CONSENTAGREEMENT
BETWEEN
MARKC.LEESON,M.D.,
AND
THESTATEMEDICALBOARDOFOHIO
ThisConsentAgreementisenteredintobyandbetweenMarkC.Leeson,M.D. [Dr.Leeson],and
theStateMedicalBoardofOhio[Board],astateagencychargedwithenforcingChapter4731.,
OhioRevisedCode.
Dr.LeesonentersintothisConsentAgreementbeingfullyinformedofhisrightsunderChapter
119.,OhioRevisedCode,includingtherighttorepresentationbycounselandtherightto a
formaladjudicativehearingontheissuesconsideredherein.
BASISFORACTION
ThisConsentAgreementisenteredintoonthebasisofthefollowingstipulations,admissions
andunderstandings:
A. TheBoardisempoweredbySection4731.22(B),OhioRevisedCode,tolimit,
revoke,suspendacertificate,refusetoregisterorreinstateanapplicant,orreprimand
orplaceonprobationtheholderof acertificateforviolationof Section
4731.22(B)(26),OhioRevisedCode,for "impairmentofabilitytopracticeaccording
toacceptable and prevailing standardsofcare becauseof habitualor excessive use or
abuseof drugs,alcohol,orothersubstancesthatimpairabilitytopractice;"and/or
Section 4731.22(B)( 10), Ohio Revised Code,"[c]ommissionof an actthatconstitutes
afelonyinthisstate,regardlessofthejurisdictioninwhichtheactwascommitted."
B. TheBoardentersintothisConsentAgreementinlieuofformal proceedingsbased
upontheviolationsofSection4731.22(B)(26),OhioRevisedCode;andSection
4731.22(B)(10),OhioRevisedCode,towit: DeceptiontoObtainaDangerousDrug,
Section2925.22,OhioRevisedCode,IllegalProcessingof DrugDocuments,Section
2925.23,OhioRevisedCode,and/orTheft,Section2913.02,OhioRevisedCode,as
setforthinParagraphEoftheStepIConsentAgreementBetweenMarkC. Leeson,
M.D.,andtheStateMedicalBoardofOhio,effectiveAugust 11,2010[August2010
StepIConsentAgreement],andassetforthinParagraphEbelow. TheBoard
expresslyreservestherighttoinstituteformal proceedingsbaseduponanyother
violationsof Chapter 4731.of theRevisedCode,whether occurringbeforeorafter the
effectivedateof thisAgreement. Suchexpressreservationincludes,butisnotlimited
to,violationsbasedonanymethodsusedbyDr. Leesontoobtaincontrolled
substancesand/ordangerousdrugsforself-use,and/oractsinvolvingpatientcareor

otherwiseinvolvingpatients,otherthan asparticularlyreferencedordescribedin
ParagraphEof theAugust2010StepIConsentAgreement.
C. Dr. Leesonisseekingreinstatementofhiscertificatetopracticemedicineandsurgery,
licensenumber35.042998,whichwasindefinitelysuspended,butnotlessthan 180
days,pursuanttotheAugust20 I0StepIConsentAgreement,acopyof whichis
attachedheretoandincorporatedherein.
D. Dr.LeesonstatesthatheisalsolicensedtopracticemedicineandsurgeryintheState
ofWestVirginia
E. Dr. Leesonadmitsthatafterenteringresidentialtreatmentforchemicaldependency
onoraboutJuly 16,2010,atShepherdHill,aBoard-approvedtreatmentproviderin
Newark.,Ohio,hewasdischarged,treatmentcomplete,onoraboutAugust27,2010.
Dr. Leesonstates,andtheBoardacknowledgesreceiptof informationtosupport,that
sincebeingdischargedfromShepherdHill,hehasremainedcompliantwiththe
aftercarecontractheenteredonoraboutSeptember15,2010,includingattendingand
participatinginatleastthree 12-stepmeetingsper ~ attendingatleastone
aftercaremeetingperweek.andsubmittingtorandomdrugandalcoholtesting. Dr.
Leesonadmitsthattheaforementionedaftercarecontractremainsineffect.
Dr.Leesonstates,andtheBoardacknowledges,thatRichardN.Whitney,M.D.,
MedicalDirectoratShepherdHill,hasprovidedawrittenreportindicatingthatDr.
Leeson'sabilitytopracticehasbeenassessedandthathehasbeenfoundcapableof
practicingaccordingtoacceptableandprevailingstandardsof care,solongascertain
treatmentandmonitoringconditionsareinplace. Dr.Leesonfurtherstates,andthe
Boardacknowledges,thatAlanShein,M.D.,MedicalDirectorof IgnatiaHallatSt.
ThomasHospital,a Board-approvedtreatmentproviderinAkron,Ohio,hasprovided
awrittenreportindicatingthatDr.Leeson'sabilitytopracticehasbeenassessedand
thathehasbeenfoundcapableof practicingaccordingtoacceptableandprevailing
standardsof care,so longascertaintreatmentandmonitoringconditionsareinplace,
includingthatDr.Leesonrestricthishourstonomorethanfortyhoursperweekfor
thefirsteighteenmonthsfromthedateof hissobriety.
Accordingly,Dr.Leesonstates,andtheBoardacknowledgesreceipttoinformationto
support,thatDr. Leesonhassubstantiallyfulfilledtheconditionsforreinstatementof
hiscertificatetopracticemedicineandsurgeryintheStateof Ohio,asestablishedin
theabove-referencedAugust2010StepIConsentAgreement.
AGREEDCONDITIONS
Wherefore,inconsiderationoftheforegoingandmutualpromiseshereinaftersetforth,andin
lieuof anyformalproceedingsatthistime,thecertificateof Dr.Leesontopracticemedicineand

