Perjured Witness for Complaint / Incident # 08-26565 & 08-2-38102-1 and do

HEREBY recognize and acknowledge the receipt of Notification of Perjured Witness and make the
official declaration to withdraw and terminate my complaint involving the DV Restraining Order.

I, John Doe (Male Spouse) SAKHI DOB: 06-03-66 of above-entitled Marital Community and
Perjured Witness for Complaint / Incident # 08-26565 & 08-2-38102-1 and do

HEREBY recognize and acknowledge the receipt of Notification of Perjured Witness and make the
official declaration to withdraw and terminate my complaint involving outstanding allegations related to
the above-entitled Incident # 08-26565 & 08-2-38102-1 inclusive of the quashing & nullification of
any investigations originating, causing suspicion from or in relation to the above-entitled incident against
and extend my sincere apologies to the following:

Mrs. Rachel SAKHI, the City of Renton, City of Seattle, Renton Police Department, and the King
County Sheriff’s Department and also hereby release the following Officers: Olmsted, Ryan E,
Hurley, William M, Peterson, Bruce and other representatives on duty involved in said incident
Are released from the liabilities, allegations of complaints, claims of injury inclusive of any and all
alleged damages or actions which allegedly arose from an alleged incident and complaint of
ASSAULT, FOURTH DEGREE, Complaint/ Incident # 08-26565 & 08-2-38102-1 and the
complaint filed on October 31, 2008. A copy of the original report is inclusive of this Declaration of
