I, INSERT NAME, the undersigned Affiant, do solemnly affirm that I have good
reason to believe, and I do believe, based on information provided by
REFERENCE, a credible person who personally observed said offense, that
ACCUSED, hereinafter called Defendant, in the year of our Lord YEAR, on or
about the DATEth day of MONTH, and before the making and filing of this
Complaint, did criminally ignore laws associated with the duties of HIS/HER
office causing harm to, and in direct violation of, the due process rights
of REFERENCE, within the territorial limits of the city of CITY, in the
county of COUNTY, and the state of Texas, while acting under color of law,
proceeded with court motions and trial proceedings in the municipal court of
COURT absent an attorney for the state in a criminal trial case and in
direct violation of REFERENCE’s right to due process as protected by the
Bill of Rights within the Constitution of the United States and the Texas
Constitution article 5 section 21, and Texas Code of Criminal Procedure
2.01, 2.02, 2.03, 2.07, as well as numerous other state and federal laws,

in so doing Defendant directly violated PENAL CODE Section 39.02 ABUSE OF

OFFICIAL CAPACITY, a Class A misdemeanor offense under subsection (a)(1), a
minimum state jail felony and possibly a felony of the first or second
degree under subsection (a)(2),

against the peace and dignity of the State.
