INSTRUCTIONS:

Mailing address will be changed at a Customer ID level. Hence mailing address will change on all accounts where the Customer ID is captured as the primary holder. Non-mailing address as

mentioned in this form will be updated. Signature on the request form need to be as per the account operating instructions as maintained on the account Self-attested address proof for the mailing address mentioned in this form needs to be provided not more than 3 months old. In case you do not have address proof for the mailing address, provide self-attested address proof of non-mailing address mentioned on the form, only if your relationship with the bank is more than 6 months old. In case you provide address of a close relative as your mailing address then a declaration as per Annexure-I needs to be submitted along with close relative's self-attested mailing address and identity proof. Please note that the declaration needs to be signed by both, the account holder(s) and the close relative. Where the form has not been submitted by you in person, a self-attested identity proof of the person signing the form is to be submitted. Further to this the same will not be processed until the bank has obtained email / telephonic confirmation from your registered contact details with us. In such a case, please ensure your email id / phone number is registered with the Bank. Address Change requested will be effected in the Bank's records by the Bank within a period of 4 to 5 working days from the date of receipt by HDFC Bank and the said changes would be effective in the systems from that date only. Address Change requested through PO Box would take additional time to process as per the Terms & Conditions mentioned on our website. In the mean while any communication by the bank would continue at the old address. Signature on the form should match with the signature on the records of HDFC Bank. Any cancellation/overwriting needs to be counter signed. Incomplete forms will not be processed.
