SCHOLARSHIP GUIDELINES 1. The following are the state universities and colleges where scholarships shall be offered: 1. Mariano Marcos State University Batac 2. Mariano Marcos State University Bacnotan 3. Pangasinan State University 4. Cagayan State University 5. Isabela State University 6. Ramon Magsaysay Technological University 7. Bulacan State University 8. Nueva Ecija University of Science and Technology 9. Polytechnic University of the Philippines 10. Cavite State University 11. Laguna State University 12. University of Rizal System 13. Marinduque State University 14. Romblon State University 15. Antique State Uniersity 16. Guimaras State University 17. West Visayas State University 18. Western Visayas College of Science and Technology 19. Carlos Ilado Memorial State College 20. Southern Leyte State University 21. University of Eastern Philippines 22. Visayas State University 23. Mindanao University of Science and Technology 24. Cotabato City State Polytechnic College 25. Caraga State University 26. Surigao State College and Technology 27. Benguet State University The applicant must be: a natural-born Filipino citizen; a less fortunate, talented and deserving student who is a dependent of a family whose head is receiving the minimum wage monthly or below; and of good moral character and in good health. The applicant must either be a DIWA member herself/himself or a dependent of the family of a DIWA Party-list member or DIWA Coalition member. The applicant will be required to present or submit the following as proof together with the application: DIWA member DIWA ID or Certification of Membership from the DIWA Secretary-General; For Education Scholarships in the Menu of Priority Programs/Projects under the Priority Development Assistance Fund (PDAF) in the General Appropriations Act (GAA) of 2010 Implementing Agencies: TESDA/CHED/SUCs/LGUs/NCIP These guidelines are issued in the spirit of transparency and efficiency and to ensure that the truly deserving shall be able to avail of the benefits of the scholarship program.

Dependent of DIWA member Photocopy of DIWA ID of family member or Certification of Membership from the DIWA Secretary-General Dependent of DIWA Coalition member Photocopy of DIWA Coalition ID of family member, Certification of Membership and Letter of Endorsement from the head of the coalition member or the President of DIWA.

The applicant shall duly accomplish the DIWA Scholarship Application Form and submit the same to the Office of Rep. Emmeline Y. Aglipay together with the following: Copy of Birth Certificate; Copy of latest Income Tax Return (ITR) of head of family; Proof required in No. 2; For High School graduates, copy of diploma, transcript of records, and NSAT results; For College students, latest copy of transcript of records and proof of enrolment in the previous semester/school year; The applicant shall be screened on the basis of compliance with the above-mentioned requirements by a duly authorized staff member of the Office of Rep. Emmeline Y. Aglipay. If necessary, an interview of the applicant shall be conducted.

In the screening of applicants, preference shall be given to families without a college graduate or student. 7. Applicants must likewise pass or comply with the admission or eligibility requirements of the university or college that they are applying to.
