Dear Mrs. Widlak: Pursuant to your request, | performed the second autopsy of the remains of your late husband, Mr, David Widlak, to identify the cause of death. My autopsy findings revealed that Mr. Widlak died as a result of a contact gunshot wound sustained to the mid-back of his neck.
