in Obtaining
Birth and
Death Records
Beginning March 1, 2011,
you will need to show identification when
requesting copies of birth and death certificates
at your local* or state health department.
 3 WAYS TO OBTAIN A

CERTIFIED COPY OF
YOU WILL NEED: A BIRTH OR DEATH
RECORD
* One primary identification document with your name
and photo such as a driver’s license, U.S. military ID card,  In person at your local* or
U.S. passport, work or school ID, etc. state health department

OR  Online at the DHSS website


(Payment with credit card required. This
service is offered by a third-party provider.
* At least two alternate identification documents. Additional fees apply.)
Documents must contain your name and the name  Mail your request
of the company or organization issuing the document . (Application form from DHSS website must
be notarized by an acceptable notary public.
Payment with check or money order required.)
ID requirements also apply to marriage and divorce
