The proposed sampling scheme for this project will be biased/judgmental, in accordance with procedures included in the Guidance for Performing Site Inspections Under CERCLA, OSWER Directive #9345.1 -05. September 1992, and Removal Program Representative Sampling Guidance, Volume I: Soil, OSWER Directive 9360.4-10, November 1991. Judgmental sampling is the subjective (biased) selection of sampling locations based on historical information, visual inspection, and the best professional judgment of the sampler(s). See Appendices A and B for additional site-specific information and site maps. The proposed number of samples is a balance between cost and coverage and represents a reasonable attempt to meet the study objectives while staying within the budget constraints of a typical integrated assessment. APPENDIX A

SITE-SPECIFIC INFORMATION FOR AN INTEGRATED PRELIMINARY ASSESSMENT/REMOVAL SITE EVALUATION OF THE SMT DRUM SITE

INTRODUCTION The Terra Tech EM Inc. (Tetra Tech) Superfund Technical Assessment and Response Team (START) has been tasked by the U.S. Environmental Protection Agency (EPA) Region 7 Superfund Division to conduct integrated preliminary assessment (PA)/removal site evaluation (RSE) activities at the SMT Drum site near Poplar Bluff, Missouri. Southern Tool and Machine (SMT) is a former machine and tool company that produced machined parts and provided tooling and special machinery for manufacturing companies. SMT employed over 100 employees in 2002, when it declared bankruptcy. Approximately 475 55-gallon drums of waste generated by the company were abandoned at the property at that time. The purpose of this investigation is to evaluate whether any threats to human health or the environment exist because of impacts of these former business activities on environmental media. The PA/RSE will include collection of the following samples: surface and subsurface soil samples from the site, sediment and surface water samples from a drainage pathway, and groundwater samples from temporary monitoring wells installed at the property. This Quality Assurance Project Plan (QAPP) identifies sitespecific features and addresses elements of the sampling strategy and analytical methods proposed for this investigation. An analysis of the data acquired during this project will proceed according to 40 Code of Federal Regulations (CFR) 300.410 and 300.415(b)(2) to determine the need for a removal action, as specified in the National Oil and Hazardous Pollution Contingency Plan (NCP). In addition, EPA guidance documents will be followed to determine need for followup pre-remedial Superfund response. SITE LOCATION/DESCRIPTION The SMT Drum site is located in a rural mixed residential and commercial area. It is located at 2721 Highway B, Poplar Bluff, Butler County, Missouri. The geographic coordinates for the site are: 36.767577 degrees north latitude, and 90.327147 degrees west longitude. The site occupies approximately 14.4 acres and includes a large metal manufacturing building. It is bounded along the east border by the Poplar Bluff Municipal Airport and to the south by Missouri Highway B. The north and west boundaries of the site adjoin agricultural land and residential lots. Elevation of the site is approximately 325 feet above mean sea level with little relief, which is typical of the southeastern lowlands of Missouri that fall within the Mississippi embaymentalso known as the Bootheel Region (see Appendix B, Figure 1).

PREVIOUS INVESTIGATIONS On March 16, 2012, EPA and the Missouri Department of Natural Resources (MDNR) responded to the SMT facility in response to a citizen complaint that leaking drums were present on the property. Ms. Sue Fisher of Poplar Bluff was identified as the current owner of the property. Ms. Fisher met with EPA and MDNR at the site and consented to an inspection of the property. Drums labeled as herbicides, sodium cyanide, mineral spirits, metal working fluid concentrate, solvents, and flammable liquids were observed. Many drums were not labeled or labels were inaccessible during the inspection. Several of the drums were observed to be lying down, bulging, and leaking. Sheds that once protected the drums had fallen, creating additional hazards. Pooled liquids that appeared to be emulsified oil were observed on the ground around some of the drums. On March 27, 2012, START and MDNR personnel inventoried drums that were accessible. Each container was numbered, and a drum inventory sheet was completed. The drum size, type, composition, color, condition, estimated volume of contents, and label information were recorded on each inventory sheet (272 containers were inventoried). On March 27 and 28, 2012, 58 drums were sampled and field screened by START and EPA. Materials meeting criteria for characteristic hazardous waste, as defined by the Resource Conservation and Recovery Act (RCRA), were identified in five of the containers according to the field screening results. These materials included corrosives (acids and bases), oxidizers, and flammable liquids. Field screening also identified halogenated compounds in three of the samples. On May 1, 2012, START and EPA returned to the site to initiate inventory, sampling, and field screening of all remaining containers at the site. On May 3, 2012, the fallen structures were moved by EPA Region 7's Emergency and Rapid Response Services (ERRS) contractor to allow access to the remaining containers. By May 4, 2012, all containers (616) had been inventoried, sampled, and field screened. Characteristic hazardous waste, as defined by RCRA, was identified in 51 of the 616 containers according to the field screening results. Six containers were identified as flammable liquids, six containers were identified as pH-neutral oxidizers, oxidizing acids were found in 14 containers, non-oxidizing acids comprised eight of the screened samples, 12 of the samples were oxidizing bases, and five were identified as non-oxidizing bases. On May 8, 2012, EPA, START, and ERRS returned to the site to bulk the hazardous wastes for disposal.

