The sayin# #oes +)hat is not monitored cannot &e controlled,. The primary o&-ecti*e sho%ld &e, any incident, encompassin# accident, )hich leads to in-%ry.dama#e, or near miss, )hich does not lead to a&o*e, ta/in# place in site sho%ld &e reported )itho%t "ail and in*esti#ated. This proced%re "or accident.incident reportin# and in*esti#ation is applica&le only to those areas, )hich are co*ered %nder +Sa"ety 0istricts, as #i*en in EPCL - HSE. The p%rpose o" accident reportin# and in*esti#ation is that all accidents or incidents that res%lts in personal in-%ry or illness, and or dama#e to property shall &e properly reported and in*esti#ated. Altho%#h accident .incident in*esti#ation is a reacti*e process, a comprehensi*e accident reportin# and in*esti#ation process is a proacti*e meas%re that can e""ecti*ely pre*ent or minimi$e "%t%re accident. incidents.
