprotectionshallbewornwhenperformingwelding,cutting, grindingoperations,jackhammeringoranyotherprocesswhereaneyeinjuryisimminent. 4.Hearingprotectionwillbeprovidedandshallbeworninareasdesignatedorpostedas"HighNoise Area"orHearingProtectionRequired,etc. 5.Fallprotectionsuchasabodyharnessandlanyardsshallbeusedtoensure100%tieoffwhen workingfromelevatedpositionswherethereisdangeroffalling. 6.Respiratoryprotectionshallbewornindesignatedareas.Protectionfornuisancedustwillbe provideduponrequest(e.g.dustmasks). 7.Specializedpersonalprotectiveequipmentandclothingwillbeprovidedandshallbeusedinwork areaswherespecifichazardshavebeenidentified(i.e.FRCandArcFlash). InsulatingProtectiveEquipment 1. Blanketsshallbetestedinanapprovedtestlaboratoryatleastonceeverysixmonths. 2. Insulatingprotectiveequipmentshouldbeconsideredasonlypartialprotectionandinnocase shoulditbedependedonasgivingcompleteprotection. 3. Onlyinsulatingprotectiveequipmentapprovedforthevoltagetobeworkedshallbeusedtocover energizedconductorsandapparatus. 4. Whenworkingdistributionvoltages50to39,000voltswherethepossibilityexiststhattheminimum approachdistancesasoutlinedinthesafetymanualmaybeinfringed,allconductorsandapparatus includingguywiresandgroundwireswithwhichemployeescancomeincontact,excepttheoneon whichtheyareworking,shallbecoveredwithapprovedprotectiveequipment.Ifgroundwires cannotbeadequatelycovered,theyshallbeclearedorshallbecarefullydisconnectedfromall groundcontactwhileworkisinprogress.Aftertheworkhasbeencomplete,groundwiresshallbe carefullyreconnected.Careshallbetakennottoopenthecommonneutralsystem. 9

RubberGloves NOTE:Actionlevelforlowvoltagewillbe50to600volts;601to39,000willbehighvoltage. 1. Rubberglovesshallbeinspectedcarefullyandgiventheairtestimmediatelypriortoeachtimethey areused.Theyshallnotbeworninsideoutorwithoutleatherprotectorgloves. Exception:Lowvoltageglovesmaybewornwithoutleatherprotectorsifthewearingofleather protectorscreatesadditionalhazardsoradditionaldexterityisrequired. 2. Rubberglovesshallbetestedinanapprovedtestlaboratoryatleastonceevery6months. 3. Rubberglovesshallbeputonbeforeclimbingtowithinreachofconductorsorapparatusenergized at50to39,000volts. 4. Rubberglovesshallbewornwhenworkingonundergroundprimaryconductors,apparatusorbuses inanytypedistributionvaultormanhole,ifgroundsareappliedatapointotherthanwherethe workisbeingperformeduntilverifiedbytestasdeenergized. 5. Rubberglovesshallbewornwhenworkingwithinreachofconductorsorapparatusenergizedat50 to39,000volts. 6. Rubberglovesshallbecarriedonemployeesbeltswhenevertheyclimbpolescarryingconductors energizedatvoltagesupto39,000volts. 7. Rubberglovesshallbeputonbeforeclimbingintoaworkingpositionabovethecommonneutralor secondaryposition. 8. RubberglovesshallbewornwhenopeningorworkinginenergizedURDtypedistributionsystems. 9. Rubberglovesshallbewornduringandshortlyafterstormswhenclimbingorworkingon distributionpolesorhandlingconductorsontheground,includingservicewires. 10. Rubberglovesshallbewornwhenpullingwireinoroutonpolescarryingenergizedconductors. 11. Rubberglovesshallbewornwhensettingpolesthroughornearconductorsorapparatusenergized at460to39,000volts. 12. Rubberglovesshallbewornwhileoperatingimpulsegenerators(thumpers). 13. Ontransmissionanddistributioncircuits,rubberglovesshallbewornwhilecheckingsecondary voltagewhenplacingtransformersinservice. 14. Highvoltagerubberglovesshallbewornwhenoperatingallenergizedgangoperatedairbreak switchesunlessanapprovehotstickisused. C.FireProtection 1.Knowthelocationoffireextinguishersandfirefightingequipmentinyourworkplace.

