1. PURPOSE :

1.1 To establish standard for the use of abbreviations and symbols in the patients medical records,
to eliminate the use of prohibited abbreviations when documenting in any records or any forms or in prescribing medications to reduce medications errors.

2. DEFINITION :

2.1 Accepted abbreviations and symbols list, is approved and published by the Medical Record
Review Committee and Pharmaceutical and Therapeutic Committee, to establish standard in documentation and prescribing medications to avoid misinterpretations and error.

3. POLICY: 3.1 Buraydah Central Hospital prepares a list of accepted abbreviations to be used and it is re commended and approved by related committee such as the Pharmaceutical and Therapeutic Committee and Medical Records Review Committee.

4. PROCEDURES: 4.1 The Pharmaceutical and Therapeutic Committee and the Medical Records Review Committee approves and publishes a list of accepted abbreviations in medical records documentation and be approved by Hospital Medical Committee. 4.2 The list of Accepted Abbreviations is distributed to all areas that receive Physicians Order. 4.3 Abbreviations, Symbols, Acronyms or Initials are not acceptable in the final diagnosis, on consent forms, or in any information given to patients,families such as educational materials. 4.4 Abbreviations with more than one definition may be when the caregiver is reasonably able to identify the purpose, when the purpose is not clear, the abbreviations should be spelled out. 4.5 The use of prohibited abbreviations will be monitored and reported through Medical Record Committee and Pharmacy and Therapeutic Committee.
