The time starts when our company signs you in on the log, time ends when our movers are done with everything that you wish for them to do and you sign them out on the log. Travel time from the beginning address to ending address is included in the time. Our company has a minimum of 2 hours. If the next hour starts you are responsible for the full payment of the hour unless Sherri from our company indicates otherwise. Call for the rates and to see if we are having any specials. Please note: We are only responsible for the workers that Jays Special Moving Service hires. We are not responsible for anybody outside of Jays Special Moving Service. You can hire extra help if you want but our company is not responsible for their actions. Note: If our company is late due to things that are out of our control such as a client going over on the move before yours, the truck breaks down, or a mover gets hurt. If your move goes over the hours you have already paid. You are still responsible for the extra over time charges. However our company will discount the total price by 10%.

Please note: That our hourly rates include 2 guys and 16ft truck, if you want extra men it will be $25 an hour more. If you want a 24ft truck it will be $100 on top of the hourly rate, which is from $90.00 to $130.00 an hour. Clients that receive deals through another company outside of Jays Special Moving Service will only receive a 16ft truck if you want a 24ft truck it will be a $100.00 extra. Note: If a 24ft Truck shows up at your residents and you order a 16ft Truck, this might be due to that another client ordering a 24ft truck before or after you. However if you fill up the 24ft truck you will be responsible for the payment of a 24ft truck. That payment will be due before truck is unloaded at your new residents. The movers will let you know where youre cut off is on the 24ft truck for a 16ft truck. If you have any questions please call our office.

What is the mileage between the start and end point of the job? The mileage starts from our zip (30088) and ends at your drop of zip. Example: 30088 to pickup zip code to drop off zip code; there is a fuel surcharge for anything over 25 miles from our zip. If you
would like to know what the charge is please call our office, someone will be there to help you.

Note: If you say you have no stairs and our company gets to either location and finds out there are stairs you will be charged for each step at $2 a step. If you let us know a head of time you will only be charged a flat fee of $15.

Note: If you say you have no elevator and our company gets to either location and finds out there is an elevator you will be charged for each trip up and down at $5 a trip. If you let us know a head of time you will only charged a flat fee of $2 per floor. For example: Say you live on the 16 th floor of a building. You will be charged $2 times 16 floors = $32 this will be a flat fee. You will not have to pay per trip up and down.

There are charges for damage coverage, blankets, and wrap. Please pick one that you wish to have on your move. Keep in mind if you do not use our supplies and damage coverage, our company is not responsible for any possible damages done to your furniture or belongings. If you do not order insurance coverage, blankets, and wrap our company is only responsible if we drop an item. Our company will not be responsible for scratches, cuts, or dings on items, floors, or walls unless you order damage coverage, blankets and wrap. If you want more coverage please see page 6 of this contract. Our Company can provide covers for the mattresses, however you will have to buy them, if you do not cover your mattresses, our company is not responsible for any dirt damages done during the move. As followed are prices for King, Queen, Full, and Twin size mattresses. King/Queen = $8.95_____ Full = 7.45______ Twin = 6.95________ Our company will make extra stops or trips; however there will be a charge that will apply to our quote. Price for extra stop or trip $25.00. Do you plan on making an extra stop or trip in between the beginning and end addresses? ________. If you say no on the contract, our movers will not stop anywhere until they get to the ending address. If you do want to stop somewhere else you will have to pay the charge.

Cancellation Policy You have 48 hours before your move to cancel, if you do not cancel within the 48 hours your card will be charged $60.00.

If Jays Special Moving Service does not receive full payment the truck will not be unloaded until paid in full. Groupon Clients please keep in mind when you bought the deal it only includes 2 men and a 16ft truck. Any other cost such as an extra man, a bigger truck, mileage, blankets, and wrap you will have to pay to our company. Groupon Clients, You must email or fax our company a copy of the Voucher that Groupon sends you to your email address without the Voucher your move will not be booked and if it is on the books it will be taken off until our company receives the Voucher.