surgeryintheStateof OhioshallbeREINSTATED,andDr.Leesonknowinglyandvoluntarily
agreeswiththeBoardto thefollowingPROBATIONARYterms,conditionsandlimitations:
1. Dr.Leesonshallobeyallfederal, state,andlocallaws,andallrolesgoverningthe
practiceofmedicinein Ohio.
2. Dr.Leesonshallsubmitquarterlydeclarationsunderpenaltyof Boarddisciplinary
actionand/orcriminalprosecution,statingwhethertherehasbeencompliancewithall
theconditionsof thisConsentAgreement. Thefirstquarterlydeclarationmustbe
receivedintheBoard'sofficesonthedatehisquarterlydeclarationwouldhavebeen
duepursuanttohisAugust2010StepIConsentAgreementwiththeBoard,oras
otherwiserequestedbytheBoard. Subsequentquarterlydeclarationsmustbe
receivedintheBoard'sofficesonorbeforethefirstdayof everythirdmonth.
3. Dr.Leesonshallappearinpersonforaninterviewbeforethefull Boardorits
designatedrepresentative. TheflfStsuchappearanceshalltakeplaceonthedatehis
appearancewouldhavebeenscheduledpursuanttohisAugust2010StepIConsent
AgreementwiththeBoard. Subsequentpersonalappearancesmustoccureverythree
monthsthereafter,and/orasotherwiserequestedbytheBoard. If anappearanceis
missedorisrescheduledforanyreason,ensuingappearancesshallbescheduled
basedontheappearancedateasoriginallyscheduled.
4. Dr. LeesonshallobtainpermissionfromtheBoardfordeparturesorabsencesfrom
Ohio. Suchperiodsof absenceshallnotreducetheprobationaryterm,unless
otherwisedeterminedbymotionof theBoardforabsencesof three monthsorlonger,
orbytheSecretaryortheSupervisingMemberoftheBoardforabsencesoflessthan
threemonths,ininstanceswheretheBoardcanbeassuredthatprobationary
monitoringisotherwisebeingperformed. Further,theSecretaryandSupervising
Memberof theBoardshallhavethediscretiontograntawaiverof partorallof the
probationarytermssetforthin thisConsentAgreementforoccasionalperiodsof
absenceof fourteendaysorless. Intheeventthat Dr. Leesonresidesand/oris
employed at a location thatis within fifty milesof the geographic borderofOhio and
anyof itscontiguousstates,Dr. LeesonmaytravelbetweenOhioandthatcontiguous
statewithoutseekingpriorapprovalof theSecretaryorSupervisingMemberprovided
thatDr.Leesonisabletootherwisemaintainfull compliancewithallotherterms,
conditionsandlimitationssetforthinthisConsentAgreement.
5. IntheeventDr.LeesonisfoundbytheSecretaryof theBoardtohavefailedto
complywithanyprovisionofthisConsentAgreement,andissonotifiedofthat
deficiencyinwriting,suchperiod(s)of noncompliancewillnotapplytothereduction
oftheprobationaryperiodunderthisConsentAgreement.

6. Dr.Leesonshallkeepa logof allcontrolledsubstancesprescribed. Suchlogshallbe
intheformatapprovedbythe onthedateuponwhichDr. Leeson's
quarterlydeclarationisdue,orasotherwisedirectedbytheBoard. Further,Dr.
Leesonshallmakehispatientrecordswithregardtosuchprescribingavailablefor
reviewbyanagentoftheBoardimmediatelyuponrequest.
7. Dr.Leesonshallnot,withoutpriorBoardapproval,administer,personallyfurnish,or
possess(exceptasallowedunderParagraph8below)anycontrolledsubstancesas
definedbystateorfederal law. In theeventthattheBoardagreesatafuturedateto
modifythisConsentAgreementtoallowDr. Leesontoadministerorpersonally
furnishcontrolledsubstances,Dr.Leesonshallkeepa logof allcontrolledsubstances
prescribed,administeredorpersonallyfurnished. Suchlogshallbesubmittedinthe
formatapprovedbytheBoardandshallbesubmittedtotheBoardnolaterthanthe
dateuponwhichDr.Leeson'squarterlydeclarationisdue,orasotherwisedirectedby
theBoard. Further,Dr.Leesonshallmakehispatientrecordswithregardtosuch
prescribing,administering,orpersonallyfurnishingavailableforreviewbyanagent
of theBoardimmediatelyuponrequest.
Sobriety
8. Dr.Leesonshallabstaincompletelyfromthepersonaluseorpersonalpossessionof
drugs,exceptthoseprescribed,dispensedoradministeredtohimbyanotherso
authorizedbylawwhohas full knowledgeof Dr. Leeson's history ofchemical
dependency. Further,intheeventthatDr. Leesonissoprescribed,dispensedor
administeredanycontrolledsubstance,carisoprodol,ortramadol,Dr. Leesonshall
notifytheBoardin writingwithinsevendays,providingtheBoardwiththeidentityof
theprescriber;thenameofthedrugDr. Leesonreceived;themedicalpurposefor
whichhereceivedsaiddrug;thedatesuchdrugwasinitiallyreceived;andthedosage,
amount, number of refills, and directions for use. Further, within thirty daysofthe
datesaiddrugissoprescribed,dispensed,oradministeredtohim,Dr.Leesonshall
providetheBoardwitheitheracopyof thewrittenprescriptionorotherwritten
verificationfromtheprescriber,includingthedosage,amount,numberof refills,and
directionsforuse.
9. Dr. Leesonshallabstaincompletelyfromtheuseofalcohol.
DrugandAlcoholScreenslDrugTestingFacilityandCollectionSite
10. Dr. Leesonshallsubmitto randomurinescreeningsfordrugsandalcoholatleasttwo
timespermonth,orasotherwisedirectedbytheBoard. Dr.Leesonshallensurethat

all screening reports are forwarded directly to the Board on a quarterly basis. The
drug testing panel utilized must be acceptable to the Secretary of the Board, and shall
include Dr. Leeson's drug(s) of choice.
Dr. Leeson shall abstain from the use of any substance and the consumption of poppy
seeds or any other food or liquid that may produce a low level positive result in a
toxicology screen. Dr. Leeson acknowledges that he understands that the
consumption or use of such substances, including but not limited to substances such
as mouthwash or hand cleaning gel, may cause a positive drug screen that may not be
able to be differentiated from intentional ingestion, and therefore such consumption or
use is prohibited under this Consent Agreement.
All such urine screenings for drugs and alcohol shall be conducted through a Board-
approved drug testing facility and collection site pursuant to the global contract
between said facility and the Board, that provides for the Board to maintain ultimate
control over the urine screening process and to preserve the confidentiality of all
positive screening results in accordance with Section 4731.22(FX5), Ohio Revised
Code, and the screening process shall require a daily call-in procedure. Further, in the
event that the Board exercises its discretion, as provided in Paragraph 11 below, to
approve urine screenings to be conducted at an alternative drug testing facility and/or
collection site or a supervising physician, such approval shall be expressly contingent
upon the Board retaining ultimate control over the urine screening process in a
manner that preserves the aforementioned confidentiality of all positive screening
results.
Dr. Leeson shall submit, at his expense and on the day selected, urine specimens for
drug and/or alcohol analysis. All specimens submitted by Dr. Leeson shall be
negative, except for those substances prescribed, administered, or dispensed to him in
confonnance with the tenns, conditions and limitations set forth in this Consent
Agreement. Refusal to submit such specimen, or failure to submit such specimen on
the day he is selected or in such manner as the Board may request, shall constitute a
violation of this Consent Agreement.
Further, within thirty days ofthe effective date of this Consent Agreement, Dr.
Leeson shall enter into the necessary financial and/or contractual arrangements with
the Board-approved drug testing facility and/or collection site in order to facilitate the
urine screening process in the manner required by this Consent Agreement. Further,
Dr. Leeson shall promptly provide to the Board written documentation ofcompletion
of such arrangements, including a copy of any contract entered into between Dr.
Leeson and the Board-approved drug testing facility and/or collection site. Dr.
Leeson's failure to timely complete such arrangements, or failure to timely provide
written documentation to the Board of completion of such arrangements, shall
constitute a violation of this Consent Agreement However, Dr. Leeson and the
Board further agree that in the event Dr. Leeson previously entered into the

