Thank you for inviting me to speak on behalf of Amerigroup Florida As pillars of the community, you are the leaders that can be a part of the solution of delivering care to your congregants Id like to begin with a little about who we are and how we deliver care to our members Amerigroup Florida is a medicaid managed care company,providing health care coverage in the state since 2003 and licenced to serve statewide We currently serve 237,000 members Through programs administered by the Agency for Health Care Administration, Department of Elder Affairs and Florida Healthy Kids Corporation, we are licensed to help people with low incomes get quality health care or long-term care services in all 67 counties Through our Medicaid and Medicare programs, we offer a range of long-term services and supports (LTSS) for low-income seniors and people with disabilities These benefits help members live independently in their homes and communities for as long as possible LTSS address peoples need for help with activities of daily living such as bathing, dressing, getting around the house and preparing meals While open to the entire family these programs are particularly focused on older, isolated adults, many of which are homebound or simply do not go out, leaving their homes once a week to get food or go to church We want to provide a vision of how church leaders, through their health ministries, can partner with a large health care organizations such as Amerigroup Florida to help these people live healthier, happier lives Our partnership with Antioch Missionary Baptist Church and their health ministry has been a true success which can serve as a model to be replicated in other ministries Our partnership has included on-site health screenings, community resource and health fair collaboration,a walking-program kick-off, and providing funding from our Foundation to help support their food pantry This year, through our Foundation we completed a giving campaign providing a total of $50,000 to churches across the state of Florida More than 50 organizations received funding, most of them were food pantries managed by faith-based organizations and several are represented here On behalf of Amerigroup Florida, we are honored to have the opportunity to share our vision with community leaders and to forge partnerships that will make a lasting impact
