The following promotion is intended for participants in the United States only and shall be construed and evaluated according to the laws of the United States. Do not proceed in this promotion if you are not a legal resident of the United States. 1. ELIGIBILITY: Contest only open to the dental industry which includes dentists, hygienists, dental office managers and staff, dental students, Endodontists, Orthodontists and their respective staff who are legal residents of the 50 United States and the District of Columbia (except residents of Puerto Rico and physicians licensed to practice in Illinois, Massachusetts, Michigan, Minnesota, New Jersey, Puerto Rico, Rhode Island and Vermont), who are at least 21 years of age or the age of majority in their state of residence, whichever is greater at the time of entry. Physicians include doctors of medicine and osteopathy, dentists, podiatrists, optometrists and chiropractors. Void where prohibited by law. This promotion is governed by U.S. law and is subject to all applicable federal, state and local laws and regulations. Employees of Henry Schein, Inc. (HSI) and E4D Technologies (E4D) collectively (the Sponsors), and their affiliates, subsidiaries, parents, advertising, promotion and fulfillment agencies and members of the immediate family or household of each, are not eligible to participate. Immediate family member is defined as a parent, sibling or any person residing in same household as employee. 2. ENTRY: Contest begins on December 1, 2013 at 12:00 a.m. Eastern Time (ET) and ends on December 6, 2013 at 11:59 p.m. ET (Promotion Period). No responsibility is assumed by Sponsors for lost, late, misdirected, illegible, or incomplete entries. To enter The E4D Technologies NEVO Vine Contest, an Entrant must complete each of the following steps to receive one (1) entry into the Promotion (an Entry): 1. Follow @HenrySchein on Twitter and Vine; 2. Create a six (6) second video in English or Spanish while demonstrating the features of the E4D Technologies NEVO Cad/Cam system; 3. Add the hashtag #E4DNEVO to the description of the Video on Vine; and 4. Share the Video on Twitter with the above stated hashtags. Standard data rates may apply as established by an entrants wireless carrier. Sponsor assumes no responsibility for any such fees or charges. All entry information and videos shall be collectively referred to herein as the Submissions or each as a Submission. All Submissions must comply with the guidelines and restrictions defined below and must be submitted during the Promotion Period. All entrants must have a valid email address. Sponsors grants entrants a limited, revocable, non-sublicensable license to use Sponsors name, product, trademarks and logos (collectively, Sponsors IP) for the sole purpose of participating in this Promotion. Entrants are not permitted to make any further use of Sponsors IP for any purpose whatsoever. In addition, entrants recognize that all rights, title, and interest in Sponsors IP shall vest exclusively to the Sponsors, and entrant agrees that he or she has not and will not take any action that might harm or adversely affect such rights. No right, title or interest in and to the Sponsors IP except for the limited license granted to entrant in these Rules is transferred or created. Each entrant further acknowledges and agrees that Sponsors IP rights are

valid and enforceable, and that entrant shall do nothing to challenge the validity or enforceability of Sponsors IP in any forum. Entrants agree that the use of Sponsors IP is permitted only for the purpose of making an entry in this Promotion, and that any use of Sponsors IP (whether in the entry or otherwise) beyond this scope infringes the rights of Sponsors and will result in irreparable harm to Sponsors. By entering, each entrant warrants and represents the following with respect to their Submission: (a) entrant is the sole and exclusive owner of the Submission; (b) the Submission will not infringe on any rights of any third parties; (c) any third parties appearing in the Submission have given entrant appropriate consent to be filmed and used as permitted herein; and (d) if the Submission is made through Vine, Instagram or Twitter such Submission complies with and abides by the applicable terms of use, privacy policies, and guidelines in creating, describing, posting and sharing the video. and other restrictions. Sponsors reserves the right to request third party releases from any third party included in any Submission at any time. Failure to produce fully executed third party releases (if applicable) may result in disqualification of the Submission, in Sponsors sole and absolute discretion. Providing a Submission constitutes entrants consent to give Sponsors a royalty-free, irrevocable, perpetual, non-exclusive license to use, reproduce, modify, publish, create derivative works from, and display such Submissions in whole or in part, on a worldwide basis, and to incorporate it into other works, in any form, media or technology now known or later developed, including but not limited to on any and all Internet media, including Sponsors web sites and properties and on social networking sites (i.e., Facebook, Instagram, Twitter etc.), for any legal purpose whatsoever. Further, by providing a Submission entrant consents to Submissions being cut, edited, reformatted, rearranged, combined with other materials and/or otherwise modified, in Sponsors sole and absolute discretion. If requested, entrant will sign any documentation that may be required for Sponsors or their designees to make use of the non-exclusive rights entrant is granting to use the entry and Entry Materials. Limit one (1) Submission per person/address/household. No automated entry devices and/or programs will be permitted, and any entrant who creates multiple social media accounts in order to enter the Contest more than once will be ineligible to win. All entries become the sole and exclusive property of the Sponsors and receipt of entries will not be acknowledged. 3. CONTENT RESTRICTIONS: By entering, each entrant agrees that his or her Submission conforms to the Submission Content Restrictions as defined below (Submission Content Restrictions) and that Sponsors, in their sole discretion, may remove any Submission and disqualify an entrant from the Contest if it believes, in their sole discretion, that the entrants Submission fails to conform to the Submission Content Restrictions. Submission Content Restrictions: Any video submitted that shows illegal, dangerous or unsafe activity that violates the rights of third parties, or that is otherwise deemed by Sponsors in their sole discretion to be obscene or inappropriate (including, without limitation, if any such video contains an image of a competitive product) will not be posted. Without limiting the generality of the foregoing, any videos containing any of the following will not be accepted or posted: (a) competitive products; (b) unsafe behavior in activity that could result in physical injury or property damage; (c) third party

