1 pm Shot-Gun Start
6 pm Social, Dinner & Awards
Entry fee: $75 per golfer (golf, cart, dinner), loads of on-course events
Format: 4-Person Golf Scramble, 18 Holes
Two Men’s Divisions (Based on handicaps)
One Women’s Division
One Mixed Division

Please print and send in this form. Team Captains may either collect checks for their teammates and send them
in as a group, or forward this email so that teammates can respond individually. Please encourage your friends
to join in the fun.

Golf Tournament - $75 per person, $55 for those with 7-day Forrest Hills season pass

If you do not have an established 18-hole handicap, please provide the average 18-hole score on a Par 71
course. Please be honest, this is a FUN event!
