Use this checklist for your convenience. Mountain State University must have the following documents before International Student Services can complete your admission and issue a Form I-20: T Mountain State University application for admission including the application fee of US$50. All applications must be signed and dated. Please make sure to specify your date of birth, gender, and program of study. In the United States dates are written as month/date/year, so please follow this format for your date of birth. T A copy of your high school diploma or other documentation that conrms that you have completed a secondary education. T Ofcial sealed transcripts of your high school (secondary) grades or marks, or attested copies of the originals. T TOEFL or IELTS score report, unless your native language is English. Undergraduate admission requires a minimum score of 173 on the computerized TOEFL, 61 on the Internet-based TOEFL, or 5.5 on the IELTS. Graduate admission requires a minimum score of 75 (composite) for the online TOEFL, with desired subscores of 22 for writing, 15 for speaking, 20 for reading, and 20 for listening; 550 on the paper-based TOEFL; or 6.5 on the IELTS. T A condential signed copy of your sponsors afdavit of support. This documentation conrms sponsorship and nancial support. The affadavit of support must also include: T A notarized statement from your sponsors bank that indicates the funds your sponsor has available to pay your expenses while you study. The estimated expense is $US19,500 per year. Remember that international students who are here on an F-1 visa cannot work off campus without authorization from the Bureau of Citizenship and Immigration. T A copy of your passport or birth certicate. T Campus housing application (Beckley campus students only). All full-time Beckley campus freshman and sophomore students under the age of 21 are required to live on campus unless they reside with a parent or guardian and commute daily to classes, are married, or have children. The housing application is available on the MSU website. T Immunization record (optional). This medical record includes all the vaccinations you received during childhood and as an adult. T A course-by-course evaluation of any post-secondary transcripts from Educational Credential Evaluation (www.ece.org) or World Education Services (www.wes.org). This item is required only if you are applying for a masters degree program or plan to transfer credits from an institution outside the United States. Note: If you are applying for an F-1, F-3, J-1, M-1, or M-3 visa for initial attendance, the U.S. government requires that you pay a SEVIS I-901 fee (generally US$200 as of winter 2010) at least three days before your interview date.