aforementionedfinancialandcontractualagreementspursuanttotherequirementsof
apriorconsentagreementwiththeBoardunderwhichDr. Leesoniscurrently
participatinginanongoingurinescreeningprocess,thenthisrequirementshallbe
waivedundertheinstantconsentagreement.
Dr.LeesonshallensurethattheurinescreeningprocessperformedthroughtheBoard-
approveddrugtestingfacilityand/orcollectionsiterequiresadailycall-inprocedure;
thattheurinespecimensareobtainedonarandombasis;andthat the givingof the
specimeniswitnessedbyareliableperson. In addition,Dr.LeesonandtheBoard-
approveddrug testingfacilityandcollectionsiteshallassurethatappropriatecontrol
overthespecimenismaintainedandshallimmediatelyinformtheBoardofany
positivescreeningresults.
Dr.LeesonshallensurethattheBoard-approveddrugtestingfacilityand/orcoJlection
siteprovidesquarterlyreportstotheBoard,inaformatacceptabletothe Board,
verifyingwhetherallurinescreenshavebeenconductedincompliancewiththis
ConsentAgreement,and whetherallurinescreenshavebeennegative.
IntheeventthattheBoard-approveddrugtestingfacilityand/orcollectionsite
becomesunableorunwiJlingtoserveasrequiredbythisConsentAgreement,Dr.
LeesonmustimmediatelynotifytheBoardinwriting,andmakearrangements
acceptabletotheBoardpursuanttoParagraph11 below,assoonaspracticable. Dr.
LeesonshallfurtherensurethattheBoard-approveddrugtestingfacilityand/or
collectionsitealsonotifiestheBoarddirectlyof itsinabilitytocontinuetoserveand
the reasonstherefore.
Dr. LeesonacknowledgesthattheBoardexpresslyreservestherighttowithdrawits
approvalofanydrugtestingfacilityand/orcollectionsiteintheeventthat the
SecretaryandSupervisingMemberof theBoarddeterminethatthedrugtesting
facilityand/orcollectionsitehas demonstrateda lackof cooperationinproviding
informationtotheBoardorforanyotherreason.
11. Dr.LeesonandtheBoardagreethatitistheintentof thisConsentAgreementthatDr.
LeesonshallsubmithisurinespecimenstotheBoard-approveddrugtestingfacility
andcollectionsitechosenbytheBoard. However,intheeventthatutilizingsaid
Board-approveddrug testingfacilityand/orcollectionsitecreatesanextraordinary
hardshipuponDr. Leeson,asdeterminedinthesolediscretionof theBoard,then
subjecttothefollowingrequirements,theBoardmayapproveanalternatedrug
testingfacilityand/orcollectionsite,orasupervisingphysician,tofacilitatetheurine
screeningprocessforDr.Leeson:
a. Withinthirty daysofthedateuponwhichDr. LeesonisnotifiedoftheBoard's
determinationthatutilizingtheBoard-approveddrugtestingfacilityand/or
collectionsiteconstitutesanextraordinaryhardshipuponDr.Leeson,he shall

submittotheBoardinwritingforitspriorapprovaltheidentityofeitheran
alternatedrugtestingfacilityandcollectionsite,orthenameofaproposed
supervisingphysician,towhomDr.Leesonshallsubmittherequiredmine
specimens. In approvinga facility,entity,oranindividualtoserveinthis
capacity,theBoardwillgivepreferenceto a facilitylocatednearDr.Leeson's
residenceoremploymentlocation,ortoaphysicianwhopracticesinthesame
localeas Dr.Leeson. Dr. Leesonshallensurethattheurinescreeningprocess
performedthrough thealternatedrugtestingfacilityand/orcollectionsite,or
throughthesupervisingphysician,requiresa dailycall-inprocedure;thatthe
urinespecimensareobtainedonarandombasis;andthatthegivingof the
specimeniswitnessedbyareliableperson. In addition,Dr.Leeson
acknowledgesthatthealternatedrugtestingfacilityandcollectionsite,orthe
supervisingphysician,shallassurethatappropriatecontroloverthespecimenis
maintainedandshallimmediatelyinformtheBoardof anypositivescreening
results.
b. Dr. Leesonshallensurethatthealternatedrugtestingfacilityand/orcollection
site,orthesupervisingphysician,providesquarterlyreportstotheBoard,ina
formatacceptabletotheBoard,verifYingwhetherallurinescreenshavebeen
conductedincompliancewiththis ConsentAgreement,andwhetherallurine
screenshavebeennegative.
c. In theeventthatthedesignatedalternatedrugtestingfacilityand/orcollection
site,orthesupervisingphysician,becomesunableorunwillingtososerve,Dr.
LeesonmustimmediatelynotifytheBoardinwriting. Dr. Leesonshallfurther
ensurethatthepreviouslydesignatedalternatedrugtestingfacilityand
collectionsite,orthesupervisingphysician,alsonotifiestheBoarddirectlyof
theinabilitytocontinuetoserveandthereasonstherefore. Further,inorderto
ensurethattherewillbenointerruptioninhisurinescreeningprocess,uponthe
previouslyapprovedalternatedrugtestingfacility, collectionsite,orsupervising
physicianbecomingunabletoserve,Dr.Leesonshallimmediatelycommence
urinescreeningattheBoard-approveddrugtestingfacilityandcollectionsite
chosenbytheBoard,untilsuchtime,if any,thattheBoardapprovesa
subsequentalternatedrugtestingfacility,collectionsite,orsupervising
physician,if requestedbyDr.Leeson.
d. TheBoardexpresslyreservestherighttodisapproveanyentityorfacility
proposedtoserveasDr.Leeson'sdesignatedalternatedrugtestingfacility
and/orcollectionsite,oranypersonproposedtoserveashissupervising
physician,ortowithdrawapprovalof anyentity,facilityorpersonpreviously
approvedtososerveintheeventthattheSecretaryandSupervisingMemberof
theBoarddeterminethatanysuchentity,facilityorpersonhas demonstrateda
lackofcooperationinprovidinginformationtotheBoardorforany other
reason.

