Sub: Public Health — Preventive Medicine — SBHI ~ Improvement of
MCCD — Reporting of causes of deaths ~ regarding,
I wish to inform you that the causes of deaths have to be classified and coded as
per 10" revision of ICD Published by the World Health Organization from 1.1.2000
Hence it is necessary that the causes of deaths have to be certified by the
doctors who attended the deceased during his last illness. Tamilnadu state have shown
only 25% of deaths were reported with MCCD during the year 2006, while Government
of India insisting to receive MCCD for all deaths. The forms received by the Birth and
Death Registrars have been sent partially to the Chief Registrar of Births and Deaths by
the concemed authorities for analysis.
For issuing MCCD, form Numbers 4 and 4A arc the prescribed formsio certify
the Institutional and Domiciliary events respectively. AUl the Birth and Death
Registrars under your control should be strictly instructed to obtain MCCD for all the
deaths registered. Person in charge of the graveyard also be instructed to obtain MCCD
form before disposing the body.