MOVING ORDER & CONTRACT


Our company will be doing an Evaluation at the beginning and end of the move. Please allow time for this before and after our move. It will not be done on your time.
TERMS & CONDITIONS OF CONTRACT 1. THIS IS THE MOVING CONTRACT - It covers only the articles and services listed. 2. LIABILITY OF COMPANY - We are responsible only for our own negligence. We assume no liability of any kind of loss or damage to goods caused by moth or other infestation, rust, deterioration, an Act of God, an act of governmental agency or public enemy, driveways that cannot support the weight of our trucks, or other causes beyond our control. We are not responsible for damage or breakage to items made of pressed wood. Our company is only untitled to pay up to $500.00 in damages per move for your belongings, if you pay the fee. If you choose not to pay the fee youre not covered by insurance. 3. FRAGILE ARTICLE - We are responsible for damage to fragile articles only when our employees pack, move, and unpack them. 4. CONTENTS OF CONTAINERS - We are not responsible for the contents of drawers, containers or other items of similar nature. 5. APPLIANCES - We are not responsible for the electrical and/or mechanical function of pianos, computers, radios, television sets, phonographs, clocks, refrigerators, washers, and dryers or other instruments or appliances, Whether or not our employees pack them. We recommend major appliances be serviced by a qualified service company. Read this carefully. 7. WEAR AND TEAR - Ordinary wear and tear in handling is not our responsibility. 8. DELIVERY - We will make every reasonable effort to complete delivery. We are not responsible if physical conditions or other special circumstances prevent completion. If we cannot deliver the goods in the ordinary way by stairs or elevator - there will be an extra charge for hoisting, lowering, shuffling, or other labor or equipment necessary. We will charge our current hourly rate for waiting time caused by lack of sufficient elevator service or other causes beyond our control. If no authorized person is present to accept goods at the agreed time, or if orders are incomplete, we will deliver the goods at your risk and expense, using reasonable judgment. 9. TERMS OF PAYMENT - All charges due upon receipt. Driver will collect before final delivery of goods at destination. If, for any reason, this matter is referred for collection, customer is responsible for all costs, including but not limited to agency fees, attorney fees or forty percent of the amount of our claim, and court costs. 10. DELIVERY FROM STORAGE - There will be additional charges for moving from storage and final delivery. All charges strictly certified funds prior to delivery. 11. CLAIMS - All claims must be made in writing within five days after delivery and accompanied by paid receipt of all charges due company. We have the right to inspect and repair allegedly damaged items. 12. AGREEMENT - If you agree with these terms, this shall become a contract for services at the rates stated and will represent the entire agreement of the parties hereto. It shall apply to all additional services rendered by the company for the owner. Only an officer of the company, owner or partner has power to modify the terms and conditions of this contract, and then only in writing. We shall not be bound by any other promise or representation. 13. TERMS - CASH: 1 1/2% per month charges on balances over 30 days.

The movers will show up: Your time starts The movers will walk through your home asking and looking at what you want done. If they see anything damaged, they are to write it down on the evaluation form and take a picture. When theyre done walking your home, you will have to sign the evaluation form and log in sheet. While doing the walk through make sure you tell the movers everything you want blanket and wrapped. Keep in mind if you do not use our blankets and wrap you will be responsible for any possible damages to your furniture and belongings. The movers will start moving your furniture and belongings, starting with the bigger furniture such as beds, dressers, and etc The Truck is loaded Driving to your new home At your new home, let the movers know where you want the furniture and belongings. THE MOVE IS DONE!!!! YOURE TIME STOPS (This is not apply to clients that order extra stops or trips. The clients that order extra stops or trip your time will stop when we are done with your move.) Then the movers will call the office, let the office know that the move is over, and then someone from the office will call you for payment. If any claim needs to are filed on damages done by our movers. You have 5 days from the date of your move to file them, after the 6th day no claims will be granted. This is something that does not happen with our company but as we all know accidents happen; however, our company does not have many of them. But by law we have to let you know what our company policy is for claims. Thank you for your business,

If you do not choose addition insurance coverage, our company is only entitled to pay up to $500.00 in damages per move for your belongings. This is how claims are handled. You have 5 days to return the claim form after the 6th day no claim will be reviewed. First = call or email the company to report the claim Second = you will receive a claim form by Email Third = you complete the form and return it to the company with pictures Fourth = Sherri will file the claim with the Insurance company Fifth = Sherri will call the right company to come out and take a look at what is damaged Sixth = the company that comes out to your house will call Sherri with a report plus email her an invoice of the damages. Seventh = the owner has 30 to 90 days to make a decision on what will be done with this claim. (However it most of the time it is up to the insurance company of what they are going to do about the claim.) Eighth = Once the decision is made, Sherri will call the client and let them know what will be taken place.

Well this is how claims will be handled, a lot of the time it does not take 90 days because our company tries to get it handled within 30 to 60 days but sometimes its out of our control.
