Fok Sex / Age : F / 60 Date of admission: 24/06/2013 Summary of Health Assessment using Gordon functional Health Pattern: Health History : Reason for admission: CVA (Cerebrovascular Accident) for investigation Medical Diagnosis : Right ischemic stroke Occupation : Retired civil servant Past health History : Good Past Health History Health perception & Health management: Miss Fok believed that health equals to having no sickness. She seldom had any sickness before admission for stoke rehabilitation; hence she would give herself 10 out of 10 marks. However, she only gave herself 5 out of 10 marks after stoke. She did not have any long-term sickness. When she caught a minor sickness, she would go to consult doctors; she would also take Chinese medicine to aid recovery. Miss Fok did not have body check regularly, but she would maintain her health buy choosing a balanced diet and exercise regularly. Nutrition & Metabolism Miss Fok had 3 meals per day regularly. She had a balanced diet of enough dietary fibres, protein and carbohydrates. She also drunk adequate fluid per day (4-5 cups of 500ml fluid).She had a great appetite even thought she was admitted to hospital. She did not have sudden change of body weight over the past 3 months. Activity & Exercise Miss Fok would go on a walk every day before she is admitted to the hospital She would also play Hula-hoop at home. She exercised for half an hour every day. She did not feel dizzy and out of breathe during exercise. She can care for herself before stoke and she was a housewife. She would cook for her family and take care of her grandchildren. Sleep & Rest Miss Fok slept at 8pm and woke up at 6 am, hence she had adequate sleep of 10

hours. She did not need to take a nap during the day. She felt well rested and refreshed after sleeping. She did not trouble sleeping and did not need the aid of sleeping pills. She seldom needed to wake up to urinate. Elimination She urinated three to four times per day. The colour of the urine is normal and she did not feel any discomfort during urination. She defecated every day. The colour and texture of the stool is normal. She did not have difficulties to defecate. Cognitive & Perception Miss Fok had astigmatism. She did not have problem for the sense of hearing. She felt that half of her body was numb since she had cerebrovascular accident. The sense of touch had deteriorated since stoke. Miss Fok admitted that her memory was not as good as before, for example, she would forget that she want to take a cup to drink water after going to the kitchen. Self-Concept Miss Fok thought that her personality is quite optimistic .She said her merit was being happy and bringing people happiness. Her flaw was not being patient and got angry easily. She thought that she always kept her mood light and happy. To concluded, she said she was an easy going person. Roles & Relationships Miss Fok lived in Ma On Shan with her husband, her daughter, son-in-law and grandson. They got along quite well. She had 4 children, 2 sons and 2 daughters. Before admission, she was responsible of cooking for the family and taking care of the housework. After the admission, Miss Foks daughter took care of her responsibility of caring for the family. Stress & Coping Miss Fok felt depressed and stressed because of her illness. She used to cry when her emotions were overwhelming. She had a bad temper and always argued with her family at the beginning when she had stroke. After a while, she learnt to control her temper and talk to her husband whenever she had difficulties. Values & Beliefs Miss Fok believed in Japanese Buddhism. She thought that her religion after her views on things and made her more optimistic. She thought that the most important things in life is health and family. Sexuality & Reproduction Miss Fok thought that being a female is superior as female could give birth while male couldnt .Hence, female is appreciable. Miss Fok had 4 children and 5 grandchildren. She used to have regular gynecological examination in healthcare centre. She also acquired basic information of gynecology such as cervical cancer

through media. The major area identified for focused assessment After a full interview, one major area for focused assessment was stress and coping. Miss Fok encountered a lot of stress due to her sickness, she did not talk to her sons and daughters about her worries as she was afraid that it might trouble them. She only shared her worries with her husband. Miss Fok should talk with her children instead of hiding her worries in order to communicate better with them.
