Bronx Health REACH is Making Health Equality a Reality By Changing Systems and Changing Communities. This type of project needs a lot of support from decision-makers. What factors might influence them? Think about decision-makers such as legislators, public policy personnel, leaders of community-based organizations, school boards, funding agencies, etc. 3. There are many organizations that collaborate with the Bronx health REACH project which help in its success. One of these organizations is the New York Faith and Justice; http://www.nyfaithjustice.org. This group is made up of churches, organizations, and individuals committed to ending poverty in New York through education and direct advocacy. New York Faith and Justices Legacy Project Pressing for Food Justice to Press Against Diabetes aimed to educate and activate the Black and Latino communities from diverse faiths in Harlem and the South Bronx regarding food disparity, and to collaborate on advocacy efforts to address food deserts in New York City through local government policies. Another organization that has teamed up with Bronx health REACH project is the Staten Island Family Health Care Coalition; http://www.projecthospitality.org/. The Staten Island Family Health Care Coalition was founded to address the glaring disparities in health care and access to health care on Staten Island. The mission of the coalition is to advocate for changes in health care policy, program and practice in order to fully ensure that all Staten Islanders, regardless of ethnicity, color, country of origin, or ability to pay, have access to quality, affordable health care, are empowered to become educated consumers of health care, and are advocates of healthy lifestyles at home, work, school, and play. Project Hospitality seeks to realize its mission both by advocating for those in need and by establishing a comprehensive continuum of care including food, clothing, health care, mental health, vocational training, and legal assistance. One other organizational asset would be Consultant, Joyce H. Davis Associates. Joyce Higginson Davis is a community leader, activist, organizer and business consultant, providing support in community building, leadership development and organizational change for both major corporations and non-profit organizations.

She specializes in models designed to inspire and empower individuals, teams and organizations to take positive action. Her goals are to provide leadership and build institutions that inspire and empower people to solve their own problems and create healthy, thriving personal lives, teams, partnerships and cultures. Since the inception of Bronx Health REACH, Ms. Davis has led the Faith-based Outreach Initiative, recruiting pastors and health coordinators, and setting an agenda for community change. She is also an active member of the Bronx Health REACH Legal and Regulatory committee. Factors of Influence The Bronx Health REACH Project needs a lot of support from decision makers like legislators, school boards and funding agencies. One factor to consider in influencing them and gain support would be having solid evidence and statistics on what the project main goal is and what it has accomplished so far. Conducting a Health Assessment plan on what the Bronx Health REACH Project has already accomplished and providing that information would create a more solid foundation for decision makers. Another factor would be its future accomplishments and support it would provide for the community as a whole. An example would be the support for schools attendance and grades provided that students have more resources for health and behavioral support. Legislators would gain both support and satisfaction not only for themselves but also through the community that they work for by aiding in what is a growing problem in there jurisdiction. Giving them a stronger chance of staying in office another term. Levels of Influence The Bronx project addresses several of the 5 levels of influence for health-related behaviors. One of these levels that they address would be Institutional or Organizational Factors. The Bronx

faith-based institutions, housing, and social service agencies. The Bronx Health Reach Project not only works with these organizations but also works to help create extended programs and organizations for current communities. Another level of health-related behaviors that is shown would be community base factors. There are 280,000 residents in the REACH target area and 95% are black and Latino, including African-Americans, Caribbean-Americans, and new immigrants from Africa, Central and South America. The Bronx is the poorest urban county in the country. 50% of Bronx households include young children living below the poverty level. The Bronx Health REACH Coalition brings together community residents, educators, healthcare providers, community and faith-based leaders, and businesses to build healthier communities. These diverse partnerships allow the REACH project to develop and implement programs and projects in many areas, including nutrition and fitness, diabetes management, and health disparity. Finally public policy factors are also a big part in the Bronx Health REACH project. The reach program is continually making efforts to change the current policies like nutrition, health and education within the communities areas to better suit the needs of the vast cultural melting pot of the area limits and also extending further outside the REACH projects current focus. This is hard work that requires tackling competing interests, political processes, and complex institutional systems. However, since Bronx Health REACH began its campaign for health equity in 1999, there have been some notable milestones in our policy work, including our successful initiative to eliminate whole milk in New York City public schools and our ongoing efforts to end the segregated specialty care system in New York State through legal and legislative action. Educational Resources

Administering Health Education Programs Bronx Health REACH publishes educational pamphlets, articles, newsletters, and other information for use by community groups, faith-based organizations, schools, businesses, and

individuals. Preventing and managing diabetes is a large focal point for the Bronx Health REACH Project. One of the resources used is an information guide for The Diabetes Care Program at the institute for Family Health; http://www.bronxhealthreach.org/wpcontent/uploads/2011/06/IFH_Diabetes_Brochure_Eng.pdf.pdf . This resource gives needed information to those in the community that are in the focus of diabetic care. Some of the information given are Locations of family practices and health centers What the practices and centers offer Special programs within the center Information on what the Institute for Family Health is all about

This resource gives the community the opportunity to gain valuable information and recourse on the targeted subject of diabetes. Having one specific resource given multiple places to go for help gives an individual options and variety. Giving an outline of what they are about, what they offer and what they do helps give a sense of reassurance towards the decision making process of the individual or group.
