Note from Tracey

I am desperately attempting to get evaluations completed for any staff who have an anniversary who are needing an annual evaluation. First, let me say that we all have areas that we can improve upon. ALL of us. I dont see that as a problem unless we are unwilling to take a look and try to make improvements. I see the evaluation process as a time for formal discovery. It is a time to openly discuss the areas of strengths and weakness and make goals for our own improvement. There may be suggestions for improvement and also kudos for what you are already doing well. Enclosed you will find a very generic form for peer evaluation. Your identity will be known only to ME, I would appreciate it if you did not talk to anyone else about who I have asked you to comment on. The information you place on this form may be used to augment the evaluation that is given to this person. It is not used in place of my evaluation, nor does it carry the weight that mine does, but it may be used to offer comments to employees and to support a job well done or areas that need to be improved upon. Any actual examples are always helpful, since most of us can not remember any specific instance when we behaved in a certain way. I need these back to me by __________.