12. AllscreeningreportsrequiredunderthisConsentAgreementfromtheBoard-
approveddrugtestingfacilityand/orcollectionsite,orfromthealternatedrugtesting
facilityand/orcollectionsiteorsupervisingphysician,mustbereceivedinthe
Board'sofficesnolaterthantheduedateforDr.Leeson'squarterlydeclaration. Itis
Dr.Leeson'sresponsibilitytoensurethatreportsaretimelysubmitted.
13. TheBoardretainstherighttorequire,andDr.Leesonagreestosubmit,blood,urine,
breath,salivaand/orhair specimensforscreeningfordrugsandalcohol,foranalysis
oftherapeuticlevelsof medicationsthat maybeprescribedforDr.Leeson,orforany
otherpurpose,atDr. Leeson'sexpenseupontheBoard'srequestandwithoutprior
notice.Dr. Leeson'srefusaltosubmitaspecimenuponrequestoftheBoardshall
result in aminimum ofone year of actual license suspension. Further, the collection
of such specimens shallbe witnessed by a representativeoftheBoard,or another
personacceptabletotheSecretaryorSupervisingMemberof theBoard.
Mo itoringPhysician
14. Beforeengaginginanymedicalpractice,Dr.LeesonshallsubmittotheBoardin
writingthenameandcurriculumvitaeof amonitoringphysicianforpriorwritten
approvalbytheSecretaryorSupervisingMemberoftheBoard. Inapprovingan
individualto serveinthiscapacity,theSecretaryandSupervisingMemberwillgive
preferencetoaphysicianwhopracticesinthesamelocaleasDr.Leesonandwhois
engagedinthesameorsimilarpracticespecialty.
ThemonitoringphysicianshallmonitorDr. Leesonandhismedicalpractice,and
shallreviewDr. Leeson'spatientcharts. Thechartreviewmaybedoneonarandom
basis,withthefrequencyandnumberof chartsreviewedtobedeterminedbythe
Board.
Further,themonitoringphysicianshallprovidetheBoardwithreportsonthe
monitoring of Dr. Leeson andhis medical practice, andon the reviewof Dr. Leeson's
patientcharts. Dr.LeesonshallensurethatthereportsareforwardedtotheBoardon
aquarterlybasisandarereceivedintheBoard'sofficesnolaterthan theduedatefor
Dr. Leeson'squarterlydeclaration.
Intheeventthatthedesignatedmonitoringphysicianbecomesunableorunwillingto
serveinthiscapacity,Dr.LeesonmustimmediatelysonotifytheBoardinwriting. In
addition,Dr.LeesonshallmakearrangementsacceptabletotheBoardforanother
monitoringphysicianwithinthirty daysafterthepreviouslydesignatedmonitoring
physicianbecomesunableorunwillingtoserve,unlessotherwisedeterminedbythe
Board. Furthermore,Dr.Leesonshallensurethat thepreviouslydesignated
monitoringphysicianalsonotifiestheBoarddirectlyof hisorherinabilitytocontinue
toserveandthereasonstherefore.

TheBoardexpresslyreservestherighttodisapproveanypersonproposedto
serveasDr.Leeson'sdesignatedmonitoringphysician,ortowithdrawapproval
of anypersonpreviouslyapprovedtoserveasDr. Leeson'sdesignated
monitoringphysician,intheeventthattheSecretaryandSupervisingMember
oftheBoarddetenninethatanysuchmonitoringphysicianhasdemonstrateda
lackof cooperationin providinginfonDationtotheBoardorforanyother
reason.
Rehabilitation Program
15. Dr.Leesonshallmaintainparticipationinanalcoholanddrugrehabilitationprogram,
suchasA.A.,N.A.,C.A.,orCaduceus,nolessthan three timesperweek.
Substitutionof anyotherspecificprogrammustreceivepriorBoardapproval.
Dr.Leesonshallsubmitacceptabledocumentaryevidenceof continuingcompliance
withthisprogram,includingsubmissiontotheBoardof meetingattendancelogs,
whichmustbereceivedintheBoard'sofficesnolaterthan theduedateforDr.
Leeson'squarterlydeclarations.
Aftercare
16. Dr.Leesonshallcontactanappropriateimpairedphysicianscommittee,approvedby
theBoard,toarrangeforassistanceinrecoveryoraftercare.
17. Dr.Leesonshallmaintaincontinuedcompliancewiththetennsof theaftercare

Work Hour Limitation
18. Dr. Leesonshall limithisworkhourstonomorethanfortyhoursofworkperweek,
untilotherwiseapprovedbytheBoard. Dr.Leesonshallkeepa logreflectingthe
dates,times,andfacilitiesand/orlocationsatwhichheworks. Dr. Leesonshall
submithisworklogforreceiptintheBoard'sofficesnolaterthantheduedateforDr.
Leeson'squarterlydeclaration.
AnyrequestbyDr.Leesonformodificationoftbelimitationonworkhourssetforth
inthisparagraphshallbeaccompaniedbydocumentationfromaphysicianaffiliated
withaBoardapprovedtreatmentprovider,orotherphysicianapprovedbytheBoard
forthispurpose,whohas evaluatedDr.Leeson,indicatingthatsuchphysician
supportsDr.Leeson'srequestformodification.

Releases
19. Dr. Leesonshallprovideauthorization,throughappropriatewrittenconsentforms, for
disclosureof evaluativereports.summaries,andrecords.of whatevernature.byany
andallpartiesthatprovidetreatmentorevaluationforDr. Leeson'schemical
dependencyorrelatedconditions,orforpurposesofcomplyingwiththisConsent
Agreement,whethersuchtreatmentorevaluationoccurredbeforeorafterthe
effectivedateof thisConsentAgreement. Totheextentpermittedbylaw,theabove-
mentionedevaluativereports,summaries.andrecordsareconsideredmedicalrecords
forpurposesof Section149.43of theOhioRevisedCodeandare confidential
pursuanttostatute. Dr.LeesonfurtheragreestoprovidetheBoardwrittenconsent
permittinganytreatmentproviderfromwhomheobtainstreatmenttonotifythe
Boardintheeventhefailstoagreetoorcomplywithanytreatmentcontractor

RequiredReportingbyLicensee
20. Withinthirty days of the effective dateofthis Consent Agreement,Dr. Leeson shall
provideacopyof thisConsentAgreementtoallemployersorentitieswithwhichhe
isundercontracttoprovidehealthcareservices(includingbutnotlimitedtothird
partypayors)or isreceivingtraining,and theChief of Staff ateach hospitalwherehe
bas privilegesorappointments. Further,Dr. Leesonshallpromptlyprovideacopyof
thisConsentAgreementtoallemployersorentitieswithwhichhecontractsto
providehealthcareservices,or applies fororreceivestraining,andtheChief of Staff
ateachhospitalwhereheappliesfororobtainsprivilegesorappointments. Inthe
eventthatDr.Leesonprovidesanyhealthcareservicesorhealthcaredirectionor
medicaloversighttoanyemergencymedicalservicesorganizationoremergency
medicalservicesprovider,withinthirty days oftheeffective dateof this Consent
AgreementDr.Leesonshallprovideacopyof thisConsentAgreementtotheOhio
Departmentof PublicSafety,Divisionof EmergencyMedicalServices. Further,Dr.