SAMPLING STRATEGY AND METHODOLOGY In support of EPA, under this task order, START will conduct sampling of environmental media at the SMT Drum site. A review of the physical characteristics of the site and the data generated during this investigation will help determine appropriate response, if necessary. Sampling procedures will follow standard operating procedures (SOP) outlined in this QAPP. Sampling activities will require two to three START personnel and will include collection of surface and subsurface soil samples, sediment samples, surface water samples, and groundwater samples. The surface and subsurface soil sampling will be conducted at locations where waste materials in drums had been staged on the property. The surface water and sediment samples will be collected from a large drainageway at the south end of the property. One groundwater sample will be collected from the area where drummed waste materials had been staged, and the other will be collected downgradient (south of) that area. Field activities are anticipated to take one day. Descriptions of the sampling strategy and procedures are presented below. Soil Sampling - START and EPA will identify 10 locations around the building, in the former drum storage area, and at other locations (as needed) to collect surface soil samples. At five of the 10 selected locations, subsurface soil sampling will be conducted (see Appendix B, Figure 2). Additionally, one background location will be selected where surface and subsurface soil samples will be collected. A grab sample will be collected with a disposable stainless steel spoon from 0 to 2 inches below ground surface (bgs) at each location, placed in an aluminum pie pan, and homogenized before the sample is placed into two 8-ounce glass jars. These samples will be submitted for metals, total petroleum hydrocarbons (TPH)-diesel range organics (DRO), and TPH-oil range organics (ORO) analyses. Pertinent data, including analyses to be performed and exact sample locations (depths and global positioning system [GPS] coordinates), will be recorded on a field sheet for each sample. All surface soil samples will be stored in coolers maintained at or below 4 degrees Celsius (C), pending submittal to the EPA Region 7 laboratory. Subsurface soil samples will be collected with a Geoprobe direct-push apparatus at five of the surface sampling locations on the property indicating stressed vegetation or staining, or at other locations of interest (as determined by EPA). Samples will be collected from 2 to 4 feet bgs, 6 to 8 feet bgs, and 10 to 12 feet bgs, or just above the soil-groundwater interface (whichever is encountered first) at each location. Each borehole will be advanced using a Geoprobe 4-foot-long Macro-Core sampler fitted with a disposable polyvinyl chloride (PVC) liner. Geoprobe sampling will be conducted in accordance with

Region 7 EPA SOP 4230.07: Geoprobe Operations. Each sample for laboratory analysis will include a grab sample for analysis for volatile organic compounds (VOC) collected in accordance with EPA SW-846 Method 5035, and will consist of two 40-milliliter vials each containing 5 grams of soil and preserved with sodium bisulfate, and two 40-milliliter vials packed with soil. After collection of the grab sample, the remaining soil from the sample interval will be placed in a disposable aluminum pie pan for homogenization, and then transferred to two 8-ounce jars. These samples will be submitted for metals, TPH-DRO, and TPH-ORO analyses. Pertinent data, including analyses to be performed and exact sample locations (depths and GPS coordinates), will be recorded on a field sheet for each sample. All subsurface soil samples will be stored in coolers maintained at or below 4 C, pending submittal to the EPA Region 7 laboratory. Surface Water and Sediment Sampling - Surface water and sediment samples will be collected from two on-site locations, plus one background location. From a perennial drainage pathway on the property, surface water and sediment samples will be collected to evaluate potential for transport of elevated concentrations of contaminants off of the property via runoff. The samples will be collected starting at the farthest downstream location and continuing upstream. At each location, the surface water sample will be collected prior to collection of the sediment sample. Surface water samples will be submitted to the EPA Region 7 laboratory to be analyzed for TPH-DRO, TPH-ORO, VOCs, and total metals. These samples will be collected by immersing the sample containers directly into the water bodies. Samples collected for analysis of VOCs will be collected in four 40-milliliter vials preserved with hydrochloric acid (HC1), and samples to be analyzed for TPH-DRO and TPH-ORO will be collected in 128-ounce amber glass jugs (one per sample). Water samples for metals analysis will be collected in 1-liter plastic containers (one per sample) and preserved with nitric acid (HNOj) to a pH <2. Sediment samples will be collected with a hand trowel from the top 6 inches of stream sediment. Grab samples collected for analysis of VOCs will be placed in two 40-milliliter glass vials. The remaining sediment will be placed in a disposable aluminum pie pan for homogenization, and then transferred to two 8-ounce jars. These containers will be submitted for TPH-DRO, TPH-ORO, and metals analyses. The hand trowel used for collection of sediment samples will be decontaminated with deionized water and Alconox after collection of each sample.

