Actor Release Form

Authorization to Reproduce Physical Likeness and/or Voice

I hereby grant SLAK Productions LLC (Producer) and any parent, subsidiary and affiliate corporation and their respective successors, assigns, licensee, employees and agents, the right to use my name, physical likeness and/or voice in and in connection with the production, exhibition, merchandising, advertising and promotion of the motion video production entitled Menschen (Production). I agree that the foregoing grant includes the right to use my physical likeness in any form, including, without limitation, a photograph, picture, artistic rendering, silhouette or other reproduction by photography, film, tape or otherwise. I represent to the best of my knowledge that the consent of no other persons, firm or corporation or labor organization is required to enable Producer to use my name, likeness, and/or voice as described herein or in connection with the Production. The rights granted herein include the right to use the Production or excerpts or stills from the Production (including excerpts or stills containing my likeness and/or voice) for any personal use granted by the Producer, (including DVD copies and photographs) or for the purposes of securing additional employment opportunities. It is the Producers sole discretion to decide what media can and cannot be used. This will not come into effect until after completion of the Production or until consent is given by Producer. Any and all media, (photography, film, voice recordings) including media taken by me or my affiliates (family, friends, etc.) pertaining to the Film cannot be reproduced, posted online, shown publicly, or used for any promotion of my career or the Production without Producers consent. The rights granted herein do not include commercial distribution or selling of the final product. All rights, title and interest in and to the results and proceeds of the services and performances rendered by me in connection with the filming of the Production or any portion therefore shall, from its inception, be the sole property of the Producer, free from any claims whatsoever by me and/or any other person.

Hold Harmless Agreement

I understand that participation in this activity involves a certain degree of risk. I have carefully considered the risk involved and I understand that participation in the activity is entirely voluntary and required participants to abide by applicable rules and standards of conduct. I release the Producer, the venue location, activity coordinators, and all employees, volunteers, related parties, or other organizations associated with the activity from any and all claims or liability arising out of this participation. I will provide my own medical insurance while on set (rehearsal, principal photography, and re-shoots) and will not rely on the Producer to provide medical services. In case of emergency, I understand that every effort will be made to provide me with the proper medical treatment, including hospitalization.

This agreement contains the full and complete understanding between the parties and supersedes all prior agreements and understanding pertaining herein and cannot be modified except by writing signed by each party.

I hereby certify and represent that I am of legal age and that I have read the foregoing and fully understand the meaning and effect thereof, and intending to be legally bound I have signed the authorization.