21. Withinthirty days ofthe effective dateof this Consent Agreement, Dr. Leeson shall
providea copyof thisConsentAgreementtotheproperlicensingauthorityof any
stateorjurisdictioninwhichhecurrentlyholdsanyprofessionallicense,aswellas
anyfederalagencyorentity,includingbutnotlimitedtotheDrugEnforcement
Agency,throughwhichhecurrentlyholdsanylicenseorcertificate. Dr. Leeson
furtheragreesto provideacopyof thisConsentAgreementattime ofapplicationto
theproperlicensingauthorityof anystateinwhichheappliesforanyprofessional
licenseorfor reinstatementof anyprofessionallicense. Further,Dr.Leesonshall

23. Dr.LeesonshallnotifYtheBoardinwritingof anychangeofprincipalpractice
addressorresidenceaddresswithinthirtydaysof suchchange.
FAILURETOCOMPLY
If, in thediscretionof theSecretaryandSupervisingMemberoftheBoard,Dr.Leesonappearsto
haveviolatedorbreachedanytermorconditionof thisConsentAgreement,theBoardreserves
therighttoinstituteformaldisciplinaryproceedingsformyandall possibleviolationsor
breaches,including, but not limited to, alleged violationsofthe lawsof Ohio occurring before
theeffectivedateofthisConsentAgreement.

If the Secretary and Supervising Member of the Board detennine that there is clear and
convincing evidence that Dr. Leeson has violated any tenn, condition or limitation ofthis
Consent Agreement, Dr. Leeson agrees that the violation, as alleged, also constitutes clear and
convincing evidence that his continued practice presents a danger of immediate and serious harm
to the public for purposes of initiating a summary suspension pursuant to Section 4731.22(G),
Ohio Revised Code.
DURAT10NIMODIFICATION OF TERMS
Dr. Leeson shall not request termination of this Consent Agreement for a minimum of five years.
In addition, Dr. Leeson shall not request modification to the probationary terms, limitations, and
conditions contained herein for at least one year, except that Dr. Leeson may make such request
with the mutual approval and joint recommendation of the Secretary and Supervising Member.
Otherwise, the above-described terms, limitations and conditions may be amended or tenninated
in writing at any time upon the agreement of both parties.
In the event that the Board initiates future formal proceedings against Dr. Leeson, including but
not limited to issuance of a Notice of Opportunity for Hearing, this Consent Agreement shall
continue in full force and effect until such time that it is superseded by ratification by the Board
of a subsequent Consent Agreement or issuance by the Board of a final Board Order.
In the event that any tenn, limitation, or condition contained in this Consent Agreement is
determined to be invalid by a court of competent jurisdiction, Dr. Leeson and the Board agree
that all other terms, limitations, and conditions contained in this Consent Agreement shall be
unaffected.
ACKNOWLEDGMENTSILIABILITY RELEASE
Dr. Leeson acknowledges that he has had an opportunity to ask questions concerning the tenns of
this Consent Agreement and that all questions asked have been answered in a satisfactory
manner.
Any action initiated by the Board based on alleged violations of this Consent Agreement shall
comply with the Administrative Procedure Act, Chapter 119., Ohio Revised Code.
Dr. Leeson hereby releases the Board, its members, employees, agents, officers and
representatives jointly and severally from any and all liability arising from the within matter.
This Consent Agreement shall be considered a public record as that tenn is used in Section
149.43, Ohio Revised Code. Further, this infonnation may be reported to appropriate
organizations, data banks and governmental bodies. Dr. Leeson acknowledges that his social
security number will be used ifthis information is so reported and agrees to provide his social
security number to the Board for such purposes.

Dr. Leesonentersinto this ConsentAgreementbeingfully informedofhis rights under Chapter
119.,Ohio Revised Code, includingthe right to representationbycounseland the right toaformal
adjudicativehearingonthe issuesconsidered herein.
BASISFOR ACTION
This ConsentAgreementisentered into onthe basis ofthe following stipulations,admissionsand
understandings:
A. The Board isempoweredbySection4731.22(B), Ohio Revised Code, to limit, revoke,
suspendacertificate, refuse to registerorreinstatean applicant, or reprimand orplace on

4731.22(B)(10),Ohio Revised Code, to wit: Deceptionto ObtainaDangerousDrug,
Section2925.22, Ohio RevisedCode, Illegal ProcessingofDrug Documents,Section
2925.23, Ohio Revised Code, and/orTheft, Section2913.02, Ohio Revised Code, as set
forth in ParagraphEbelow. The Board expresslyreservesthe right to instituteformal
proceedingsbaseduponany otherviolationsofChapter4731. ofthe RevisedCode,
whetheroccurringbeforeorafter theeffectivedate ofthis Agreement. Such express
reservationincludes, but isnot limitedto, violationsbased onany methodsused byDr.

D. Dr. Leesonstatesthathe isalso licensedto practice medicineand surgeryinthe
WestVirginia. - U
E. Dr. Leesonadmitsthat he ischemicallydependent,and thathe inappropriatelyobtained
Vicodin for his ownuse. Dr. Leesonfurtheradmitsthat he self-reportedtothe Board
that heentered inpatienttreatmenton or aboutJuly 16,2010, forchemicaldependency
at ShepherdHill,aBoard-approvedtreatmentproviderinNewark,Ohio,and that such
inpatienttreatmentcontinuesto date. Dr. Leesonfurtherstatesthathe has not
previouslyhad any treatment, inpatientor outpatient,for chemicaldependency.
Dr. Leesonadmitsthat beginninginapproximatelythe early2000's,he inappropriately
obtainedVicodinfor self-use. Dr. Leesonfurtheradmitsthat heobtained Vicodinfor
self-useexclusivelythroughthe followingmethods: Dr. Leesonstatesthat inthe
1990'she was appropriatelyprescribedViciodin for pain inconnectionwith
degenerativediskdiseaseand arthritis,and beginninginor around 2000to2004,he
tookVicodin,on afew occasions,that had beenprescribedto a family member; inor
around2007or 2008,he requestedaprescriptionfrom a family member'streating
physicianfor Vicodin,ostensiblyforthe family member, but he tookthat medication
himself;and from approximately2007to2010,Dr. Leeson wroteprescriptionsfor
Vicodinbysigningthe name ofhis treatingphysicianstothe prescriptions,withoutthe
authorityor knowledgeofthe prescribingphysicians. Dr. Leesonspecificallydenies
obtainingany dangerousdrugsor controlledsubstancesfor self-usebyany other
methods,and he statesthat at no time did heeverdivert,sell or give any dangerous
drugsor controlledsubstancestoanyoneelse (otherthanmedicationsthat he
appropriatelyprovidedto patientsfor legitimateand therapeuticpurposesinthe course
ofmedical treatment). Dr. Leesonfurther statesthat he neverdirectlyor indirectly
involvedpatientsinany ofhis effortsto obtaindangerousdrugsor controlled
substancesfor self-use. Dr.Leeson furtheratteststhat heonly tookthe Vicodinwhileat
home,and he neverused itwhileon call or at work.
AGREED CONDITIONS
Wherefore,inconsiderationofthe foregoingand mutual promiseshereinafterset forth, and inlieu
ofany formal proceedingsatthistime, Dr. Leesonknowinglyand voluntarilyagreeswith the Board
to the followingterms,conditionsand limitations:

Suchperiodsofabsenceshall notreducethe probationaryterm, unlessotherwise
determinedby motionofthe Boardfor absencesofthreemonthsorlonger,orby the
Secretaryor theSupervisingMemberofthe Boardfor absencesoflessthanthree
months,in instanceswherethe Boardcanbe assuredthatprobationarymonitoringis
otherwisebeingperformed. Further,the Secretaryand SupervisingMemberof theBoard
shall havethediscretionto grantawaiverofpartor all ofthe monitoringtermsset forth
in thisConsentAgreementfor occasionalperiodsofabsenceoffourteendaysor less. In
the eventthat Dr. Leesonresidesand/orisemployedat a locationthat iswithinfifty
milesofthe geographicborderofOhioand any ofits contiguousstates,Dr. Leesonmay
travel betweenOhio and that contiguousstatewithoutseekingpriorapprovalofthe

Releases; QuarterlyDeclarationsandAppearances
6. Dr. Leesonshallprovideauthorization,through appropriatewrittenconsentforms, for
disclosureof evaluativereports,summaries,andrecords,of whatevernature,byany and
all partiesthatprovidetreatmentor evaluationfor Dr. Leeson'schemicaldependencyor
relatedconditions,or for purposesofcomplyingwiththisConsentAgreement,whether
suchtreatmentor evaluationoccurredbeforeor aftertheeffectivedateofthisConsent
Agreement. To the extentpermittedby law, the above-mentionedevaluativereports,

7. Dr. Leesonshall submitquarterlydeclarationsunderpenaltyofBoard disciplinaryaction
and/orcriminalprosecution,statingwhetherthere has been compliancewith allthe
conditionsofthis ConsentAgreement. The first quarterlydeclarationmust be received
inthe Board'sofficeson the first dayofthe third monthfollowingthe monthinwhich
this ConsentAgreementbecomeseffective,oras otherwiserequestedbythe Board.
Subsequentquarterlydeclarationsmust bereceived inthe Board'sofficesonor before
the first day ofevery third month.
8. Dr. Leesonshall appearinpersonforan interviewbeforethe full Boardor itsdesignated
representativeduringthe third month following the effectivedate ofthis Consent
Agreement. Subsequentpersonalappearancesmust occurevery three monthsthereafter,
and/oras otherwiserequested bythe Board. Ifanappearanceismissedor isrescheduled
forany reason, ensuingappearancesshall bescheduled based on the appearancedate as
originallyscheduled.
Drug & Alcohol Screens;Drug TestingFacilityand CollectionSite
9. Dr. Leesonshall submitto randomurine screeningsfordrugs and alcohol atleast four
timesper month,or as otherwisedirected bythe Board. Dr. Leesonshall ensurethat all
screeningreportsare forwarded directlytothe Board on aquarterlybasis. The drug
testingpanelutilized must beacceptabletothe Secretaryofthe Board,and shall include
Dr. Leeson'sdrug(s)ofchoice.
Dr. Leesonshall abstainfrom the use ofany substanceand the consumptionofpoppy
seeds or anyotherfood or liquid that mayproducealow level positiveresult ina
toxicologyscreen. Dr. Leesonacknowledgesthat heunderstandsthat the consumption
oruse ofsuch substances, includingbut not limitedto substancessuch asmouthwashor
hand cleaninggel, may causeapositivedrug screenthat may not beable to be
differentiatedfrom intentional ingestion, and thereforesuch consumptionoruse is
prohibitedunderthis ConsentAgreement.
All such urinescreeningsfor drugs and alcohol shall beconductedthroughaBoard-
approveddrug testingfacility and collectionsite pursuantto the global contractbetween
said facility and the Board,that providesforthe Board to maintainultimatecontrol over
the urine screeningprocessand to preservethe confidentialityofall positivescreening
results inaccordancewith Section4731.22(F)(5),Ohio RevisedCode,and the screening
processshall requireadaily call-inprocedure. Further, inthe eventthat the Board
exercisesits discretion,as provided inParagraph 10below,to approveurine screenings

Dr. Leeson shall submit,athisexpenseand onthe dayselected, urine specimens.fordBlg:
and/oralcohol analysis. All specimenssubmittedbyDr. Leesonshall be -'
exceptforthose substancesprescribed,administered,or dispensed tohim in 0
conformancewith theterms,conditionsand limitationsset forth inthis Consent
Agreement. Refusalto submitsuch specimen,orfailure to submitsuch specimenonthe
dayhe isselectedor in such mannerasthe Board may request, shall constitutea
violationofthis ConsentAgreement.
Further, withinthirty days of the effectivedate of this ConsentAgreement,Dr.Leeson
shall enter into the necessaryfinancial and/orcontractual arrangementswith the Board-
approveddrug testingfacility and/orcollectionsite inorderto facilitatethe urine
screeningprocessinthe mannerrequired bythis ConsentAgreement. Further, Dr.
Leeson shall promptlyprovidetothe Board writtendocumentationofcompletionofsuch
arrangements,includingacopy ofanycontractenteredinto betweenDr.Leeson and the
Board-approveddrug testingfacility and/orcollectionsite. Dr. Leeson's failure totimely
completesuch arrangements,orfailure totimely providewrittendocumentationtothe
Board ofcompletionofsuch arrangements,shall constituteaviolationofthis Consent
Agreement.
Dr.Leesonshall ensurethat the urine screeningprocess performedthrough theBoard-
approveddrug testing facility and/orcollectionsite requiresadaily call-inprocedure;
that the urine specimensareobtainedonarandom basis; and that the giving ofthe
specimeniswitnessed byareliableperson. Inaddition,Dr. Leeson and the Board-
approved drug testingfacility and collectionsite shall assure that appropriatecontrol
over the specimenismaintainedand shall immediatelyinform the Board ofanypositive
screeningresults.
Dr.Leeson shall ensurethat the Board-approveddrug testingfacility and/orcollection
site providesquarterlyreportstothe Board,inaformat acceptabletothe Board,
verifyingwhetherall urine screenshave beenconducted incompliancewith this Consent
Agreement,and whetherall urine screens have been negative.
Intheeventthat the Board-approveddrug testing facility and/orcollectionsite becomes
unable or unwillingto serve asrequired bythis ConsentAgreement,Dr.Leeson must
immediatelynotify the Board inwriting,and make arrangementsacceptabletothe