Pertinent data, including analyses to be performed and sample location data, will be recorded on a field sheet for each sample. All surface water and sediment samples will be stored in coolers maintained at or below 4 C, pending submittal to the EPA Region 7 laboratory. Groundwater Sampling - START will install Geoprobe temporary monitoring wells at two locations on the property. One will be in the former drum storage area, and the second location will be downgradient of that area. A background sample will also be collected at the location selected for background soil sampling. START will install Geoprobe temporary monitoring wells at the three locations to depths of approximately 30 feet bgs (just below the water table). Once the total depth has been reached, a disposable PVC screen will be deployed, and polyethylene tubing will be inserted into the rod string to the depth of the screen. Groundwater samples will be collected through the polyethylene tubing using a peristaltic pump or check valve. New tubing will be used for each sample. The screen holder will be decontaminated with deionized water and Alconox between samples. A field sheet will be completed for each groundwater sample. The field sheets will include the following information: property ownership information, exact sample locations, and analyses to be performed. The groundwater samples will be submitted to the EPA Region 7 laboratory to be analyzed for TPH-DRO, TPH-ORO, VOCs, and total metals. Groundwater samples collected for analysis of VOCs will be collected in four 40-milliliter vials preserved with HC1 to a pH<2. Water samples submitted for TPH-DRO and TPH-ORO analyses will be collected in 128-ounce amber glass jugs (one per sample). Water samples for metals analysis will be collected in 1-liter containers (one per sample) and preserved with HN0 to a pH <2. All water samples will be stored in coolers maintained at or below 4 C, pending

submittal to the EPA Region 7 laboratory. A summary of all anticipated samples for this project is in Table 1 of the attached QAPP form. The SOPs and chain-of-custody procedures referenced in the QAPP will be followed throughout the sampling activities to verify the integrity of the samples from the time of collection until they are submitted to the laboratory for analysis. Disposal of investigation-derived wastes (IDW) will be addressed in a site-specific health and safety plan prepared by START. IDW is expected to consist of disposable sampling supplies (gloves, paper towels, pie pans, etc.) that will be disposed of off site as uncontaminated solid waste. Quality Control Sampling - To evaluate sample quality control (QC), one soil trip blank and one water trip blank will be submitted for laboratory analysis of VOCs. Additionally, a water field blank will be submitted for analysis of total metals, TPH-DRO, TPH-ORO, and VOCs, as specified in Section 2.5 of

the QAPP form. Sufficient sample volume will be provided with each sample matrix to allow the laboratory to conduct matrix spike/matrix spike duplicate (MS/MSD) analyses. ANALYTICAL METHODS An Analytical Services Request (ASR) form will be completed and submitted to the EPA Region 7 laboratory prior to field activities. Appropriate containers and physical and chemical preservation techniques will be employed during the field activities to help verify acquisition of representative analytical results. Samples will be submitted to the EPA Region 7 laboratory in Kansas City, Kansas, for analyses for metals, TPH-DRO, TPH-ORO, and/or VOCs, according to the SOPs and methods referenced or described in the QAPP. Standard turnaround times and detection limits for the analytical methods referenced in this QAPP will be adequate for this project. As tentatively scheduled, START will submit samples to the laboratory on June 29, 2012.
