Suspected Heart Valve Infection Suspected Meningitis Markedly Elevated (>12,000) or Depressed (<3,000) ( ) White Blood C Cell Count Unexplained High or Low Blood Pressure
Examples
DRG 871 & 872 (Sepsis) $6,500-$12,000 DRG 864 (Fever unknown origin) $4,500-$5,500 DRG 690 (Urinary tract infection) $4,000-$5,000 DRG 795 (Pneumonia) $4,000-$5,000
Afebrile sepsis with false negative blood culture costs THOUSANDS!
Critical Point
Physician must document sepsis in discharge summary for hospital to receive sepsis diagnosis. Concurrent review DRG coders review charts daily for reimbursable diagnoses and work with physicians on d documentation. t ti Mi Microbiology bi l l lab bi intervention t ti not t needed. Retrospective review DRG coders do not receive chart until discharge. DRG coders must contact physician to rewrite discharge summary if sepsis not acknowledged Very Problematic
Easy Fix
When blood culture turns positive for significant pathogen immediately photocopy and give to coder. Coder then goes to floor, checks chart for proper documentation. documentation If necessary, coder works with physician to improve documentation prior to discharge. Keep financial stats, score points with CEO/CFO for quantitative good deeds.
Buggy Bingos
(Retrospective Review Hospital)
Staph. aureus
USUAL SOURCES: Serious wound or abscess. COMPLICATIONS: Exoenzymes destroy vital organ tissues and organ function (Purple, People Eater). SUPPORT MEASURES: Drain or excise abscess or wound.
Strep. pneumoniae
USUAL SOURCES: Community or hospital q p pneumonia acquired COMPLICATIONS: Very resistant to WBC phagocytosis, making it very invasive. Most common cause of non-trauma adult meningitis. SUPPORT MEASURES: Ceftriaxone to prevent meningitis, with or without azithromycin.

SIGNIFICANT POSITIVES
Contact Physician Directly
Bacteremia has high mortality, a fact often not appreciated by nurses and office personnel. Physician may increase dosage and/or add second drug for synergy. Physician may address source of infection, alert nurses to possible complications, and/or institute other support measures.

Versa Trek
More sensitive than Bact/Alert standard bottle Liquid only media Inactivation of antibiotics by dilution is main selling point No split aliquot head to head clinical studies vs Bactec resin or Bact/Alert FAN
Three metropolitan NYC hospitals consolidate lab services to core lab. act/ e t std bottle bott e and a d ESP S Used Bact/Alert preconsolidation. Switched to Bactec resin post consolidation. With same number of blood cultures drawn, 40% increased sensitivity and $1,700,000 extra reimbursements.