2.Allcompressedgascylindersmustbesecuredinanuprightposition,valvesclosedandcapsinplace whennotinuse. 3.Flammableandcombustiblematerialsmustbestoredinapprovedcontainersandindesignated locations. 4.Flammableandcombustiblematerialcontainersandstorageareasmustbeproperlylabeledfor contentsandshallonlybeutilizedforlabeledcontents. 5.Smokingandtheuseofopenflamesorsparkproducingtoolsisprohibitedwithinfifty(50)feetofany andallflammablestorageareas. D.MaterialHandling 1.Neveroverexertyourselfwhenlifting.Iftheloadismorethanyoucanhandle,gettheassistanceof otherpersonnelorequipment.Alwaysuseproperliftingtechniques. 2.Slingsandhooksshallbevisiblyinspectedbeforeeachuse.Ifdefectsareobserved,thatpieceof equipmentshallberemovedfromservice. 3.Anydeviceusedinliftingorsupportingneedstobeaproperlyratedapparatus. 4.Whenpowerequipmentisusedandsignalsarerequired,onlydesignatedpersonsshallgivethe signals.Thepersontosignalshallbedesignatedbythepersoninchargeoftheactivity. 5.Neverworkunderasuspendedload. 6.Alwaysusetaglineswhenhandlingloads. 7.Donotuseanymaterialforscaffoldingotherthanmaterialsmanufacturedforthatpurpose.All scaffoldplanksshallbesecuredtoscaffoldsupportspriortouse.Inspectladdersandscaffoldingpriorto use. 8.MaterialSafetyDataSheets(MSDS)identifythehazardsinvolvedwithchemicalproductsandspecify therecommendedpersonalprotectiveequipment(PPE)needed.Itistheemployeesresponsibilitytobe familiarwiththeMSDSandPPErequirementsinvolvedwiththechemicalproductshe/shecomesin contactwith. 9.Usehazardouschemicalsorproductscontaininghazardouschemicalsforintendedpurposesonlyand onlyasdirected.ReadandfollowallsafetywarningsontheMSDS. 10.CompleteandupdatedMSDSbooksarereadilyavailableatthemainoffice.OnecompleteMSDS bookwillbemadeavailableateachclientworklocation. E.OperationofEquipment 1.Onlyauthorizedorqualifiedpersonswithcurrenttasktrainingshalloperateequipment. 11

2.Beforeoperation,allselfpropelledmobileequipmentrequiresawalkaroundinspectionandthe properdocumentationtobecompleted.Anydefectsthataffectsafetyshallberepairedimmediately. 3.Companyand/orleasedequipmentshallbeoperatedinacourteousmannerandwiththehighest regardforsafetyofpersonnelandproperty. 4.AllpersonneloperatingequipmentorridinginBHIvehiclesshallwearseatbelts. 5.Observeandadheretoallwarningsignswhenoperatingequipmentorvehicles. 6.Whenoperatingequipmentorworkingnearenergizedoverheadpowerlines,ensurethatadequate clearanceanddistanceismaintained. 7.Equipmentoperatorsshallgroundallhydraulicspriortoexitingtheequipmentunlessproperly blockedorlockedinplace. 8.Keepwindshieldsandlightscleanandclearofobstructions. 9.Operatorsofvehiclesandmobileequipmentshallnotcrossoveranyelectricpowercablesorcords unlesscablesorcordsareprotectedbyapropercrossingdevice. 10.Nevermount,dismountorgetunderapieceofequipmentthatisinoperation. 11.Whentransportingloads,careshallbetakentoensurethattheloadisproperlyloadedandsecured againstmovement.DOTregulationsshallatalltimesbefollowed. 12.Alloutriggersshallbefirmlydownonlinetruckswhentheboomisinuse.Alloutriggers,where provided,shallbefirmlydownonaerialdeviceswhenthemanliftingdeviceisinuse. Iftheproperclearancecannotbemaintained,thelinewillbeinsulatedtopreventaccidental contactorthelinewillbedeenergizedandgrounded.
