II. INTRODUCTION First of all before we got to PNPGH much that we visited another hospital such AFPMC and PMHA because we got AFPMC we consulted another hospital and it was the PNGH. We even hesitated to go further because we seem to be far less well and we carried money for fare mercy of god and also the money we settled at first so we do not yet included because they are worn away and we walked the high away by noon because we felt impressed we saving save. Until we board the bus to Camp Crame in PNPGH we dropped to gate two we walked under the sun its so ht we burn it. Thinking both of us if where the hospital at when we approached we saw immediately the hospital and were going inside we immediately go to admin we wasted no time and finally we received. And when we begun we nervous were divided us and we alternate to MRS/OPD Records and no deficit was sorted properly we start our job.

OBSERVATIONS IN THE COMPANY What I have observed at Philippine National Police General Hospital First is that staff must always in their control they talking to the patient who arguing with them. Dont tolerate/forget any papers, ID, patient Index card that patient indicate their Hospital no./records on hospital. The strength and challenges of this company is that they Requiring more than usually expected or through due, especially being physically or mentally strong, great patience, effort an skills. I think the best practice of this company is Systematic Training by multiple repetitions. An idea into action and for being professional on work/task you can carry out and engage in. My over-all observation in (PNPGH) is attentive all the time be on yourself and be professionalism good attitude to patient most specially to the priority of the hospital. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: Were indexing the index of the outpatient. Interview to the patient (what and where they checked) encoding the index of the patient which indicate hospital #. What we do weekly are the following: Indexing Interviewing patient What we do monthly are the following: Indexing and encoding patient records and also were interview all out-patient in OPD records if where or what they will be checked. During my whole training in Philippine National Police General Hospital (PNPGH), I think the problem/s that I have encountered are: 1. Misplaced charts 2. Loose sheets 3. When doctor hold the patients records 4. When patients take their own charts at home 5. Wrong terminal number upon update 6. Wrong stapled chart 7. Late submission of charts 8. When patient forget their own terminal number tend to say that they are not yet registered.

Indexing interviewing the outpatient in OPD recording in log book before we give the records in every clinic

Indexing the index cards of the patient making personalize IDs interviewing the patient and all concerns round in different clinic .

Like on the other week I do IDs and interviewing the patient encoding indexing the records.

Indexing, encoding interviewing were write into the log book before we give the records of the patient into the nurse clinic depending on where they consulted.

The skills I learned is how to talk formally into the patient whether its a PNP personnel, Retires Personnel or a civilians and the charts must also in a good stapled. Skills and Current Technology Learned/Enforced Encoding- enhance typing skill and familiarized in proper use of keyboard. Printing-learned and familiarized in proper use of printer.
B. Equipment, tools, testing apparatus handled (software's used included)

Injection, Microsoft Windows Equipment, Machinery, Testing Apparatus, etc.Handled Printer--learned how to operate and use properly Scanner- learned how to operate and use properly Xerox machine-learned how to operate and use properly Telephone learned how to respect awfully. Paper Cutter- learned some easy technique on how to use this.
C. Strong points versus weak points

Strong Points Versus Weak Points Base on On-JobTraining program the strong point fight weak point when the determination on a trainee begins so that all task given will work will finish but in a times of problem or unexpected task to be work on it might come the weak point for worried to fail and being not prepared

Best Experience on the Job The best experience OnJob-Training program is when we first interviewing the patient. Also when I gave their records and the Customize IDs. Last is communicating the employees and payers.