materials that you have no rights to, including videos or photos owned by third parties, images of celebrities, third party trademarks, artwork, etc.; (d) unlawful activity, including drug use; (e) obscenity or profanity; (f) anything defamatory or libelous; (g) anything disparaging Sponsors or any of their competitors; (h) anything racist or sexist; (i) sexual activity or implied sexual activity; (j) gambling; (k) alcohol; or (l) anything in poor taste. Videos submitted must be your original creation; in other words, they must be created by you. If the videos have not been created by you, the person who created the video must submit it. By submitting a video you represent and warrant that such video is your original creation, has not been copied in whole or in part from any other work, does not violate or infringe any copyright, trademark or other proprietary right of any person or entity (including any rights of privacy or publicity), and is your sole and exclusive property. Sponsors respect the rights of others. Submissions may contain materials of third parties and Sponsors may also transmit and store such third party materials, including your materials. As a result, materials of third parties, not owned or controlled by Sponsors, may be included in the Submission or may be stored or transmitted over Sponsor's digital online communication systems. To the best of Sponsor's knowledge, these materials do not infringe the rights of others. Upon receipt of proper notice of claimed infringement, Sponsors will respond expeditiously to resolve the claim between the notifying party and the alleged infringer who provided the materials in issue, including, where applicable, removing or disabling access to materials claimed to be infringing. The proper party to whom you should address such notice is social.media@henryschein.com. 4. PRIZES/ODDS: (1) Prize will be awarded a Keurig Vue V1200 Commercial Coffee Brewer. Approximate retail value: $266.99. The odds of winning depend on the number of eligible entries received. ALL EXPENSES, TAXES AND COSTS ASSOCIATED WITH ACCEPTANCE AND USE OF PRIZES NOT SET FORTH IN THESE RULES ARE THE OBLIGATION OF THE WINNERS. Prizes are not transferable, except at discretion of Sponsors. This prize is non-transferable and no substitution is permitted, except by Sponsors, who reserves the right, in their sole discretion, to substitute a prize of equal or greater value, if prize, or portion of prize, becomes unavailable. 5. SELECTION PROCESS: The winner will be selected by a panel of qualified judges selected by the Sponsors (Judges) from all eligible entries received on or about December 8, 2013. The Judges will evaluate each entry on how it best demonstrates the features of the E4D Technologies NEVO Cad/Cam system. The potential winner will be notified by direct message on Twitter. If Sponsors are unable to contact the potential winner after reasonable effort, or if the potential winner fails to respond within three days of first attempted notification, prize may be forfeited and awarded to an alternate winner. 6. GENERAL: The potential winner will be required to sign and return a Warranty of Eligibility and Liability Release and Publicity Release within ten (10) days of first attempted notification. In the event of noncompliance, the potential winner may be disqualified and an alternate winner selected, at Sponsors sole discretion. By entering, participants (a) agree to be bound by the official rules and the decisions of the Sponsors (including but not limited to eligibility determinations) which are final and legally binding in all respects; (b) agree to release Sponsors, and their employees and agents from any and all liability, loss, damage or injury resulting from participation in this promotion or awarding, receipt, possession use and/or misuse of any prize awarded herein and acknowledge that Sponsors and their employees and agents have

INTERNET: If, for any reason, the Internet portion of this promotion is not capable of running as planned, whether due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsors which corrupts or affects the administration, security, fairness, integrity, or proper conduct of this Contest, Sponsors reserves the right at their sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the Contest. In the event of termination, a random drawing to award the prize will be conducted from entries received up until the time of termination. Sponsors assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries.
