II. Nursing Health History A. BIOGRAPHIC DATA Patient MGM is a 30 year old single, female.

She was born Filipino citizen on February 9, 1982 and is affiliated to Roman Catholic. She is presently residing at Madayegdeg, San Fernando City, La Union. She finished her college at AMA Dagupan. B. SOCIO-ECONOMIC

Patient MGM has eight siblings and she is presently residing in a townhouse at Madayegdeg, San Fernando City, La Union together with her one brother. Her father was a retired policeman and her mother was a plain housewife. She worked as a domestic helper in Hong Kong in the year 2010 and went back here in the Philippines last July 2012. Now she is working as a caretaker at the house of her sister who is presently working abroad.

C. REASON FOR SEEKING HEALTH CARE Patient MGM had an onset of intermittent fever with more than 13 bouts of vomiting and complains for dizziness and diarrhea.

HISTORY OF PRESENT ILLNESS One day prior to admission patient MGM had an onset of intermittent fever associated with more than 13 bouts of vomiting. Patient also had an onset of dizziness and diarrhea is noted. According to her she did not ate any unusual food aside from her usual diet. Patient sought consult with her medical doctor and was advised for blood check. Patient was then advised for admission for further evaluation and management.

E. PAST HEALTH HISTORY She was born via normal spontaneous delivery without complications. She received her complete dose of all immunizations and she has no known allergies to foods and drugs. According to her this was her first time to be confined in a hospital. She takes over the counter drugs for fever or for colds but never tried herbal medicines as an alternative.

F. FAMILY HEALTH HISTORY There are no known hereditary diseases such as: heart diseases, hypertension and diabetes mellitus. The family usually experience most common diseases such as: fever, common colds, head ache and cough.

G. LIFE STYLE AND HEALTH PRACTICES 1. Description of a typical day Patient MGMs day begins by waking up in the morning at around 7am, eating breakfast and doing some simple household chores. She do her daily exercise too every morning. During her past time, she surfs the internet and play computer games. She doesnt do a lot of strenuous activities because according to her they have a housemaid so she usually spend her day sitting or watching TV shows but she cooks for their food also whenever she would want to.

Nutrition Patient MGM eats 3 meals and 2 snacks in a day. Usually she likes eating a meal of rice, meat and vegetable. She is not very particular with her diet. She eats whatever she wants. She is not very fond of eating junk foods but she likes candies and chocolates and drinks either soda or juice.

Activity Level and Exercise As she wakes up in the morning what she does is to help in simple house hold chores.

She also has her daily exercise through jogging around their barangay.

Sleep and Rest Patient MGM usually have her siesta in the afternoon and sleeps at around 9 or 10 pm.

She takes her rest by sitting or watching TV shows.

Health Care Patterns Whenever patient MGM gets sick, her first action is to observe the symptoms; she will take a good rest and will take OTC drugs for fever or for colds but if she thinks her condition is getting worse then thats the time that she will seek for medical assistance.

DEVELOPMENTAL TASK In line with Erick Ericksons Psychosocial Theory, Patient MGM is under the stage which she will most probably experience crises or problems regarding Generativity vs. Stagnation. In this stage, Patient MGM must have already established a good working relationship with others; had already figured out what she is going to do for a living and must have thoughts about settling down already but it seems that patient MGM is not yet certain about these things. Patient MGM may have developed a feeling of stagnation or dissatisfaction with the relative lack of productivity.
