NOTES FROM THE 2012 CANADIAN HEALTH POLICY ASSEMBLY


Introduction
The second annual Canadian Health Policy Assembly (HPA) was held in Banff Alberta from November 8-10, 2012. The Assembly provided a forum for leaders in health care from across Canada to meet to discuss strategic issues facing the health care system, with a focus this year on chronic disease. The primary objectives of this Assembly were to bring together a small group of key opinion leaders; to share knowledge and opinions; to widen personal and professional networks; and to enable participants to return home better equipped to tackle the issue at hand in their own community.

Overview
The effective prevention and management of chronic diseases is a topic of growing interest in Canada and internationally as these conditions account for an increasing burden on the healthcare system; both in terms of prevalence and cost. The HPA debated chronic disease management and prevention in Canada in three different perspectives: 1. Prevention and management of chronic disease 2. The roles of eHealth, mHealth, informatics and social media in managing chronic disease 3. Treating the patient and not just the symptoms

Action Items
After two full-days of discussion participants identified a number of actions that were needed to better tackle chronic disease. Over-arching Build community capacity to prevent and manage chronic disease through the continuum of care Educate and empower the public Advocate a coordinated, national initiative for redesign of our health information structure Design and develop a streamlined structure and governance model for a pan-Canadian approach to data, standards and quality Patient perspective/engagement Advocate that every citizen is linked to a 24/7 primary health team where every team member accepts their professional duty to be responsible for the patient journey Develop a strategy that moves us toward patient, family and caregiver skill development, education and capacity building; intended to build health literacy and informed decision making thus shared power Collate and publish an inventory of best practices of patient engagement mechanisms and lessons learned Prevention and management Create a powerful description of an exciting future state that includes a power shift to primary health care and the community Encourage deep public engagement by creating a groundswell and demand for this future state Expand our understanding of implementation science and how it can effectively be applied to the management of chronic conditions Convene a meeting of leadership institutes Increase and protect investment in prevention eHealth, mHealth and social media Attach health records to individuals Help individuals be informed, responsible, aware and ready to participate in self-management Page 2 of 2

Next Steps
Continue this as an annual Assembly, if possible; Between Assemblies, continue the dialogue by all means possible amongst as many stakeholders as possible; Network, where possible, with other like-minded initiatives.
