No. 47 s, 2011 TO DATE SUBJECT : : : Public Elementary and Secondary School Administrators Schools ICT Coordinator March 1, 2011 ICT PHYSICAL INVENTORY AND CEP ANNUAL YEAREND REPORT

1. The ICT Technical Team will conduct physical inventory on the existing ICT equipment used in the school. Schedule of physical inventory will be done simultaneously with the Division Supply Office (refer to Division Memorandum No. 37, s. 2011 re: Schedule of Physical Property Inventory). 2. ICT Coordinator or its representative must be present during the inventory to guide the location of the equipment. A hardcopy of the modernization report, reflecting all the ICT equipment must be readied for checking and validation (see separate sheet for format). Complete records as to the source of funds, date acquired/delivered, and equipments model/type must also be prepared. 3. For the CEP Annual Year-end Report the following are required to be submitted to this office: a. Modernization status Report validated by the ICT Technical Team during the Physical Inventory schedule. b. List of ICT enrolees (regular, special, science and CACEP) c. For schools with CEP collection: Financial Statement, Collection Report, Expenses Report, Copy of Original Official Receipt, Copy of bank account, and copy of board resolution; d. For schools with no CEP collection but still has an outstanding balance from the previous CEP collection, submit the requirements mentioned in letter c. 4. For information and compliance.