Leesonshallsubmithis urine specimensto the Board-approveddrugtestingfacility and
collectionsite chosenbythe Board. However,inthe eventthat utilizingsaid Board-
approveddrugtestingfacilityand/orcollectionsite createsan extraordinaryhardship
uponDr .Leeson,as determinedinthe sole discretionofthe Board,then subjecttothe
followingrequirements,the Boardmay approvean alternatedrugtestingfacilityand/or
collection site, or a supervisingphysician,to facilitatethe urinescreeningprocessfor Dr.
Leeson:
a. Withinthirtydaysofthe date uponwhichDr. Leesonis notifiedofthe Board's
determinationthatutilizingthe Board-approveddrugtestingfacilityand/or
collectionsite constitutesan extraordinaryhardship uponDr. Leeson,heshall
submitto the Board in writingfor its priorapproval the identityofeitheran
alternatedrugtestingfacilityand collectionsite, or the nameofaproposed
supervisingphysician,to whomDr. Leesonshall submitthe required urine
specimens. In approvingafacility, entity,or an individual to serveinthis capacity,
the Boardwill give preferenceto afacility located nearDr. Leeson'sresidenceor
employmentlocation,or toa physicianwho practicesin the samelocaleas Dr.
Leeson. Dr. Leesonshall ensurethat the urine screeningprocessperformed
throughthe alternatedrugtestingfacilityand/orcollectionsite,orthroughthe
supervisingphysician,requiresadaily call-in procedure;that the urine specimens
are obtainedon a random basis ;and that the givingofthe specimeniswitnessed by
areliableperson. In addition, Dr. Leesonacknowledgesthat the alternatedrug
testingfacilityand collectionsite, orthe supervisingphysician,shall assurethat
appropriatecontroloverthe specimenismaintainedand shall immediatelyinform
the Boardofany positivescreeningresults.
b. Dr. Leesonshall ensurethat the alternatedrug testingfacilityand/orcollectionsite,
or the supervisingphysician,providesquarterlyreportsto the Board,ina format
acceptableto the Board,verifyingwhetherall urinescreenshavebeenconductedin
compliancewiththis ConsentAgreement,and whetherall urine screenshave been
negative.
c. Inthe eventthatthe designatedalternatedrugtestingfacilityand/orcollectionsite,
orthe supervisingphysician, becomesunableor unwillingto so serve,Dr. Leeson
mustimmediatelynotifythe Boardinwriting. Dr. Leesonshall furtherensurethat
the previouslydesignatedalternatedrug testingfacility and collectionsite, orthe
supervisingphysician,also notifiesthe Boarddirectlyofthe inabilitytocontinueto
serveand the reasonstherefore. Further, inorderto ensurethat therewill be no

approvalofanyentity,facility orperson previouslyapprovedto soserve inthe
eventthat the Secretaryand SupervisingMemberofthe Board determinethatany
such entity, facility orperson hasdemonstratedalack ofcooperationinproviding
informationtothe Board orforanyotherreason.
11. All screeningreports required underthis ConsentAgreementfrom theBoard-approved
drug testingfacility and/orcollectionsite,orfrom thealternatedrug testing facility
and/orcollectionsite orsupervisingphysician,must bereceived inthe Board'soffices
nolater than the due date for Dr. Leeson'squarterlydeclaration. It isDr.Leeson's
responsibilitytoensurethat reports aretimely submitted.
12. The Board retainsthe right to require, and Dr.Leesonagrees to submit,blood, urine,
breath, salivaand/orhair specimensforscreeningfordrugs and alcohol, foranalysis of
therapeuticlevels ofmedicationsthat maybeprescribedforDr. Leeson, orforanyother
purpose,atDr. Leeson'sexpenseupon the Board'srequestand withoutprior notice. Dr.
Leeson'srefusal to submitaspecimenupon request ofthe Board shall result ina
minimumofone year ofactual license suspension. Further,the collectionofsuch
specimensshall bewitnessed byarepresentativeofthe Board,oranotherperson
acceptabletothe SecretaryorSupervisingMemberofthe Board.
RehabilitationProgram
13. Withinthirty days ofthe effectivedate ofthis ConsentAgreement, Dr. Leeson shall
undertakeand maintainparticipationinanalcohol and drug rehabilitationprogram, such
asA.A., N.A., C.A.,orCaduceus,nolessthan three timesper week. Substitutionofany
otherspecificprogrammust receive prior Board approval.
Dr.Leesonshall submitacceptabledocumentaryevidenceofcontinuingcompliancewith
this program,includingsubmissiontothe Board ofmeetingattendancelogs, which must
be received inthe Board'sofficesnolaterthan the due date forDr. Leeson'squarterly
declarations.
14. Immediatelyupon completionofanyrequired treatmentforchemicaldependency, Dr.
Leeson shall enterinto anaftercarecontractwith aBoard-approvedtreatmentprovider
and shall maintaincontinued compliancewith theterms ofsaid aftercarecontract,

providedthat, wheretheterms oftheaftercarecontractconflictwith theterms ofthis
ConsentAgreement,theterms ofthisConsentAgreementshall control.
CONDITIONS FOR REINSTATEMENT

practiceincompliancewithacceptableandprevailingstandardsofcareunderthe
provisionsofhis certificate. Suchdemonstrationshall include butshall notbe
limited tothe following:
1. Certificationfrom atreatmentproviderapproved under Section 4731.25of
theRevisedCode that Dr.Leeson hassuccessfullycompletedanyrequired
inpatienttreatment,including atleasttwenty-eightdaysofinpatient or
residential treatmentforchemical abuse/dependence,assetforth inRules
4731-16-02and 4731-16-08,Ohio AdministrativeCode, completed
consecutively.
11. Evidenceofcontinuingfullcompliancewith, orsuccessful completionof,a
post-dischargeaftercarecontractwith atreatmentproviderapprovedunder
Section4731.25 ofthe Revised Code. Suchevidenceshall include, butnot
belimitedto,acopyofthesigned aftercarecontract. Theaftercare contract
must complywith rule4731-16-10oftheAdministrativeCode.
111. Evidenceofcontinuingfullcompliancewith this ConsentAgreement.
IV. Two writtenreports indicating thatDr. Leeson'sability topractice hasbeen
assessedand that hehasbeen found capable ofpracticingaccording to
acceptableand prevailingstandardsofcare. The reports shall bemadeby
physiciansknowledgeableintheareaofaddictionologyand whoareeither
affiliatedwith acurrentBoard-approvedtreatmentproviderorotherwise have
been approved inadvance bytheBoard toprovideanassessmentofDr.
Leeson. Further,thetwo aforementionedphysiciansshall not beaffiliated
with the same treatmentproviderormedical group practice. Priortothe
assessments,Dr.Leeson shall provide theevaluatorswith copies ofpatient
records from anyevaluationsand/ortreatmentthat hehas received,anda
copy ofthis ConsentAgreement. The reports from theevaluatorsshall
includeanyrecommendationsfortreatment,monitoring,orsupervisionof Dr.
Leeson,and anyconditions,restrictions,orlimitationsthatshould beimposed

on Dr.Leeson'spractice. The reports shall also describethe basis forthe
evaluator'sdeterminations.
All reportsrequiredpursuanttothis paragraphshall bebased upon ~
examinationsoccurringwithin the three monthsimmediatelyprecedingany
applicationfor reinstatement. Further, atthe discretionofthe Secretaryand
SupervisingMemberofthe Board, the Board mayrequestan updated ~
assessmentand report ifthe Secretary and SupervisingMemberdetermine
that such updated assessmentand report iswarrantedfor anyreason.

