AFFIDAVIT OF CESSATION OF BUSINESS OPERATIONS

That I, Rainier Sim Rashid Apalisok Valdez, Filipino, of legal age, residing at 87 Ermin Garcia St. Cubao Quezon City, after having been sworn in accordance with law hereby depose and state: 1. That I am the registered owner of Rainier Sim Rashid Carinderia located at No.1 Don Alfonso Egea St., Bgy., Pinagkaisahan, Quezon City.

2. That said establishment commenced to start its business in July 2009, based on the Lease Contract with LESSOR Nely Coralde, effective for one year.

3. That the said establishment renewed and paid its Liquor Regulatory Permit for the year 2008 and 2009. 4. That after one year, the said venture proved not to be profitable, I, the registered owner, decided to close the said establishment.

5. That sometime in August 2009, Rainier Sim Rashid Carinderia has officially ceased its business operations. 6. That the said commercial space, is currently being occupied by a bicycle store, as of November 2009. 7. That due to its cessation of business operations in August 2009, I, the registered owner no longer renewed its Liquor Regulatory Permit.
