AFFIDAVIT

I, Ganji Sambasiva Rao, S/o Punna Rao, Hindu, Aged 50 years, R/o D.No: 4-176, Pandaripuram, 5th line, Chilakaluripet, Guntur Dist. A.P. India. Do hereby solemnly affirm and state as follows. That my son namely GANJI MADHUSUDHAN RAO belongs to Hindu Padmasali, which comes under OBC Group, My son is completed M.C.A. And his date of birth is 01-05-1986. I am the father, I am doing private clerk work source of earning to my family is Rs 60,000/-P.A. we belong to Hindu Padmasali, OBC that my son GANJI MADHUSUDHAN RAO had applied for (Hindu Padmasali) OBC certificate which comes under other than Backward Classes before the Tahsildar, Chilakaluripet Mandal and Guntur Dist. So, I hereby declare that as my son GANJI MADHUSUDHAN RAO belongs to Hindu Padmasali Caste which comes under OBC and thereby we will come under other Backward Classes. So, my son GANJI MADHUSUDHAN RAO deserved for any Government jobs under OBC as we belongs to Hindu Padmasali Caste which comes under OBC in other Backward Classes. I further affirm that I do not belong to the creamy layer section of the community. I am swearing to this affidavit that the facts stated above are true and correct to the best of my knowledge, information and belief and nothing is false therein and nothing matter had been concealed there form and if anything id found false in it I will be held responsible to discharge the same.
