The PIQC Most Outstanding Alumni Award Overview: The PIQC Most Outstanding Alumni Award is an honour bestowed by PIQC and PIQCAAI upon the alumni in recognition of their significant and exemplary contributions in various fields of specialization and endeavours. Objectives: The Most Outstanding Alumni Award is presented to an alumnus/alumna who: Has made exceptional achievements and contributions to his/her profession and/or community, Brings honour and distinction to the heritage of PIQC Provides a positive role model to the current and future generation of students attending PIQC. The award also aims to bring the awardee close to the school by sharing his/her expertise through mentoring, forum, workshop and other programs that will be beneficial to the school and its student body.

Eligibility Criteria: 1. A nominee should be a student of PIQC (or graduate of PIQC) and must have attended high school in PIQC for at least two years. 2. The nominee is a positive role model, worthy of being emulated by the current and future generation of students of PIQC. 3. The nominee has either made significant contributions to the community or has achieved extraordinary success in his/her profession or chosen field. 4. The nominee is able to set standards of excellence in his/her professional field. 5. The nominee exhibits outstanding leadership qualities, integrity and service.

Nomination Procedure: 1. All nominations must be submitted through the official nomination form (required).

2. In addition to the submission of completed nomination form, the following supporting documents are required to further strengthen the achievements of the nominee. Such documents include, but not limited to: a. Three letters from non-family members, preferably those who are related to the chose field or profession, that support the nomination b. Current curriculum vitae of the nominee c. Award or certificate of achievement d. Newspaper or magazine article feature of the nominee 3. The nominator can be non-alumni, but cannot be immediate family members 4. An individual can nominate as many qualified alumni in different fields or professions. 5. Nomination period is from July 29, 2013 to November 30, 2013. 6. Only completed nomination will be considered. 7. The following members of the Award Committee are neither allowed to submit nominations nor provide letter of support, namely: a. Perfecto Yap b. Paul L. So c. Johnny Miguel Chua d. Shen Ratilla e. Edward Hernandez f. Ken Sioco g. Aurora Zosimo

Method of Selection of Award Recipients: The Most Outstanding Alumni Award Committee will use the following guidelines for the Selection of the Award Recipients: 1. The Committee will accept nominations and will review and validate against established criteria (July 29, 2013 to November 30, 2013). 2. Selection Criteria: a. Professional/Career Achievement s 40% b. Service to the School 10% c. Community Involvement 10% d. Academic Achievements 40% 3. Accepted nominations will be published in the school bulletin board. 4. The Committee will conduct get to know you interviews for pre-selected nominees from December 15, 2013 to January 10, 2014. For those nominees based in Metro Manila, the interviews will be done face to face, In the event that the nominee is based outside Metro Manila or Philippines, other modes of

communication may be considered, eg., Skype, video conferencing, long distance calls, etc. The school board will do the final selection of awardees. Announcement of Awardees will be on January 15, 2014 Awardees will be informed either through email or phone call Recognition will be held during the Grand Alumni Homecoming on February 9, 2014. Awardees are highly encouraged to attend. If an awardee will not be physically present, an immediate family member or duly authorized representative can accept the award on his behalf.
