Care plan Wound care Aim: To achieve wound healing

-Ensure that the wound dressing is done as and when it is required depending on the frequency of dressing Purpose: Remove the dead tissue(slough) and promote wound healing -Make sure patient eats well daily, check on the nutritional status of the patient Purpose: Good nutrition promotes healing of the wound and prevent possible occurrences of neuropathic ulcers as a result of diabetes - Give patient pain relievers before performing wound dressing - Wrap the left foot with plastic to prevent water from entering the wound Avoid contact with water as water retention leads to pus formation which can worsen the condition of the wound As the patient has no caregiver, wound care can either be continued at the community hospital or either arrange for home nursing services. Rehabilitation Aim: To resume premorbid status and achieve independence

Premorbid Status: ADLs independent and community ambulant -Physiotherapy to train the gait of the patient and regain previous walking ability - Occupational therapy to allow her to regain independence in activities of daily activities like cooking - Nurses to educate, train her to be more independent in activities such as showering, personal hygiene and encourage the patient to participate in their own care Before discharge, assess that the patient is able to perform all the activities independently Other concerns: Financial issues -Medical Social Worker to assess her financial situation and provide adequate assistance
