This survey as a part of research on Patient Safety culture in Kamineni Institute of Medical Sciences (KIMS), Narketpally asks your valuable opinions about patient safety issues, medical error, and event reporting in the hospital and will take about 10 to 15 minutes to complete. An event is defined as any type of error, mistake, incident, accident, or deviation, regardless of whether or not it results in patient harm. Patient safety is defined as the avoidance and prevention of patient injuries or adverse events resulting from the processes of health care delivery. Nurses opinion is well received in this clinical area. In this clinical area, it is difficult to speak up if I perceive a problem with patient care. Decision-making in this clinical area utilizes input from relevant personnel. The physicians and nurses here work together as a wellcoordinated team. Disagreements in this clinical area are resolved appropriately (i.e., not who is right, but what is best for the patient). I am frequently unable to express disagreement with the attending /staff physicians here. It is easy for personnel here to ask questions when there is something that they do not understand. I have the support I need from other personnel to care for patients. I know the first and last names of all the personnel I worked with during my last shift. Handing over before the start of a shift (i.e., to plan for possible contingencies) is important for patient safety. Handing over is common in this clinical area. I am satisfied with the quality of Support and cooperation that I experience with staff physicians in this clinical area. I am satisfied with the quality of Support and cooperation that I experience with nurses in this clinical area. The levels of staffing in this clinical area are sufficient to handle the number of patients. I would feel safe being treated here as a patient. I am encouraged by my colleagues to report any patient safety concerns I may have. Personnel frequently disregard rules or guidelines (e.g., hand-washing, treatment protocols/clinical pathways, sterile field, etc.) that are established for this clinical area. The culture in this clinical area makes it easy to learn from the errors of others. Medical errors are handled appropriately here. I know the proper channels to direct questions regarding patient safety in this clinical area. In this clinical area, it is difficult to discuss errors. Hospital management does not knowingly compromise the safety of patients. This institution is doing more for patient safety now, than it did one year ago. Leadership is driving us to be a safety-centred institution. My suggestions about safety would be acted upon if I expressed them to management. When a mistake is made, but is caught and corrected before affecting the patient, how often is this reported? When a mistake is made, but has no potential to harm the patient, how often is this reported? When a mistake is made that could harm the patient, but does not, how often is this reported?
