welcome email to Mom Emailed mom about several missing assignments and how they are impacting [Student]s grade in math. multiple missing assignments in both ELA and math text mom to let her know that missing assignments were coming home today text mom that work that was sent home last week was still not returned and that [Student] is still struggling to follow class procedures and routines [Student]s behavior has been much better the last two days

Sent home extra copies and gave mom the titles of the pages.

None, [Student] brought them all back the next day.

[Student] would not follow directions or work on assignment; needed huge number of redirections and timeouts to get him on-task and to stop disrupting others [Student] had a lot of trouble in class today-"shouldering" two students at the beginning of class and arguing that he had done nothing; refusing to stay seated when asked; turning from his desk in the corner to shout at students to stop bothering him [Student] said his mom wanted to know how his grade is in Social Studies

Communicated better behavior to Mom; sent [Student] home with packet of missing work called Mom during class and had her speak to [Student]

Create and get [Student] started on routine checklist folder N/A

[Student] was calling out in class, could not be redirected, given warning that his family would be called [Student] had repeatedly called out and argued about his behavior, was not using his time in Homework Connections [Student] had gone through the series of behavior interventions and continued to talk out. He was given a final warning while in line, and continued to argue and complain. Since he had already been warned the day before, his behavior was deemed referralworthy I chose [Student] as my student of the week when he chose to complete his work in Homework Connections rather than take thank you letters down to the Main Office
