Childrens Single Point of Access (CSPOA) Application Information

WHAT IS CHILDRENS SINGLE POINT OF ACCESS (CSPOA)?


Childrens Single Point of Access (CSPOA) links families with services that are provided by St. Lukes Hospital and other community-based mental health programs throughout New York City. CSPOA serves children between the ages of five and 17.9 who suffer from serious emotional disturbance or have behavioral disorders that interfere with functioning in the community. The child may also be at risk for being hospitalized, re-hospitalized, or requiring residential placement. CSPOA service provision is determined by the familys needs, borough of residency and program availability. Community-based mental health programs include: Home and Community-Based Services Waiver (HCBSW) Childrens Case Management Services (ICM, SCM and BCM) Family-Based Therapeutic Intervention (FBTI) Bronx Only Community Residence (CR)

APPLICATION PROCESS
To apply for CSPOA, you need the following: A Current Psychiatric Evaluation for In-Home Services (HCBSW, ICM, SCM, BCM) the evaluation must be 12 months or newer. For Out-of-Home Service (CR), psychiatric evaluations must be three months or newer. A Current Psychosocial Evaluation for In-Home Services (Waiver, BCM), the evaluation must be 12 months or newer. For Out-of-Home Service (CR), psychosocials must be three months or newer. A Physical - within 12 months signed by an MD or nurse practitioner. For CR, physicals must be three months or newer. A Psychological and/or Vineland may be requested by CSPOA if there is a question about IQ. A current IEP or Educational Assessment will be requested if Out-of-Home Services are deemed appropriate for your child. If you have any questions, please call your CSPOA Specialist.
