Pikes Peak Interagency Transition Team (PPITT) Meeting Notes 12/19/13 Carol Lampe, Facilitator Presentations: Ann McKenzie,

Program Manager, presented for Bethesda Lutheran Communities which has just expanded services into Colorado Springs within the last 60 days. Corporate offices are located in Watertown, WI. So far, Bethesda is serving nine SLS clients and has three host homes ready to accept individuals with I/DD. Bethesda operates camp ministries at several places in the United States. They are accredited by the Council on Quality and Leadership (CQL). For more info, contact Ann McKenzie at ann.mckenzie@mailblc.org or call 641-3609. Annette Stahuljak is the CEO and Randee Van Ness is the COO of the newly expanded Community Smart which has been in business for eleven years. They will do SSA assessments, appeals, and advocacy; they attend evaluations and medical examinations that SSA schedules. They complete vocational assessments for ages 18 and up and are Ticket to Work approved, providing vocational training, finding employment, and job coaching. They will also assist parents with becoming representative payees for benefits and/or obtaining

guardianship. They focus on Social Securitys work incentive program, PASS (Plan to Achieve Self-Support), that allows an individual to start his/her own business. They complete PASS plans to submit to SSA for approval. They have a life skills program, an internship program, and a small business (micro-business) program. They will work with school district transition programs. So far, they have 76 employers willing to work with people with I/DD and can handle up to 250 students in their new facility. They are funded through a private funder and will charge $40/hour in the community or $32/hour for on-site services. Their new facility is at 6510A Academy Blvd., #281. Call 964-8142 or visit www.CommunitySmart.org for more info. Agency Updates: The first Monday of the month, Mosaic hosts a networking lunch (Discover the Possibilities) from 11-12:30. If interested in attending the next lunch (January 6th), contact Gayle at 208-3191 or gayle.gross@mosaicinfo.org. In January, Mosaic will expand its day services to three shifts, the last one ending at 9 pm and will also expand to Saturdays. The Broken Brain Achievers Club is meeting this Sunday for a Christmas potluck lunch. Bring a white elephant gift. Club/). They are going to sell entertainment books as a fundraiser. Cheyenne Village held a caregiving class (five week program / two hours per week). It was attended in equal numbers by direct service providers and caregivers. They will offer another class in February, and class time will be determined based on feedback from participants. CV also has private pay options in three-hour increments and distributed a flyer listing the private pay options and comprehensive residential services. Carmel is hosting several classes on disability etiquette and the ADA on Mondays from 3-4 pm, on the following days: 1/13/13, 2/3/13, 3/3/13. A four-hour sign language workshop will be held from 10 am to 2 pm on Wednesday, 1/29/14, at 4740 Flintridge Drive, #220. Cost is $20 in advance or $25 at the door. The second Saturday of each month from 9-10:30 am is for children. Cost is $15/class/child Pikes Peak Library District will kick off its Adult Reading Program on Saturday, 1/11/13, at 1:00 pm at the East

Library on 5550 N. Union Blvd. Refreshments and entertainment will be provided. The reading program is for adults (18+) with developmental disabilities. Participants will have a Reading log and will be able to collect prizes. For more information, email arp@ppld.org. TRE received approval to provide case management services for the Children with Autism (CWA) Waiver. At present, there are 75 children statewide receiving services and 300 on the waitlist. CWA is for 0-6 years of age and covers behavior therapy only. Supported Living Services (SLS) waivers will be offered during December to all those who have been on the waitlist since 2007 and want services now (ASAA). In January, we will begin to make SLS offers to those on the waitlist since 2008. Sign Language Network is celebrating 19 years in business congratulations! Resource Fair: For the April 17th Resource Fair, we must purchase food from the AspenPointe Caf (pizza for vendors) rather than delivery as in last year. PPITT will contract with Sign Language Network to provide interpreters. We will not need signs in Braille. C/S School District 11s print shop will be paid to print the resource guides for the fair. The cost per agency is $15/table. Will The Arc invoice agencies? We will need payment from agencies by April

1st in order for them to participate. As of today, PPITT has $2414.27 in its account. Resource Directory: The Directory Committee is comprised of Glenn Azare, Gayle Gross, Julie OBrien, and Sonya Teigen. The resource directory was passed around for individuals to make changes. The deadline to be part of the directory and to make changes is March 17th. We should be able to go to the website and update the resource directory for our individual sections.
