I, CORA D. AQUINO of legal age, filipino and a resident of Blk 4C Lot 5 B Camella Ridge View 1, Tunasan, Muntinlupa City, after having been duly sworn to in accordance with law hereby freely depose and say that: 1. That I purchased a parcel of land from Cuevas Ville Realty Corporation herein described as One Hundred (100) square meter property located at Cuevas Ville Bo. Molino Municipality of Bacoor, Cavite registered under TCT Number T1150939 and Tax Dec no. 03 015 90783; 2. That one of the requirements for the transfer of the certificate Title is the payment of real property taxes covering both the land and any improvements if there was any; 3. That I hereby certify that my property does not have any improvement of whatever kind and nature and it remained idle up to the time the property was sold;

It is my understanding that for over a year already you have been remiss and neglecting your duty and responsibility to provide financial assistance /material support to my client and your three (3) children despite having the means to provide some to them being an overseas foreign worker. Likewise, you have virtually abandoned your family following my clients discovery of your extramarital relations with another woman as to bring your other woman to live with you at the conjugal home causing great suffering, anguish and despair to your wife Analyn. Please be advised that my client could very much cause to have you lose your job for immorality and put you in jail for the crimes of concubinage and for violation of the provision fo R.A. 9262 ( The Violence Against Women and Children Act) arising from your actions against her and your children. While Ms. Analyn already accepts the loss of her husband, she finds your irresponsibility towards you children as unacceptable. As the father of her three (3) children, you have the duty and responsibility to provide for their support and well-being, failing which you would be made to suffer the consequences of you unjust and illegal act. This shall be my final demand. Kindly contact and make arrangements with the undersigned from hereof regarding the provisions for the financial support of my clients children. to institute the appropriate administrative and criminal charges against you to protect my clients interest. unfair dealings is the Chowking Restaurant in Zapote, Bacoor, Cavite, where their airconditioning unit/s are inoperable for a number of months (November-December 2009) to the detriment and inconvenience of its customers while still maintaining their standard prices for food items. 2. This practice is a case of simple unfairness to the general public who chose to avail of the services of an establishment thinking better service and convenience though on a much higher price only to find out that it is no better than the other ordinary establishments. This situation is not specifically covered by the local ordinances nor by the Local Government Code.

3. To correct this kind of unfair activity, the draft affidavit of undertaking will cover the same thus helping in particular the Office of the Legal Officer on its investigation of cases involving abuse of privileges (which include permits granted) under Section 481(b)(3)(v) of the Local Government Code of 1991. Moreover, the draft also mentioned the mandatory observance of the Six Point STRIKE Program of Municipality of Bacoor to ensure coordinated delivery of basic services of the government. 4. WITHAL, it is recommended that the attached Affidavit of Undertaking be made as a requirement to accompany all issued Mayor Permits, whether newly applied or renewal, on all business or activity with the territorial jurisdiction of Municipality of Bacoor. 5. Recommend approval of para 4 hereof.

1. This pertains to the enforcement of Municipal Tax Ordinance over all business establishment found in the Ammie Virata property covered by of TCT- No. T-264028 (SM Bacoor vicinity) containing an area of 1,618 Sq. Mts. 2. Considering that this property is subject for condemnation (expropriation), and considering further that records of the Office of Mun. Treasurer incate that this property is delinquent in the payment of realty tax since 2008 in the amount of P28,408.58 (as of last quarter of 2009), the establishments found therein should not be given any permits (Business and Mayors permit in particular) for the operation of any business or activity. 3. WITHAL, it is recommended that all establishments found in the said real property under TCT No. T-264028 containing an area of 1,618 SQ MTS, and owned by Ammie Virata, situated in the corner of Aguinaldo Hi-way and Tirona Hi-way (beside SM Bacoor) BE DENIED of any Municipal Permits or Clearance from this Municipality (including but not limited to Business and Mayors Permit) in the coming taxing period (January, 2010). 4.
