Summary of the Lesson Plan: Students will learn a song about their name, address and phone number. They will create a model of their home and put their address and phone number on the model. A. Target Population: Grade Level: 1st grade Skill Level: All levels Grouping: Whole grouplearning and singing of song Individualcreate a model of their house with their address and phone number B. Materials: Letter home to parents about students home address and phone number. Song about name, address and phone number. Construction paper Glue Crayons Pre-cut shapes C. Objectives: o NV State Social Studies Standards o G6.1.4 Recall home address and phone number. Student-Friendly Standards I can say my home address and phone number.

D. Procedure: 1. Send home a paper asking for each students home address and phone number to help the students recall or memorize. 2. Teach the students the name, address and phone number song. 3. Have the students sing the song several times putting in their own name and address and phone from information provided from parents. 4. Have the students create a house out of pre-cut shapes (triangle, squares, rectangles) 5. Have the students write their address and phone number on the street portion of their model of their own home. 6. Sing the sing a few more times pointing to different students when it comes to the name, address and phone number part of the song.

7. CLOSURE: Today, we learned a song about our address and phone number. E. Assessment: What will you use to measure student understanding? The student will sing their address and phone number. Explain how you will know students understand the concepts from the lesson. I will assess each student by listening to them sing our new song about their address and phone number. I will also assess if they know their address and phone number by checking the model of their home with address and phone displayed on it. F. Reflection: 1. Which part of the lesson do you think will be the easiest for you to teach? I think that the easiest part of this lesson to teach will be putting the students model home together. Students love to create and do hands on projects. 2. Which part will be most challenging for you to teach? I think that the hardest part to this lesson will be teaching the students the song about their address and phone number. 3. How will you follow up or extend this lesson? We will review our address and phone number song in the upcoming weeks. 4. What will you do for students who dont grasp the concepts? I will review the song with individual students that do not understand the concept. 5. Which part of the lesson, if any, do you think might need to change? If the students are not getting the song by me singing it to them, I might need to find a version on line that has the music with it. 6. When you were writing this lesson plan, what was the most difficult part? I thought that finding a song about both the students address and phone number was a little hard. Many songs have just the address or just the phone number but I wanted one that had both.
