1 Forwarding letter requesting Member Secretary for Endorsement. (All communication copies). 3 4 Information regarding Nodal officer for MQ.(Name-e-mailID-mobile No) Advertisement for admission (This advertisement shall mention no. of seats available for MQ and NRI quota, starting and last date of issuance and receipt of application forms, complete address and communication details of institute and Date, time and Place of counseling and other details in strict accordance to the guidelines issued time to time for filling up MQ/NRI Seats) i. ii. iv. v 5 6 7 8 Full page of news paper in which advertisement is appeared. Copy of paper cutting duly signed by Head of institute. Certificate of circulation for competent authority certifying circulation of newspaper is more than 50000 copies. Institutes website link showing the adv.of admission on MQ/NRI Quota. Specimen Copy of Application Form Application Forms Issue and Receive register of Application form. Inter-se Merit list &MQ and NRI individual merit list(Fair, Transparent and non-exploitative manner) as per format on website Specimen copy of call letter, copy of speed post/ registered post receipt posting of such call letters shall be at least 7 days in advance of counseling date. 9 Attendance Register of counseling ( attended students should sign the register and absent should mark by competent authority if any ) 9a Photocopy of option form of all students called for counseling. 10 Admitted student list- as per format on website (to be endorsed by ACPC Branch Wise) along with i. Copy of the students application form ii. Copy of B.E/B PHARM Marksheet(whichever applicable for admission criteria)) iii. Copy of GPAT/GATE/CET Mark-sheet/scorercard iv Provisional certificate of BE/Bpharm v Leaving certificate(whichever applicable)

2 Copy of letter addressed to ACPC in regards of surrendering MQ/NRI Seats

iii. Copy of Bill of advertisement.

Addition documents (other than mentioned in 10 In case of NRI:-

i-v) for candidates

admitted on NRI/ NRI sponsored candidates:-( as applicable) 1. Copy of passport of admitted student and copy of passport of his/her mother and/or father proving NRI status. 2. Affidavit of father/mother of NRI candidate undertaking responsibilities of tuition fee, other fees and other responsibilities till completion of course. 3. Certificate of equivalence from Association of Indian University, Delhi , if NRI candidate passed qualified exam from other countries. In case of NRI Sponsored :1. Proof of sponsorers NRI detail. 2. Copy of passport of sponsorer NRI ( sponsorers signature should be clearly visible) 3. Affidavit of sponsorer NRI which indicates relation with student and undertaking responsibilities of tuition fee, other fees and other responsibilities till completion of course. (Signature of affidavit should match with signature in passport). NOTE: Endorsement will be done in accordance with inward at ACPC and endorsed letter will be issued only after submission of query.
