EXPLAIN WHAT IS WRONG WITH THE PATIENTS THAT HAVE THE FOLLOWING DIAGNOSIS ON ADMISSION. Explain as if you were telling someone with no medical knowledge. (2T means secondary to) SOB, HTN, hyperglycemia The patient has shortness of breath, high blood pressure and high blood sugar. UTI, CHF The patient has a urinary tract infection and has congestive heart failure, where the heart is not pumping enough. r/o new CVA The patient is being evaluated for cerebrovascular accident, to see if a stroke has occurred. ASHD The patient has atherosclerotic heart disease where blood is hard to flow to the heart because the vessels to the heart are clogged. CABG x 4 2T CAD The patient had a coronary artery bypass grafting. They had 4 of the heart arteries replaced with stents to restore blood flow to the heart from the blocked arteries. Aortic and mitral insufficiency The patients blood flows from the left ventricle and flows back into the left atrium this damages the mitral valve and enlarges the left atrium and ventricle. Palpitations The patient is experiencing abnormal heartbeats which are to fast or to slow. Clotted AV fistula (think dialysis) The patient has kidney failure and has an artriovenous (AV) fistula connected to a vein in their body. A blood clot has formed in the AV fistula. r/o DIC The patient is being evaluated for Disseminated Intravascular Coagulation which might be a blood clot disorder. 10. MVA, multiple lacs The patient was in a motor vehicle accident and had multiple lacerations, wounds and cuts.