v. Inthe eventthat the Board initiates future formal proceedingsagainst Qi;.
Leeson, including but not limited to issuanceof a Noticeof Opportunity'for
Hearing, Dr. Leeson shall beineligiblefor reinstatementuntil such
proceedingsare fully resolved byratification bythe Board ofasubsequent
ConsentAgreementor issuancebythe Board ofafinal Board Order.
c. Dr. Leesonshall enter intoawritten consentagreementincludingprobationary
terms, conditionsand limitationsasdetermined bythe Board within 180days of
the date upon which all theabove-specifiedconditionsfor reinstatementor
restorationhave been completedor, ifthe Board and Dr.Leeson are unable toagree
on the termsofawrittenConsentAgreement,then Dr.Leeson further agrees to
abide byany terms, conditionsand limitationsimposedbyBoard Order aftera
hearingconductedpursuantto Chapter 119.ofthe Ohio RevisedCode. TheBoard
shall providenoticeto Dr.Leeson that said hearing has been scheduled,advising
Dr. Leesonof his hearing rights, and stating the date, time, and location of the
hearingatwhich the Board willpresentitsevidence,after whichthe Board will
make adeterminationofthe matter byBoard Order.
Further,upon reinstatementofDr.Leeson'scertificatetopracticemedicineand
surgeryinthis state, the Board shall require continuedmonitoringwhich shall
include,butnot be limitedto,compliancewith the writtenconsentagreement
entered into before reinstatementorwith conditionsimposed byBoard Order after
ahearingconductedpursuanttoChapter 119.ofthe Revised Code. Moreover,
upon terminationoftheconsentagreementorBoard Order,Dr.Leeson shall
submittothe Board forat least twoyearsannual progressreportsmade under
penaltyofBoard disciplinaryaction orcriminal prosecutionstatingwhetherDr.
Leeson has maintainedsobriety.
16. Inthe eventthat Dr. Leesonhas not beenengagedintheactivepracticeofmedicineand
surgery for a period inexcessoftwoyears prior toapplicationfor reinstatement,the
Board mayexercise itsdiscretion under Section4731.222,Ohio RevisedCode, to
requireadditionalevidenceofDr.Leeson'sfitness to resume practice.

providesanyhealthcare servicesorhealth care directionor medical oversighttoany
emergencymedicalservicesorganizationoremergencymedical servicesprovider,within
thirty days ofthe effectivedate ofthis ConsentAgreementDr. Leeson shall providea
copy ofthis ConsentAgreementtothe Ohio DepartmentofPublicSafety, Divisionof
EmergencyMedical Services. Further,Dr. Leeson shall providethe Board with oneof
the followingdocumentsas proofofeach required notificationwithin thirty daysofthe
date ofeach such notification: (I) thereturn receipt of certified mail within thirty days
ofreceivingthat return receipt,(2)anacknowledgementof delivery bearing theoriginal
inksignatureofthe persontowhomacopy ofthe ConsentAgreementwas hand
delivered,(3)the original facsimile-generated report confirmingsuccessfultransmission
ofacopy ofthe ConsentAgreementtothe person orentity towhomacopy ofthe
ConsentAgreementwas faxed, or(4)anoriginalcomputer-generatedprintoutof
electronicmail communicationdocumentingtheemail transmissionofacopy ofthe
ConsentAgreementtothe person orentity towhom acopy ofthe ConsentAgreement
was emailed.
18. Withinthirty days ofthe effectivedate ofthis ConsentAgreement,Dr. Leeson shall
provideacopy ofthis ConsentAgreementtothe properlicensingauthorityofanystate
orjurisdictioninwhich hecurrentlyholds anyprofessional license,aswell asanyfederal
agency orentity, includingbutnot limited tothe Drug EnforcementAgency, through
which hecurrentlyholds any license orcertificate. Dr. Leeson further agrees toprovide
acopy ofthis ConsentAgreementattime ofapplicationtotheproperlicensingauthority
ofanystate inwhichheappliesforanyprofessional license or reinstatementofany
professional license. Further,Dr.Leeson shall providethe Board with oneofthe
followingdocumentsasproofofeach required notificationwithinthirty daysofthedate
ofeach suchnotification: (l)the return receipt ofcertifiedmail withinthirty daysof
receivingthat return receipt, (2) anacknowledgementofdelivery bearing the original ink
signatureofthe persontowhom acopy ofthe ConsentAgreementwas handdelivered,
(3)the originalfacsimile-generated report confirmingsuccessful transmissionofacopy
ofthe ConsentAgreementtothe person orentity towhomacopy ofthe Consent
Agreementwas faxed, or(4)anoriginal computer-generatedprintoutofelectronicmail
communicationdocumentingthe email transmissionofacopy ofthe ConsentAgreement
tothe personorentity towhom acopy ofthe ConsentAgreementwas emailed.

19. Dr. Leeson shall promptlyprovideacopyofthis ConsentAgreementtoall and r ,
entities that provideDr. Leeson chemicaldependencytreatmentormonitoring. Further,
Dr. Leeson shall providethe Board with one ofthe followingdocumentsasproof.<if

20. Dr.Leeson shall notify the Board inwriting ofany changeofprincipal practiceaddress
orresidenceaddresswithinthirtydays ofsuch change.
DURATIONIMODIFICATION OF TERMS
The above-describedterms,conditionsand limitationsmaybeamended orterminated inwriting at
anytime upon theagreementofboth parties. Intheevent that the Board initiatesfuture formal
proceedingsagainstDr. Leeson,includingbutnot limited to issuanceofaNoticeofOpportunityfor
Hearing, this ConsentAgreementshall continueinfull force and effect until such time that itis
superseded byratificationbythe Board ofasubsequentConsentAgreementor issuancebythe
Board ofafinal Board Order.
Intheevent thatany term, limitation,orconditioncontainedinthis ConsentAgreementis
determinedto beinvalidbyacourtofcompetentjurisdiction,Dr.Leesonand the Board agree that
all other terms, limitations,and conditionscontainedinthis ConsentAgreementshall beunaffected.
FAILURE TO COMPLY
If,inthediscretionofthe Secretaryand SupervisingMemberofthe Board, Dr. Leeson appears to
have violated orbreachedany term orconditionofthis ConsentAgreement, theBoard reserves the
righttoinstituteformal disciplinaryproceedingsforanyand allpossibleviolationsorbreaches,
includingbutnot limitedto, allegedviolationsofthe lawsofOhio occurringbefore theeffective
date ofthis ConsentAgreement.
ACKNOWLEDGMENTSILIABILITY RELEASE
Dr. Leeson acknowledgesthat hehas hadanopportunitytoaskquestionsconcerningtheterms of
this ConsentAgreementand that all questionsasked have been answered inasatisfactorymanner.
Anyaction initiated bythe Board based onalleged violationsofthis ConsentAgreementshall
comply with the AdministrativeProcedureAct,Chapter 119.,Ohio RevisedCode.
