Description of the tool: Few schools have all the resources the# need. This eans that efforts to reduce health pro"le s that hinder #oung people+s attendance at school and their a"ilit# to learn ust "e carefull# targeted. The# ust supple ent/ not duplicate/ health services availa"le in the co unit#/ de onstrate tangi"le results and "e ore cost0effective than other solutions. Such careful targeting can "e achieved through regular assess ent of co unit# health needs and co unit# health resources. This tool can "e used "# school health progra e planners to collect infor ation needed to deter ine priorit# actions and to esta"lish effective partnerships with those having the resources to contri"ute to the i prove ent of co unit# health.
