The age must be as accurate as possible. Check immunization card for date of BCG. That will give you an idea of month of birth at least. 3. For pregnant women and mothers of infants, ask about age at marriage and try to calculate current age. 4. For pregnant women, the option for other services received cannot mark delivery assistance (option 1) not yet delivered. The question is related to current pregnancy. 5. For mothers of infants, the option for other services received option 1 includes any trained person doctor, nurse, trained dai). If delivery conducted at home, ask if done by trained person. If not sure, do not select this option. 6. Check both PDS card and job card if available to cross check information.

Iron and Folic acid ( IFA 100 mg) 100 if not anemic; 200 if anemic 3. Eat extra pulses ( dhal, beans, nuts) green leafy vegetables and drink milk and extra water 4. Have small amounts of food at a time; eat 4- 5 times a day 5. Only let a trained person conduct your delivery in an institution preferably. 6. Start breastfeeding your baby within one hour after delivery. Give only breast milk for first six months nothing else. High risk signs Ask for complaints of: a) Giddiness, severe headache, fits b) Bleeding or discharge per vagina c) Breathlessness; feeling weak Check for a) b) c) d) e) Age less than 18 years Weight less than 45 kgs Height less than 1.52 metres Swollen feet Looking very pale

WHAT TO DO a) Fill referral slip and send to PHC medical officer b) Give name to the ANM and request to follow up urgently.

All babies should be breastfed immediately after birth till the age of 6 months No other foods or milk should be given for the first 6 months of life After 6 months, start simple porridge with local cereals. Add a new food once a week. Use safe drinking water at all times Children must wash hands before and after meals. Give children pulses, green leafy vegetables daily. A child needs to eat 4-5 times a day. If available, give milk daily. Get weight checked every month. Send the child to the anganwadi regularly. If unable to go, get the take home ration.
