Allie Pool 06-03-12 Chucks Drug Box -Chuck is a 35 yr old male -At his first doctor visit, his

issues were high blood pressure and high cholesterol: *BP=139/89 -pulse= 86 -RR=12 *fasting cholesterol=320optimal <200 *LDL=257 optimal <100 *HDL=15optimal >60 -Triglycerides=30 **Possible medical diagnoses before any changes: -Prehypertension stage 2 -Hypercholesterolemia It was first recommend that Chuck get on a healthy diet and a regular exercise plan to make natural adjustments before trying medications. It has been a couple months since Chuck made his lifestyle changes such as diet and exercise. However, he is still having a problem with high blood pressure. If he would have still had cholesterol issues after his diet and exercise changes then we would have looked into prescribing some medications that would lower his cholesterol such as the following: *Cholesterol Meds: Statin: Lipitor (atrovastatin)take 40mg q day in the evening -lowering of total and LDL cholesterol and triglycerides (drops for a short period of time then comes back up to baseline) & slightly increases HDL Bile Acid Sequestrants: Welchol (colesevelam)take 1.9g bid with meals -Lowers lipids/decreases cholesterol and blood glucose Since high blood pressure is the remaining issue we will prescribe some hypertension medications. The following medications will help him lower his blood pressure and heart rate. This was his blood pressure after his lifestyle modifications: Mon: 140/96 Tues: 135/92 Wed: 133/74 Thurs: 138/63 Fri: 137/85

Allie Pool 06-03-12 **Possible medical diagnosis after lifestyle modifications: -Prehypertension stage 2stage 1 hypertension He was placed on a diuretic therapy a couple years ago and came in for his yearly check up to find that his blood pressure was still rising: Mon: 145/92 Tues: 150/96 Wed: 135/85 Thurs: 142/88 Fri: 140/74 He was compliant with his medication regimen so it is now time to explore a new option to help with his high blood pressure. The next step after a diuretic is a loop diuretic so we will start there and continue to monitor Chucks blood pressure and make modifications in his medication regimen if needed. *Hypertension Meds: -Since Loop Diuretics: Capoten (captopril)take 12.5mg bid, take one hour before meals -Lowers blood pressure in patients with hypertension Beta 1: Corgard (nadolol) take 40mg q day at the same time each day -Decreases heart rate and blood pressure (management of HTN)
