ADMISSION PROCESS (i) After the declaration of official result of the Entrance Test, the admission process for public sector medical and dental colleges will start in October, 2013. All those candidates having domicile of Punjab or Federal Capital Area (Islamabad) who will score 80 per cent or above aggregate marks (excluding Hifz-e-Quran marks) as per PMDC formula, shall be eligible to apply for admission on Open Merit basis in Public Sector Medical / Dental Colleges of the Punjab. As per PMDC Regulation, the aggregate marks will be calculated by adding the marks of a candidate as follows: Matriculation or equivalent HSSC / FSc or equivalent UHS Entrance Test 2013 10% 40% 50%

Then the formula for calculation of aggregate marks is as follows: X / A x 1100 x 0.10 + Y x 0.40 + Z x 0.50 = Aggregate Marks Example: Marks obtained in Matriculation Marks obtained in HSSC/FSc Marks obtained in Entrance Test

742 / 850 944 / 1100 840 / 1100

By applying the aforementioned formula: 742 / 850 x 1100 x 0.10 + 944 x 0.40 + 840 x 0.50 Aggregate Marks = 96.02 + 377.6 + 420=893.62 Aggregate Percentage=893.62 x 100 / 1100=81.2381 Note: Final calculation in percentage will be rounded up to four decimal points.

The aforementioned condition of 80% or above marks is not applicable to candidates seeking admission against any category of reserved seats (Disabled Students, Under-Developed Districts, Cholistan) who can apply if they fulfill basic eligibility criteria as laid down in the Prospectus and also available on UHS website www.uhs.edu.pk. There are 09 seats available on Reciprocal basis for Punjab domiciled candidates in the medical colleges of Balochistan, KPK and Azad Jammu & Kashmir. Candidates applying against open merit seats can exercise their option for these seats as well. No separate application will be invited for these seats. The Admission Kit, containing Prospectus and Admission Form will be available, free of cost, at UHS Lahore, Rawalpindi Medical College Rawalpindi and Nishtar Medical College Multan, as per given schedule. The candidates will have to provide the attested copies of their SSC/FSc and Entrance Test result cards and domicile certificate to get the Admission Kit. Duly filled Admission Form, complete in all respect, can be submitted, only by hand, at the same centre from where the Admission Kit was received as per given schedule. Candidates will give their preference for medical and dental colleges in one single Admission Form which once given shall be final and cannot be changed subsequently. An applicant shall not be considered for a college he/she has not named in his/her order of preference. One candidate can only submit one form. There is no need to submit separate forms for MBBS and BDS. Muslim Hafiz-e-Quran will be awarded 20 marks (which will be added in his/her HSSC marks and NOT in his/her aggregate score) provided that he/she passes the Hifiz-e-Quran Test conducted by the University. To appear in that test, those Huffaze-Quran who have an aggregate score of 80% or above marks, will have to get themselves registered for the test at the University as per given schedule. They must bring the original and attested copies of their Matriculation, FSc / HSSC, Entrance Test 2013 result cards, Hifiz-e-Quran Certificate issued by a registered madrassa and Entrance Test Admittance Card at the time of registration and Hifiz-eQuran Test. Candidates must attach the following documents with the Admission Form. The documents attached with the Admission Form must be attested by a Government Officer in BS 17 or above. The stamp of the officer must bear his / her full name, designation and current place of duty: a) Three (03) attested copies of Secondary School (Matriculation) Certificate. Detailed Marks Certificate (Result Card) issued by the Board is also acceptable. However, computer generated result cards will not be acceptable. Candidates having foreign qualifications must provide the attested copies of Equivalence Certificate issued by IBCC. If date of birth is not mentioned on Secondary School (Matriculation) Certificate or Equivalence Certificate, the candidate will have to attach three (03) attested copies of his/her Birth Certificate issued by NADRA. In case of any discrepancy in the date of birth mentioned on various documents, the date of birth mentioned on the matriculation certificate or equivalence certificate will be considered correct. Three (03) attested copies of Higher Secondary School (FSc Pre-Medical) Certificate issued by the Board. Candidates having foreign qualifications must provide the attested copies of Equivalence Certificate issued by IBCC. Valid Provisional Equivalence Certificate issued by IBCC will also be acceptable. Three (03) attested copies of Domicile Certificate of the candidate. No other certificate or document (e.g. Birth Certificate, B-Form, CNIC etc.) is acceptable in lieu of Domicile Certificate of the candidate. Any candidate found to have domicile of more than one place shall be disqualified. Three (03) attested copies of Character Certificate from the institution last attended. Three (03) attested copies of CNIC of Father / Guardian. Four (04) recent photographs. One to be pasted on the Admission Form and to be attested on the front, and other three, attested on the back, to be attached with the form. Your photograph must be: Colour; Passport size (4.5 cm high x 3.5 cm wide); A recent and true likeness, showing full face, with no hat, helmet or sunglasses, although you can wear everyday glasses; Original- that is not digitally or optically altered ( for example it must not be photoshopped or touched-up ); Taken against a light blue background. One photograph to be pasted on the form and attested on the front and other three, attested on the back, to be attached with the form after putting them in a small envelope. Original Fitness Certificate issued by a registered medical practitioner / Government Medical Officer. Original Affidavit on Rs.20/- stamp paper as per specimen given as Annexure-I of the Prospectus. Candidates claiming to be Muslim Huffaz-e-Quran shall attach three (03) attested copies of Hifz-e-Quran Certificate issued by a registered Madrassa.

Candidates claiming to be disabled students shall attach the copy of a Certificate issued by a specialist working in a Government Hospital describing the nature of disability. Candidates seeking admission against seats reserved for UnderDeveloped Districts shall attach Original Certificates from the Heads / Principals of the concerned educational institutions where they have studied from class one to twelve. These certificates must be dully verified by concerned EDO (Education) and DEO (Colleges) and countersigned by the DCO of relevant district. These candidates will also submit a Surety Bond on Rs.20/- stamp paper as per specimen given as Annexure-II of the Prospectus. Candidates seeking admission against the seat reserved for Cholistan shall have to provide a certificate issued by Cholistan Development Authority clearly stating that the candidate is a Cholistani by birth and has actual residence in Cholistan.

IMPORTANT 1. No other document (e.g., Birth Certificate, B-Form, etc.) shall be acceptable in lieu of the domicile certificate of the candidate. Any candidate found to have domicile of more than one place shall be disqualified. Receipts of applications of domiciles shall not be accepted at this stage. Any candidate found to have submitted forged certificates or fake document at any stage of admission shall be permanently debarred from admission to any medical/dental college. Admission Forms once submitted shall not be returned. Incomplete and unsigned forms will be rejected. Applications / Forms received through post shall not be entertained. No Application / Form shall be entertained after due date for submission. Revision of result or improvement of marks by any Board after the submission of Admission Form shall not affect the merit list of admissions for the current year in any way. In other words, revised marks certificate shall not be acceptable once the candidate has submitted his/her Admission Form.
